Evaluating the Safety of a Ketamine Sedation Protocol In a Pediatric Oncology Population in Tanzania: A Quality Improvement Project.

This project aimed to reintroduce and evaluate a pediatric ketamine sedation protocol at a Zonal Referral Center in Mwanza, Tanzania, where nonanesthesia clinicians provide sedation when trained anesthesia personnel are unavailable. The objectives were to assess knowledge retention, confidence, and protocol compliance. The study design used a pre- and postintervention design. Nonanesthesia clinicians at a > 1,000-bed hospital were surveyed to assess their knowledge and confidence about ketamine sedation. Over 10 days, 50 clinicians received training that included lectures and cognitive aid res

Incidence and impact of remimazolam-induced paradoxical reactions in pediatric same-day bidirectional endoscopy: a randomized, controlled clinical trial.

BACKGROUND: Remimazolam, a novel ultrashort-acting benzodiazepine, offers advantages such as fast onset, rapid recovery, organ-independent metabolism, and cardiorespiratory stability. Intravenous (IV) remimazolam administration could induce paradoxical reactions (PRs) in children, including excitement, body movement and fear. The aim of this study was to explore the incidence of PRs and their impact on sedation efficacy when different premedication remimazolam doses were administered before pediatric same-day bidirectional endoscopy. METHODS: This prospective, randomized, controlled clinical

Anesthesia Practices for Preterm Infants: A Survey in the Nordic Countries and Review of the Literature.

BACKGROUND: Preterm infants are at increased risk of perioperative mortality and are among the most vulnerable patients anesthesiologists can encounter. The literature on anesthesia practice in preterm infants is limited, mainly focusing on their physiological immaturity and related pharmacodynamics and pharmacokinetics. AIMS: Our aim was to gather comprehensive data on the current practice of anesthesia management in preterm infants and, when available, to compare these findings with existing literature or guidelines. METHODS: A cross-sectional study was conducted using a structured survey

Application of the Quadratus Lumborum Block in Pediatric Gastrointestinal Surgery: A Scoping Review.

BACKGROUND: Effective pain control is critical in pediatric gastrointestinal surgery to support recovery and improve postoperative outcomes. Regional anesthetic techniques have gained increasing interest, with the quadratus lumborum block (QLB) emerging as a promising option. As its use expands, this scoping review aimed to map the available evidence on QLB in pediatric gastrointestinal surgery. Specifically, it examined its application, gastrointestinal procedures involved, reported outcomes, and potential advantages and disadvantages. METHODS: This scoping review followed PRISMA-ScR guideli

Effect of high-flow nasal oxygen therapy on perioperative hypoxemia in children: a systematic review and meta-analysis.

BACKGROUND: Hypoxia is the leading cause of complications and death during anesthesia. The risk of hypoxemia in children is higher than in adults. The use of high-flow nasal cannula (HFNC) can prevent desaturation of blood oxygen saturation in children. This meta-analysis aimed to compare the effectiveness of HFNC and conventional oxygenation methods in reducing perioperative desaturation in pediatric patients. METHODS: The standard database was searched from the beginning till April 2024. Studies including perioperative use of HFNC had at least one of the following five outcomes: (1) incidenc

Acute pain management in children: challenges and recent improvements.

PURPOSE OF REVIEW: The evidence regarding the efficacy of analgesics available to guide postoperative pain treatment in pediatric patients is limited. Opioid medications are very often an important component of pediatric postoperative pain treatment but have been associated with perioperative complications. We will focus on initiatives aiming to provide effective treatment minimizing the use of opioids and preventing the long-term consequences of pain. RECENT FINDINGS: Interpatient variability in postoperative pain is currently managed by applying protocols or by trial and error, thus often l

Research priorities for paediatric peri-operative medicine identified by Australian children and young people.

INTRODUCTION: Parents and caregivers play a critical role in the care of their child peri-operatively. Our team undertook previous research with parents/carers, which identified Australian parents' top 10 research priorities for paediatric anaesthesia and peri-operative medicine. While this was an important exercise, it focused on parent-reported experiences rather than the priorities of the children themselves; however, the experiences and concerns of parents/carers may not always reflect those of their child. It has been shown previously how integral it is to listen to each child's own voice

Error Traps in Pediatric Adenotonsillectomy: Clinical Patterns, Cognitive Pitfalls, and Evidence-Informed Mitigation Strategies.

Adenotonsillectomy is one of the most common elective pediatric surgeries. However, its routine occurrence can mask substantial preventable risks arising from practitioner inexperience, underappreciated comorbidities, airway proximity to the surgical field, and challenging postoperative pain management. This review highlights common "error traps" that contribute to perioperative adverse events and outlines evidence-based mitigation strategies. Key preoperative challenges include unrecognized moderate-to-severe obstructive sleep apnea (OSA) and recent upper respiratory infections (URIs), both o

Airway management in pediatric tongue flap division for oronasal fistula closure: A case report.

Oronasal fistulae (ONF) could remain after surgery in some patients with cleft palate. ONF ultimately requires intraoral surgery, which may lead to perioperative airway obstruction. Tongue flap surgery is a technique used to repair ONF. During the second surgery for performing tongue flap division, the flap transplanted from the tongue dorsum to the palate of the patient acts as an obstacle to airway management, which poses a great challenge for anesthesiologists. In particular, anesthesiologists may face difficulty in airway evaluation and patient cooperation during general anesthesia for ton

Tympanostomy Tube Placement in Children Using a Single-Pass Tool with Moderate Sedation.

Insertion of tympanostomy tubes is a common elective pediatric surgical procedure and is typically performed under general anesthesia. The potential to reduce general anesthetic requirements for young children has led to increased interest in alternatives for tympanostomy tube placement. A tympanostomy tube system, developed to enable tympanostomy tube placement in a single pass on conscious patients under moderate sedation, was evaluated. A prospective study on 128 children and 253 tympanostomy tube placements conducted at 4 centers in the United States demonstrated an 88.3% success rate in p

Anesthetic management of three pediatric cases with Pena-Shokeir syndrome.

Pena-Shokeir syndrome is a rare, early lethal disease. It is characterized by fetal growth restriction; craniofacial deformities, for example micrognathia and microcephaly; multiple ankyloses; and pulmonary hypoplasia. For patients with this syndrome, maintenance of airway and control of perioperative respiratory complications are important for anesthetic management. We report 3 pediatric cases of Pena-Shokeir syndrome undergoing tracheostomy and arthrolysis under general anesthesia using sevoflurane, nitrous oxide, fentanyl, and vecuronium bromide. Anesthetic procedures including mask ventila

Introduction to interventional radiology's role in palliative care for children with cancer: A COG Diagnostic Imaging Committee/SPR Oncology Committee White Paper.

As palliative care continues to be an area of increasing emphasis in pediatric oncology programs, it is important to raise awareness about minimally invasive, image-guided procedures that can supplement more conventional palliative interventions, such as systemic analgesics, external beam radiation, and locoregional anesthesia. These procedures, when performed for appropriately selected patients, can often facilitate discharge from an inpatient facility, and help patients meet their end-of-life goals. This article specifically discusses three palliative procedures performed by interventional r
