In children with acute lymphoblastic leukaemia (ALL), measurements of minimal residual disease (MRD) during therapy provide crucial information about the response to treatment and the risk of relapse. Flow cytometry is a practical and widely applicable tool for monitoring MRD in patients with ALL. This approach is based on the identification of immunophenotypes expressed by leukaemic cells but not by normal lympho-haematopoietic cells in bone marrow and peripheral blood. Patients with acute leukaemia may have spuriously low arterial oxygen tensions (PaO2). The markedly increased numbers of white blood cells in these patients rapidly consume dissolved plasma oxygen resulting in dramatically decreased PaO2 and calculated oxygen saturations. We report the first case of a thymic cyst appearing in the course of treatment for non-Hodgkin lymphoma of the anterior mediastinum. The patient was a 9-year-old child in whom an abnormal contour of the left cardiac border persisted after chemotherapy, suggesting residual disease. The mass was found at thoracotomy to be a benign thymic cyst. The lesion was not present 2 years previously, and most likely represented cystic degeneration of the thymus, secondary to lymphomatous involvement. CT scan was not helpful in distinguishing the cystic lesion from residual lymphoma.

We report a series of 57 adults patients with lymphoblastic lymphoma treated at our Institution between 1975 and 1987. Mediastinal involvement was seen in 74% (42 cases); extranodal involvement (not including bone marrow) in 24% (14 cases); stage IV in 84% (48 patients) and leukemic phase in 19% (11 patients). In 15 cases immunologic studies were performed 14 proved to be of T type and the other one lack both T and B markers. Survival and duration of remission was better when the patients were treated with aggressive chemotherapy that included maintenance and central nervous system prophylaxis

Candida sepsis in 18-year-old man with acute lymphoblastic leukemia was associated with multisystem alteration accentuated in cardiac and skeletal muscles. Candida tropicalis identified in the post-mortem blood culture was resistant to azole-chemotherapeutics and semi-sensitive to polyene-antibiotics. Unusual myocardial involvement and a role of Candida spp. during opportune infections in immunosuppressed patients are discussed.

Chylothorax is an unusual complication of childhood cancer. It causes to additional morbidity and mortality during management. It should be kept in mind that chylothorax may occur due to mass shrinkage during treatment in patients with mediastinal mass and ductus thoracicus invasion at the initial diagnosis and necessary measures should be taken. This case was presented because of the rarity in pediatric oncology practice.

AIM: To evaluate ondansetron as the sole antiemetic in children treated with emetogenic chemotherapy and irradiation.
METHODS: Fifteen children aged 3-11 years were studied. Seven had acute lymphoblastic leukaemia, two acute myeloid leukaemia, two lymphoma and four had other tumours. Ondansetron 5 mg/m2 IV or 4 mg by mouth was given immediately before chemotherapy or radiation treatment and continued eight hourly for 24 hours. Nausea and vomiting was assessed during treatment and for the next 48 hours, and graded using WHO criteria.
RESULTS: Thirty-eight courses of chemotherapy were assessed
