Ending the HIV Epidemic in Metropolitan Atlanta: a mixed-methods study to support the local HIV/AIDS response.

INTRODUCTION: Four counties within the Atlanta, Georgia 20-county eligible metropolitan area (EMA) are currently prioritized by the US "Ending the HIV Epidemic" (EHE) initiative which aims for a 90% reduction in HIV incidence by 2030. Disparities driving Atlanta's HIV epidemic warrant an examination of local service availability, unmet needs and organizational capacity to reach EHE targets. We conducted a mixed-methods evaluation of the Atlanta EMA to examine geographic HIV epidemiology and distribution of services, service needs and organization infrastructure for each pillar of the EHE initi

Mechanisms of Heat-Induced Sleep Disruption in Aging: A Narrative Review.

Rising global temperatures and more frequent heat waves pose a growing threat to public health, particularly among older adults. Age-related declines in thermoregulatory capacity such as reduced sweating, impaired vasodilation, and diminished hypothalamic responsiveness make it more difficult to cope with elevated nighttime temperatures. These thermal challenges disrupt sleep by prolonging sleep onset, fragmenting slow-wave and REM sleep, and suppressing melatonin secretion. Importantly, sex-related differences in thermoregulatory aging-particularly menopause-associated vasomotor instability i

Improving Access to the Transportation Alternatives Program for Rural Communities.

Rural small towns with small main streets and compact downtown development can be ideal locations to create walkable communities. The Centers for Disease Controls and Prevention (CDC)'s High Obesity Program (HOP) funds Cooperative Extension programs to implement strategies to improve food access and support active transportation in high obesity (often rural) counties. The Louisiana State University (LSU) AgCenter HOP program had previously partnered with rural low-income communities to create Complete Streets plans but was challenged to find ways to implement those plans. A technical assistanc

The newest epidemic: a review of HIV/AIDS in Central and Eastern Europe.

HIV/AIDS has emerged as a grave public health threat in Central and Eastern Europe and in the Central Asian republics over the past five years. Massive political, social, cultural, and behavioural changes - along with economic upheaval and collapse of the public health infrastructure in many countries - have created circumstances conducive to the rapid spread of HIV. This paper reviews HIV and sexually transmitted disease (STD) data for all countries in the region, as well as behavioural, social, cultural, and other HIV epidemic enabling factors. The epidemiological picture of HIV in the regio…

[Estimating the real price of CAR-T cells in France: From list price to net price].

INTRODUCTION: CAR-T cells represent a breakthrough in relapsed or refractory hematologic malignancies. In France, they are commonly associated with prices ranging from €300,000 to more than €400,000, based on early access indemnities and initial list prices. Yet these figures do not reflect the actual cost for the healthcare system, which is reduced by confidential rebates and pricing adjustments negotiated between companies and public authorities. This study aims to estimate the real (net) price of commercial CAR-T cells therapies in France and analyze the evolution of their public list price

The role of syndromic knowledge in Ethiopian veterinarians' treatment of cattle.

Veterinarians play a significant role in the treatment and prevention of livestock diseases at the farm level, safeguarding public health and ensuring food safety. In sub-Saharan Africa, access to quality veterinary services is a major challenge for livestock farmers due to the low number of publicly employed veterinarians, underfunding and privatisation of veterinary services. Low investment in veterinary services and infrastructure, including a lack of laboratories for diagnosis, has made veterinarians rely on their experience and knowledge of cattle disease symptoms developed over years of

Climate-resilient livestock systems in Sub-Saharan Africa: a one health systems framework integrating pastoralism, animal health, and sustainable development.

Pastoral livestock systems in Sub-Saharan Africa face accelerating climate threats that compromise food security, animal health, and human well-being across millions of rural households. Rising temperatures, recurrent droughts, erratic rainfall, and floods degrade rangeland productivity, restrict pastoral mobility, and reshape disease ecology, driving outbreaks of trypanosomiasis, tick-borne infections, and Rift Valley fever. These biophysical pressures interact with governance failures, farmer-herder conflict, and weak veterinary infrastructure to generate compound vulnerabilities that extend

Understanding the spatiality in neglected tropical diseases: An ethnographic study of scrub typhus in Puducherry, India.

