This case report describes the treatment of an 18-year-old man with a skeletal Class III pattern and a full-step Class III malocclusion. The orthodontic treatment included distal movement of the mandibular dentition with temporary skeletal anchorage devices. The total active treatment time was 30 months. His occlusion and facial appearance were significantly improved by the orthodontic treatment. Posttreatment records 2 years later showed excellent results with good occlusion and facial balance.

PURPOSE: This study investigated the association between the anteroposterior position of the mandible and the perceived need for orthognathic surgery.
MATERIALS AND METHODS: Profiles of 2 men, 1 black and 1 white, and 2 women, 1 black and 1 white, were photographed and altered to produce photos with a straight profile and 6 simulations of mandibular discrepancies, 3 by retrusion and 3 by protusion from each original face. BACKGROUND: Prominent lower front teeth may be associated with a large or prognathic lower jaw (mandible) or a small or retrusive upper jaw (maxilla). Edward Angle, who may be considered the father of modern orthodontics, classified the malocclusion in this situation as Class III. Two hundred and fifty patients were assessed preoperatively using the Mallampati classification and by measuring their thyromental distances. The ease or difficulty of direct laryngoscopy was assessed at the time of induction of anaesthesia. A 23-year-old woman with a skeletal Class III relationship, a normal vertical jaw relationship, and missing maxillary lateral incisors was treated with bidimensional fixed edgewise appliance therapy combined with orthognathic surgery. A functional and esthetic occlusion in an improved facial profile was established at the end of the surgical and orthodontic treatment. Pretreatment, posttreatment, and long-term follow-up records for the patient are presented.
