This report describes a series of six cases of inflammatory periapical disease with small aggregates of Langerhans cells as a minor component. Immunohistochemical findings confirm that the cells are phenotypically related to Langerhans cells. Aggregates of these cells are not normally found in radicular cysts or periapical granulomas and have been interpreted to represent chronic localized Langerhans' cells histiocytosis. Chronic periapical lesions were recognized clinically and radiologically in 109 patients aged from 10 to 79, twice as frequent in the maxilla as in the mandible. Histopathological analysis revealed periapical granulomas and cysts. Forty-eight periapical granulomas were studied histobacteriologically. Histopathological investigations were done in 72 cases of periapical granulomas and 9 cases of periapical cysts demonstrating that both granulomas and cysts were composed of granulation tissue varying in maturity. Immunohistochemical studies by the peroxidase method PAP showed the presence of light chains (kappa and lambda) and heavy chains (IgA, IgM, IgG, IgE). Gangrenous cellulitis of the face of dental origin had a very abrupt onset, spread rapidly in spite of treatment and led to severe pleuropericardial complications. The outcome was favorable after repeated thoracic surgery. Preventive therapeutic measures for these major accidents are discussed, following a detailed report on the case presented and treatment modalities applicable, fatal outcomes still being possible in 1984 in spite of progress in therapy.

Dens in dente is the result of an invagination of the enamel organ into the developing dental papilla. The full pathologic potential of this lesion is often not fully appreciated. A case is reported in which a cyst resulting from a dens in dente obliterated the maxillary sinus and necessitated root canal therapy and apicoectomies in four adjacent teeth.