Individuals from marginalised backgrounds are often "placed" in spatial planes marked by disadvantages and poor resource availability, impacting their health outcomes. This study explored the role of spatiality in shaping health outcomes, with scrub typhus-a neglected tropical disease (NTD) of zoonotic origin-as an illustrative case. Employing a spatial justice framework, the study investigated the intersection of geospatial and socio-spatial factors influencing health and illness experience in Puducherry, India. An interdisciplinary mixed-methods approach was employed, involving geospatial ho

Urban Accessibility or Social Inequality? A Multilevel Spatial Analysis of Built Environments and Type 2 Diabetes in Milan.

Type 2 diabetes mellitus (T2DM) follows a social gradient; however, access to health-related services and amenities in urban areas may also impact its prevalence. This study investigates the association between characteristics of the built environment and T2DM prevalence in Milan, Italy. Utilising administrative health care data and open access territorial information, we examined the relationship between neighbourhood-level features and T2DM among the Milanese population in 2019. The analysis focused on six domains: food environment, physical activity infrastructure, cultural facilities, neig

Introduction of Heat-Stable Carbetocin for Postpartum Hemorrhage Prevention in Public Sector Hospitals in Kenya: Provider Experience and Policy Insights.

BACKGROUND AND OBJECTIVE: In Kenya, the leading cause of maternal deaths is obstetric hemorrhage (39.5%), with postpartum hemorrhage (PPH) accounting for 50% with quality of uterotonics as one of the biggest challenges. The World Health Organization (WHO) in 2018 included heat-stable carbetocin (HSC) for the prevention of PPH in settings where the quality of oxytocin cannot be guaranteed. Maintenance of the cold chain for uterotonics is a challenge. HSC does not require refrigeration, reducing pressure on the fragile cold chain infrastructure. The main objective was to understand PPH preventio

Mobility-informed hospital siting for accessible and lower-carbon healthcare travel: an ensemble learning study in Tianjin, China.

Healthcare facility layouts shape access to care, travel burdens, and travel-related carbon emissions. Conventional siting assessments, however, often rely on supply-side indicators without accounting for how patients actually travel. This study examined nine tertiary Grade-A hospitals in central Tianjin using 2,596 questionnaires from hospital visitors and spatial data on road networks, bus and metro stations, expressways, and points of interest. Six spatial and transport indicators were evaluated: surveyed medical-visitor density, regional service coverage, transport accessibility, distance

Transportation control and epidemic containment: empirical evidence from China's Wuhan lockdown.

BACKGROUND: The rapid development of modern transportation networks, coupled with increasing urbanization and population mobility, presents a dual challenge for global public health systems: while enhancing connectivity and economic integration, these networks also accelerate the spatial dissemination of infectious diseases. The COVID-19 pandemic, which emerged in Wuhan, China, in late 2019, starkly illustrated this paradox. As a major national transportation hub with dense high-speed rail (HSR) connectivity, Wuhan exemplifies how modern transport infrastructure can facilitate the rapid interc

Divide and recombine approaches for fitting logistic regression to large-scale health surveillance data: application to diabetes risk prediction in BRFSS.

The global rise in diabetes mellitus poses a major public health concern, highlighting the urgent need for effective risk prediction models to support early identification and prevention strategies. Yet, applying conventional statistical techniques to large-scale health surveillance data remains challenging due to memory limitations and high computational demands. One promising alternative, the divide and recombine (D&R) approach partitions the data into smaller subsets, fits logistic models independently within each, and combines the estimates to approximate full-sample maximum likelihood res

Promotion of Preventive Sleep Research from the Perspective of Health and Public Safety in Japan - Secondary Publication.

Ensuring good sleep quality and adequate sleep duration is crucial for health. Sleep apnea syndrome (SAS) impairs sleep quality and increases the risk of diabetes, cardiovascular diseases, and accidents. The author has significantly advanced the understanding of SAS in Japan through over 20 years of epidemiological studies. Findings have revealed that individuals with a 3% oxygen desaturation index (ODI) ≥15 events/h have a 1.69-fold higher risk of developing diabetes. Those with mild to severe sleep-disordered breathing (SDB) with a 3% ODI ≥5 events/h face high risks of heart disease and lacu
