Methodology for targeting physicians for interventions to improve breast cancer screening.

INTRODUCTION: Uneven increases in mammography utilization rates call for methods to efficiently target educational interventions to women who do not regularly use mammography and physicians who do not adhere to national guidelines for breast cancer screening. This paper discusses a method for identifying physicians who are nonadherers to breast cancer screening guidelines or in need of continuing medical education (CME) in this area. METHODS: A 1995 community-based telephone survey of randomly selected women aged 50-80, residing in four Long Island, NY, townships was used to identify women wh

Carbapenem-resistant Escherichia coli harboring Klebsiella pneumoniae carbapenemase beta-lactamases associated with long-term care facilities.

Nine carbapenem-resistant Escherichia coli isolates harboring Klebsiella pneumoniae carbapenemase (KPC)-2 or KPC-3 enzymes were identified in patients residing in 7 distinct long-term care facilities. Cefotaxime-hydrolyzing (CTX-M)-type beta-lactamases were also documented in 3 isolates. The identification of these enzymes in patients staying in long-term care facilities should be of great concern to all components of health care systems.

Evidence for targeting autonomic dysfunction in systemic sclerosis: A scoping review.

Autonomic dysfunction is a common and early complication among patients with systemic sclerosis, suggesting that it may play a role in the pathogenesis of the disease and be a potential target for therapeutic interventions. Although the true prevalence of autonomic dysfunction among patients with systemic sclerosis is still unclear, it is estimated that as many as 80% of patients may be affected. Autonomic dysfunction may lead to widespread multi-organ dysfunction through its effects on the cardiovascular system, gastrointestinal tract, urinary tract, sweat and salivary glands, and pupils. Three unusual mimics of primary angiitis of the central nervous system.

To present three rare mimics of primary angiitis of the central nervous system (PACNS). We describe 3 patients with rare diseases that can mimic PACNS at clinical presentation and neuroimaging. We describe the clinical course of these patients and also present a review of the literature regarding these three diagnoses. All 3 patients presented with neurological symptoms and had abnormal findings on neuroimaging suggestive of PACNS. After detailed history, careful review of systems, thorough laboratory workup and consideration of lack of a response to immunosuppressive therapy, PACNS was ruled

mNGS facilitates the accurate diagnosis and antibiotic treatment of suspicious critical CNS infection in real practice: A retrospective study.

Whether metagenomic next-generation sequencing (mNGS) could benefit patients with suspected severe central nervous system (CNS) infection in terms of diagnosis and antibiotic treatment remains unknown. We retrospectively analyzed 79 patients with suspected CNS infection and undertook mNGS. The value of mNGS was investigated in terms of identification of pathogen and guidance for the adjustment of antibiotic treatment. The relationship between the time of initiating mNGS since onset and the Glasgow Outcome Scale (GOS) score after 90-day follow-up were analyzed. Fifty out of 79 cases with suspic

Controlled anaesthesia: a clinical evaluation of an approach using patient characteristics identified during uptake.

The performance of a system to control the alveolar concentration of halothane in patients undergoing halothane and nitrous oxide or halothane anaesthesia with controlled ventilation has been evaluated. The method involved the identification and quantification of the uptake characteristics of patients from their early response to the anaesthetic and implements the vaporizer control necessary to achieve and maintain a desired alveolar halothane concentration. Initial targets are based on the concept of MAC, but modifications to the desired alveolar concentration may be effected readily by the a

[Recommendations for the diagnosis and treatment of optic neuromyelitis in children and adolescents].

Neuromyelitis Optica spectrum disorder (NMOSD) is a severe autoimmune disease of the central nervous system characterized primarily by inflammation of the optic nerve, spinal cord, and brainstem. The identification of IgG antibodies against aquaporin-4 (AQP4-IgG) has transformed the understanding and classification of the disease, distinguishing it from other inflammatory CNS disorders. However, some seronegative patients remain a subject of debate.

The first imported case of melioidosis in a patient in central China.

Here, we report the first imported case of melioidosis from Laos in central China. COMPACT VITEK2 identification system and PCR, as well as sequencing methods confirmed that the patient was infected by Burkholderia pseudomallei, a bacterial species closely related to an isolate detected in Thailand. These findings are highly valuable for an early diagnosis, treatment and to prevent the spread of this emerging infectious disease in central China.

The epidemiology of prurigo nodularis in Belgium.

