Making endoscopy mobile: a novel initiative for public healthcare.

Public healthcare has progressed immensely in the past few decades. However, the rural masses in the developing and underdeveloped countries very often have limited access to the advanced healthcare technology. At the Asian Institute of Gastroenterology, Hyderabad, India, the Rural Health Care Project has been initiated in an attempt to provide cost-effective gastrointestinal care to the rural population. The Project was implemented by traveling to remote rural areas in a bus that had been converted into a mobile hospital and equipped with basic diagnostic facilities including a custom-made en

Narcolepsy and cardiovascular health: A big picture perspective.

Narcolepsy is an uncommon, chronic neurologic disorder characterized by pervasive excessive daytime sleepiness (EDS) that can occur with cataplexy (narcolepsy type 1) or without cataplexy (narcolepsy type 2). People with narcolepsy have an increased risk of comorbid psychiatric disorders and primary sleep disorders, as well as increased healthcare utilization compared to the general population. Over the past several years, clinicians and patients in the narcolepsy community have frequently received communications regarding cardiovascular disease (CVD) risk but disproportionately inadequate inf

Lessons from +LOVE, Improving HIV Care for Black Men Who Have Sex with Men in New Orleans Using an Integrated Behavioral Health Crisis Support Model of Care.

Black men who have sex with men (BMSM) experience higher rates of HIV infection than other groups. They also face disparities in linkage and retention to HIV care, as well as in viral suppression. To address the needs of the BMSM population living with HIV, we developed a novel intervention program (+LOVE), which integrated case management with behavioral health/crisis support. The intervention consisted of (1) a dedicated therapist; (2) an SMS (text message)-based engagement program, CareSignal that provided medication reminders and administered routine ecological momentary assessments (EMAs)

Understanding sexually transmitted infection screening and management in Indiana community health centers.

BACKGROUND: The role of community health centers (CHCs) in preventive health care is central to health reform, yet little is known about how CHCs identify and manage sexually transmitted infections (STIs). METHODS: A survey of Indiana CHCs from April to May 2013 measured reported STI services, clinic expectations for STI testing and management, barriers to screening and management, and partner services. Reported practices were compared with current Centers for Disease Control and Prevention (CDC) guidelines for STI testing in clinical settings. RESULTS: Although most CHCs reported screening

Knowledge of Nairobi East District Community Health Workers concerning HIV-related orofacial lesions and other common oral lesions.

BACKGROUND: Human immunodeficiency virus (HIV) related orofacial lesions (HROLs) impact negatively on the health of patients and could be managed at primary healthcare (PHC) level. Community health workers (CHWs) are crucial in optimal patient management through patient identification, education and early referral for professional care. The study objective was to assess knowledge of Nairobi East district CHWs regarding HROLs and other common oral diseases. METHODS: Of the total population of CHWs, 815 [94.5%] completed a 56-item questionnaire covering 5 topics: general dental knowledge, knowl

Dysphagia and Oral Health in Older Adults: Examining Demographic and Clinical Associations.

Oral health is a modifiable risk factor with significant implications for general health, particularly in the geriatric patient population. Poor oral hygiene has been linked to respiratory disease, aspiration pneumonia, and systemic conditions such as diabetes and cardiovascular disease. Oral health is a critical component of clinical care for elderly patients with dysphagia; however, limited literature exists to guide clinicians in describing the patient characteristics associated with inadequate oral health in this population. This retrospective study aimed to identify associations between p

Identifying salient behavioral health access and social determinant of health factors among a Latino population with uncontrolled diabetes living on the U.S.-Mexico border.

BACKGROUND: Valid measures of behavioral health access and social determinants of health (SDOH) can inform health promotion, programming, and strategies to address identified gaps. Existing SDOH screening tools and domains were not developed using data from low-income Latino populations or community-based interventions, which may limit their relevance for these purposes. AIM/OBJECTIVES: This study sought to identify salient constructs of behavioral health and SDOH based on factor analysis among a low-income, underinsured Latino population with type 2 diabetes. METHODOLOGY: We gathered data f

Improvements in Glycemic Control and Depressive Symptoms Among Adults With Type 2 Diabetes: Retrospective Study.

