Successful stereotaxic evacuation of an acute pontomedullary hematoma. Case report.

A healthy young woman developed a rapidly progressive pontomedullary lesion 24 hours after delivery of her first child. The lesion was shown on computerized tomography (CT) to be a primary hematoma. Stereotaxic aspiration was carried out, and the patient recovered. Angiography and CT scanning demonstrated a vascular lesion compatible with an arteriovenous malformation.

Wernickes encephalopathy with central pontine myelinolysis presenting with hypothermia.

A male alcoholic who presented with hypothermia and confusion was diagnosed as suffering from Wernicke's encephalopathy. He showed in addition signs of bulbar damage with cranial nerve signs, weakness of all limbs and absent oculo-vestibular responses. His course was complicated by recurrent episodes of aspiration pneumonia with death resulting from this cause. Neuropathological findings included typical features of Wernicke's encephalopathy as well as central pontine myelinolysis.

[Granular neurons of the cochlear complex in rats].

In the cochlear complex of white male rats the dorsal nucleus, the anterior ventral nucleus, the posterior ventral nucleus and the granular layer were studied histologically and morphometrically. While counting the number of different cells in all the above mentioned formations, by means of ocular grid (S.B. Stefanov, 1974) it was revealed that the granular neurons of the cochlear nuclei of the rats form 72--86 per cent of all the cells. The granular cells represent an independent type of neurons and these differ clearly from other neurons by the diameter of their nuclei and by the size of the

Lymphomatoid granulomatosis clinically confined to the CNS. A case report.

A 38-year-old man developed pain and peripheral-type weakness on the right side of his face and was discovered to have decreased hearing bilaterally, as well as optic nerve swelling on the right. The pain and optic nerve swelling subsided over a period of six weeks, but hearing loss and facial weakness persisted. Thirty months later, he developed dysphagia, ataxia, dysarthria, nystagmus, and progressive spastic quadriparesis. He died approximately four years after the onset of the illness. Although no evidence of disease was found other than in the central nervous system during life, two nodul

The basilar pontine gray in the adult monkey (Macaca mulatta): a Golgi study.

In Golgi preparations of the adult monkey (Macaca mulatta) two types of neurons are distinguished in the pontine gray: (1) larger neurons wh … e the equivalent of the large, medium and small neurons that Ramón y Cajal ('09) illustrated in the pons of the 5-day-old infant. Their cell bodies are rounded, polygonal, triangular, egg-, pear-, and spindle-shaped. Some have somatic spines. Usually four to seven dendrites issue from the cell body and as they branch they attentuate. The dendrites have …

Paralytic pontine exotropia in subarachnoid hemorrhage. A clinicopathological correlation.

A syndrome of combined right lateral gaze palsy and right internuclear ophthalmoplegia, followed by a left exotropia in a case of subarachnoid hemorrhage is presented. At necropsy a single small pontine end-zone infarction was found, involving the medial part of the right paramedian pontine reticular formation, the tectospinal tract and the ventral part of the right medial longitudinal fasciculus. The fila radicularia of the right abducens nerve passed through the necrotic area. The mechanism of these characteristic ocular motor disturbances in relation to the post mortem findings and the topo

Dorsolateral pontine lesions produce distinct ocular motor abnormalities with anatomical correlations.

BACKGROUND AND PURPOSE: The dorsolateral portion of the caudal pons contains the vestibular nucleus (VN) and inferior cerebellar peduncle (ICP) that play important roles in conveying and processing vestibular and ocular motor signals. This study aimed to characterize ocular motor abnormalities along with their anatomical correlations in dorsolateral pons (DLP) lesions. METHODS: We analyzed clinical features, and results of neuro-otological evaluations and neuroimaging of 18 patients with unilateral DLP lesions (17 with DLP infarction and 1 with cavernous malformation) from among 506 patients …

Brainstem atrophy is associated with fatigue in myelin oligodendrocyte glycoprotein antibody-associated disease.

BACKGROUND: Fatigue is a common symptom in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), but its underlying mechanisms are not well understood. OBJECTIVE: To investigate MRI volumetric changes associated with fatigue in MOGAD patients. METHODS: Forty-three MOGAD patients and 29 healthy controls (HC) were recruited from January 2020 to December 2022. Brain volumes, including the brainstem and its subregions (midbrain, superior cerebellar peduncle [SCP], pons, and medulla), were analyzed using FreeSurfer v7.1. Volumetric measures were compared between groups, adjuste…

Hypophysitis and central nervous system involvement in association with Sjögren's syndrome along with hypoparathyroidism: a case report.

BACKGROUND: Patients with autoimmune diseases can develop multiple autoimmune diseases over a long period of time, and the presence of more than one autoimmune disease in a single patient is defined as polyautoimmunity. Polyautoimmunity may be clinical evidence that autoimmune diseases share similar immunological mechanisms. CASE PRESENTATION: We report a 30-year-old woman with a unique combination of autoimmune diseases predominantly affecting the central nervous system, with hypoparathyroidism, hypophysitis, medulla involvement, and pons and temporal lobe involvement associated with primary…

Effectiveness of Lee Silverman Voice Treatment (LSVT-LOUD) in Parkinsonian-Type Multiple System Atrophy (MSA-P): A Case Report.

We describe a case of Parkinsonian-type multiple system atrophy (MSA-P) treated with Lee Silverman Voice Treatment (LSVT-LOUD; LSVT Global, Inc., Phoenix, AZ, USA). At age 73, the patient developed motor symptoms, including gait disturbances with a tendency to fall, as well as swallowing difficulties and impaired dexterity in his right hand, prompting a visit to our hospital. Brain magnetic resonance imaging (MRI) revealed atrophy in the cerebellum and brainstem, particularly in the pons, along with enlargement of the fourth ventricle; however, the "cross sign" on the pons was not clearly visi…

The vascular locked-in and locked-in-plus syndrome: A retrospective case series.

The locked-in syndrome (LiS) is defined as the loss of most voluntary muscle movements with preserved cognitive abilities due to a ventral p … ous degrees of impaired consciousness (LiPS). Lesions of LiS patients are round and confined to the pons, whereas lesions of LiPS patients are more elongated and reach neighbouring areas such as the mesencephalon, thalamus or ascending reticular activating system. Lesions involving the mesencephalon and the thalamus are strong indicators of LiPS, where …

Reversible Hearing Impairment Due to Inferior Colliculi Compression by a Pineal Glial Cyst.

Pineal glial cysts associated with bilateral hearing impairment are very rare. Here, we present the case of a 13-year-old boy with a pineal … 40 dB on audiometry. Since the auditory pathway has both crossed and uncrossed fibers at the upper pons and midbrain level, compression at the lateral lemniscus or inferior colliculus level can cause bilateral hearing impairment. In the present case, there was a possible slow pineal cyst growth that eventually compressed the upper pons to the midbrain …