Prurigo nodularis (PN) is a rare, severely pruritic chronic skin disease, which seriously reduces patients' quality of life. PN prevalence in Belgium is unknown. To investigate PN prevalence, patient profile, diagnosis, and treatment strategies in Belgium. These insights will better support patient identification and treatments choice to address their specific needs. An anonymous questionnaire was sent to 800 dermatologists for data collection; 65 dermatologists (8%) completed the survey. Also, a search query in the electronic medical records system of a Belgian university hospital was perform

Voltage mapping of Koch's triangle in atrioventricular nodal reentrant tachycardia ablation : Results from an observational multicenter prospective registry.

BACKGROUND: The identification of a "low-voltage bridge" to guide ablation of atrioventricular nodal reentry tachycardia (AVNRT) has been described as a safe and effective strategy in children. We investigated the presence of a low-voltage bridge in adult patients undergoing AVNRT ablation, to evaluate its anatomical correspondence with the successful ablation site. We also investigated the possible correlations between Koch's triangle anatomy and patients' biometric characteristics. METHODS: This observational registry prospectively collected data from 200 patients undergoing AVNRT ablation,

Clinical Impact of Laboratory Implementation of Verigene BC-GN Microarray-Based Assay for Detection of Gram-Negative Bacteria in Positive Blood Cultures.

Gram-negative bacteremia is highly fatal, and hospitalizations due to sepsis have been increasing worldwide. Molecular tests that supplement Gram stain results from positive blood cultures provide specific organism information to potentially guide therapy, but more clinical data on their real-world impact are still needed. We retrospectively reviewed cases of Gram-negative bacteremia in hospitalized patients over a 6-month period before (n = 98) and over a 6-month period after (n = 97) the implementation of a microarray-based early identification and resistance marker detection system (Verigen

Prehospital critical care prediction score for emergency transfusion requirements in major trauma: a multicenter validation study.

BACKGROUND: Early identification of trauma patients requiring emergency transfusion remains a critical challenge in prehospital critical care. Massive hemorrhage accounts for approximately 40% of trauma-related deaths and represents the most significant modifiable prognostic factor. Current assessment methods rely primarily on vital signs and physical examination, which have limited sensitivity for detecting compensated shock states. METHODS: We conducted a prospective, multicenter, derivation-validation study across three Spanish Emergency Medical Services systems from January 2021 through J

Category specificity in normal episodic learning: applications to object recognition and category-specific agnosia.

Studies of patients with category-specific agnosia (CSA) have given rise to multiple theories of object recognition, most of which assume the existence of a stable, abstract semantic memory system. We applied an episodic view of memory to questions raised by CSA in a series of studies examining normal observers' recall of newly learned attributes of familiar objects. Subjects first learned to associate arbitrarily assigned colors or textures to objects in a training phase, and then attempted to report the newly learned attribute of each object in a recall task. Our subjects' pattern of recall

Patient-level performance evaluation of a smartphone-based malaria diagnostic application.

BACKGROUND: Microscopic examination is commonly used for malaria diagnosis in the field. However, the lack of well-trained microscopists in malaria-endemic areas impacted the most by the disease is a severe problem. Besides, the examination process is time-consuming and prone to human error. Automated diagnostic systems based on machine learning offer great potential to overcome these problems. This study aims to evaluate Malaria Screener, a smartphone-based application for malaria diagnosis. METHODS: A total of 190 patients were recruited at two sites in rural areas near Khartoum, Sudan. The

Astrocytes generated from patient induced pluripotent stem cells recapitulate features of Huntington's disease patient cells.

BACKGROUND: Huntington's Disease (HD) is a devastating neurodegenerative disorder that clinically manifests as motor dysfunction, cognitive impairment and psychiatric symptoms. There is currently no cure for this progressive and fatal disorder. The causative mutation of this hereditary disease is a trinucleotide repeat expansion (CAG) in the Huntingtin gene that results in an expanded polyglutamine tract. Multiple mechanisms have been proposed to explain the preferential striatal and cortical degeneration that occurs with HD, including non-cell-autonomous contribution from astrocytes. Although

Clinical Significance of Uncommon, Non-Clinical, and Novel Autoantibodies.

Autoantibodies are a common mark of autoimmune reaction and their identification in the patients' serum, cerebrospinal fluid, or tissues is generally believed to represent diagnostic or prognostic biomarkers of autoimmune diseases or autoinflammatory conditions. Traditionally, autoantibody testing is an important part of the clinical examination of suspected patients, and in the absence of reliable T cell tests, characterization of autoantibody responses might be suitable in finding causes of specific autoimmune responses, their strength, and sometimes commencement of autoimmune disease.
