Here, we offer a step-by-step description of the technique for a Delorme's mucosectomy and muscular plication in our favoured prone jack-knife position, which is our preferred approach in patients who are frail, and unable to tolerate the pneumoperitoneum required for minimally invasive surgery. An elderly woman with an acute abdomen due to a perforated peptic ulcer is discussed to illustrate the problem of atypical presentation of illness in the elderly. The importance of not dismissing non-specific symptoms and signs such as confusion, restlessness, abdominal distention and non-localising abdominal tenderness in the elderly, is highlighted. In addition, the useful radiological features of pneumoperitoneum are described. The need for functional assessment and rehabilitation are emphasised as important components in the practice of geriatric medicine.

Extensive surgical emphysema can lead to a life-threatening condition causing hemodynamic instability and significant physician challenges in its management. Here we describe an uncommon case of extensive subcutaneous emphysema caused by stabbing neck, which led to complications such as pneumothorax, pneumopericardium, pneumomediastinum as well as pneumoperitoneum. The role of radiological imaging is crucial in managing this relatively uncommon presentation. This article highlights clinical presentation, radiological findings, and various management options.

Two patients have presented postoperative otorrhagia following gynaecologic laparoscopic procedures. This occurred after uneventful anaesthesia and recovery for surgery performed in forced Trendelenburg position (35 degrees with horizontal position). Different responsible mechanisms are discussed including haemodynamic changes induced by both the Trendelenburg position and the pneumoperitoneum. Particularities of external ear blood supply directly submitted to arterial and venous pressure changes, may also have contributed to the appearance of otorrhagia.

The presence of pneumothorax, pneumomediastinum, or cervical subcutaneous emphysema due to perforated duodenal ulcer is a rare presentation. We report a 23-year-man who showed bilateral cervical subcutaneous emphysema, pneumomediastinum, and pneumothorax with no respiratory abnormality. He was found to have active duodenal ulcers, but no detectable pneumoperitoneum or duodenal leak. A sealed perforation from the duodenal ulcers was suspected, and he fully improved after conservative management.

Three preterm infants exposed antenatally to indomethacin developed a characteristic syndrome consisting of edema and hydrops with a bleeding disorder at birth, oliguric renal failure during the first 3 postnatal days, and acute pneumoperitoneum resulting from localized ileal perforation(s) at the end of the first week of life.
