[Laser flare measurement with 3 different nonsteroidal anti-inflammatory drugs after phacoemulsification with posterior chamber lens implantation].

In a prospective, randomised, double-masked, parallel-group study we compared the antiinflammatory effect of diclofenac sodium 0.1%, flurbiprofen 0.03%, and indomethacin 1.0% ophthalmic suspension in 99 patients undergoing phacoemulsification and posterior chamber lens implantation. The reduction in anterior chamber flare from day 1, as measured with the laser flare-meter (FM-500, KOWA) on day 4-5 postoperatively was significantly greater in the diclofenac group than with flurbiprofen (p = 0.022). Patients treated with diclofenac had significantly less burning and stinging than patients on flu…

Tectonic DSAEK with phacoemulsification: a unique approach to manage corneal perforation with advanced cataract in a patient with rheumatoid arthritis.

Management of large corneal perforations in patients with rheumatoid arthritis is often challenging. It is further challenging when associated with advanced cataracts. We describe a case of corneal perforation in a patient with secondary Sjogren's syndrome, which was initially managed with cyanoacrylate glue and amniotic membrane as temporising measure. However, during the follow-up at 8 weeks, he developed advanced cataract in the same eye. He was successfully managed with a tectonic posterior corneal lamellar graft with phacoemulsification and intraocular lens (IOL) implantation. Phacoemulsi…

Four-dimensional ultrasound versus torsional ultrasound phacoemulsification in cataracta nigra: a single-nucleus ex vivo bench pilot.

Cataracta nigra, an extremely dense nuclear cataract, often requires prolonged ultrasound use during phacoemulsification, increasing the risk of corneal endothelial injury and incision-site thermal damage. We compared the Alcon UNITY Cataract System using 4-dimensional (4D) ultrasound with the Alcon CENTURION Vision System with torsional ultrasound in an ex vivo cataracta nigra bench model. A single human cataracta nigra nucleus was divided into four fragments; two fragments were emulsified with UNITY and two with CENTURION under identical user-selected console settings (35% power; IOP 36 mmHg…

Intrastromal misplacement of a foldable IOL during phacoemulsification: Intraoperative OCT identification of type-1 dua's layer separation.

BACKGROUND: Intrastromal misplacement of a foldable intraocular lens (IOL) during cataract surgery is exceedingly rare. Immediate identification of the separation plane is critical to prevent additional corneal injury. CASE REPORT: A 76-year-old woman with bilateral glaucoma underwent routine phacoemulsification. During insertion of a one-piece acrylic IOL, the lens was inadvertently delivered into the corneal stroma. The IOL remained partially unfolded within a stromal pocket. Intraoperative anterior segment optical coherence tomography (AS-OCT; RESCAN 700) clearly demonstrated deep stromal …