BACKGROUND: The prevalence of diabetes remains high, with traditional lifestyle interventions demonstrating limited success in improving diabetes-related outcomes, particularly among individuals with diabetes-related mental health comorbidities. Digital health interventions provide the ability to ease the sustained and rigorous self-management needs associated with diabetes care and treatment. Current interventions though, are plagued by small sample sizes, underpowered pilot studies, and immense heterogeneity in program intervention, duration, and measured outcomes. OBJECTIVE: Therefore, thi

Management of neurogenic orthostatic hypotension in the heart failure patient.

Neurogenic orthostatic hypotension (nOH) is a common comorbidity in patients with neurodegenerative diseases. It is associated with an increased risk of falls, incident cardiovascular disease, and all-cause mortality. There are over 5 million individuals in the U.S. with heart failure (HF) with an associated 50% mortality rate at 5 years. The prevalence of nOH and HF increase with age and, as the population continues to age, will be increasingly common comorbid conditions. Thus, the effective management of these conditions has important implications for public health. The management of orthost

Population recovery of a common vole population (Microtus arvalis) after population collapse.

BACKGROUND: Population collapses in small mammals occur naturally after natural disasters and during multi-annual population fluctuations as well as after man-made intervention such as rodent management action. Although there has been extensive previous work on patterns and mechanisms of population fluctuations and cyclicity, little is known about population recovery after collapse. In Europe, the common vole (Microtus arvalis) is the major pest species in agriculture, damaging crops, competing with livestock and potentially posing a health risk to people. In this study, we investigated popula

How can polygenic inheritance be used in population screening for common diseases?

Advances in genomics have near-term impact on diagnosis and management of monogenic disorders. For common complex diseases, the use of genomic information from multiple loci (polygenic model) is generally not useful for diagnosis and individual prediction. In principle, the polygenic model could be used along with other risk factors in stratified population screening to target interventions. For example, compared to age-based criterion for breast, colorectal, and prostate cancer screening, adding polygenic risk and family history holds promise for more efficient screening with earlier start an

Profile of pig farms combining high performance and low antimicrobial usage within four European countries.

Pig farmers are strongly encouraged to reduce their antimicrobial usage because of the rising threat from antimicrobial resistance. However, such efforts should not compromise the herd health status and performance. This study aimed to describe the profile of so-called 'top-farms' that managed to combine both high technical performance and low antimicrobial usage. A cross-sectional study was conducted among 227 farrow-to-finish farms in Belgium, France, Germany and Sweden. Among them, 44 farms were allocated to the top-farms group and were compared with the 'regular' farms group in terms of fa

What is Collaborative Care? Reexamining the Principles of Collaborative Care Through Scaling to Complex Populations and Settings.

The Collaborative Care Model (CoCM) is a principle-based, behavioral health integration model used in primary care to improve access and quality of behavioral health treatment. CoCM was originally developed and tested for the treatment of depression. This JGIM Perspective argues that when principle-based models are scaled to complex populations and settings beyond those for which they were originally developed, the boundaries of the principles are strained. In scaling CoCM for the treatment of opioid use disorder (OUD) co-occurring with PTSD and/or depression in a randomized trial, we found ch

Zika virus: An overview.

The Zika virus has been in the news for quite some time due to the ongoing recent outbreak in the Southern America, which started in December 2015. It has been declared a public health emergency by the World Health Organization in February 2016 owing to its association with the congenital deformities, particularly microcephaly in infants borne to the infected mothers. The rapid spread of the virus throughout the United States of America and subsequently to Asia has raised serious international concerns. Its spread to countries neighboring India is a serious threat to the Indian population. Thi

Gynecological disorders in geriatric emergency medicine.

Emergency physicians must be competent in caring for elderly women because they constitute the fastest growing segment of the population in the United States. Familiarity with acute geriatric gynecologic issues is crucial to providing satisfactory health care for these patients with complaints relating to incontinence, pelvic floor dysfunction, and other gynecologic conditions. Vigilant suspicion for malignancy should be maintained in facilities that service patients without primary health care or insurance. This article provides a systematic approach to emergency department management of comm
