The cellular prion protein (PrPc) is a membrane glycoprotein expressed on many human cells including platelets. We investigated the cellular localization of platelet PrPc. In resting platelets most PrPc was localized inside the cells. The correlation of PrPc and P-selectin surface up-regulation after platelet activation suggested its association with alpha-granules. A postsplenectomy patient had idiopathic thrombocytopenic purpura (ITP) that was exacerbated with both her pregnancies. She failed to have sustained platelet counts in her previous pregnancy in spite of receiving steroids and chemotherapy. She underwent elective cesarean section with a platelet count of 7,000 but did not receive platelet transfusions. No unusual bleeding occurred, and an improvement in the ITP was noted postpartum.

A 57-year-old man with idiopathic thrombocytopenic purpura (ITP) underwent coronary artery bypass grafting and repair of ventricular septal defect. He had not been taken any treatments for ITP for the last two years and platelet count was only 26,000 on his admission. He received 5 days of intravenous high dose immunoglobulin therapy but there was no significant increase of platelet count. He received platelet transfusion during and after the surgery resulting in good postoperative course and no problems of hemostasis.
