The Choice of Osteosynthesis for Distal Radius Fractures: A Matter of Taste, Fracture Instability, or Patient-Related Factors? A Retrospective Study of Functional Outcome in 346 Distal Radius Fractures Operated With Percutaneous Wires or Volar Plate Fixation.

BACKGROUND: Surgery with volar locking plate (VLP) for distal radius fractures (DRFs) has become dominant over percutaneous Kirschner wire (K-wire) (PKW) fixation. Not many studies have proved advantages of the VLP and the increasing dominance of the VLP is thus not derived from evidence of superiority but influenced by other factors. METHODS: By retrospectively classifying 346 DRFs treated with either PKW or VLP fixation, according to the Buttazzoni classification system, we aimed to investigate the determining factors for choice of surgical method, and by review of the patients' medical rec…

Clinical Outcomes in Distal Radius Fractures Accompanied by Volar Lunate Facet Fragments: A Comparison between Dorsal and Volar Displaced Fractures.

Background: The treatment strategy for distal radius fractures accompanied by volar lunate facet fragment is controversial. In most cases, only the bone fragment size was discussed and a plate for distal placement is selected due to the accompanying volar lunate facet fragment regardless of the direction of bone fragment displacement. In this study, we divided distal radius fractures accompanied by volar lunate facet fragment into dorsal and volar displaced fractures, and treated these surgically based on different treatment strategies. Methods: The subjects were 25 patients with distal radius…

Treatment Modality of Distal Ulnar Head and Neck Fractures Associated With Operatively Treated Distal Radius Fractures Does Not Affect Outcomes.

Background: This study compares the functional outcomes and complications between operatively and nonoperatively treated distal ulnar head and neck fractures associated with internal fixation of concomitant distal radius fractures. Methods: A 7-year retrospective chart review was performed to identify patients with operatively treated distal radius fractures associated with distal ulnar head and neck fractures. Ulnar styloid fractures were excluded. Fifty-eight patients who had a minimum of 6 months of follow-up were identified. Patients were divided into 3 treatment groups: nonoperative, open…

Distal radius fractures: What are the effects of ulnar styloid fracture and face-toface rehabilitation on the functional outcome?

OBJECTIVE: This study aimed to investigate the effects of the accompanying ulnar styloid fracture and 2 different postoperative rehabilitation protocols on the final outcomes following surgical treatment of distal radius fractures. METHODS: In this retrospective study, 47 patients (11 male, 35 female; mean age=52.6 years, age range=24-85) who underwent plate fixation for distal radius fractures were divided into 4 groups based on the presence of an ulnar styloid fracture and type of rehabilitation. To evaluate clinical outcomes, wrist range of motion (ROM), grip strength, lateral pinch streng…

Rare triad of fractures in the upper limb. Simultaneous fractures of the distal radius, with the radial head and the ulna styloid.

We're reporting a 35-year-old gentleman that presented with a rare combination of ipsilateral fracture of both proximal and distal ends of the radius associated with ulna styloid fracture due to a fall with outstretched hands. Thorough clinical examination of 2 adjacent joints (elbow and wrist) is critical in identifying such an unusual or rare combination of injuries. Appropriate management for such cases depends on the characteristics of the injury, such as fracture patterns, job, hand dominance, and age of the patient. The treatment should aim to preserve the radial head to avoid the possib

Simultaneous ipsilateral Monteggia fracture-dislocation and distal radius fracture: A report of a pediatric case and review of the literature.

We present the case of a 12-year- old boy who suffered from a combination of Monteggia fracture-dislocation along with ipsilateral distal radius fracture. The patient underwent closed reduction of the Monteggia lesion and the distal radius followed by percutaneous pinning. The postoperative course was satisfactory. Associated Monteggia fracture-dislocation and ipsilateral distal forearm fracture are rare. Our case and the literature review highlight the presence of common clinical features in pediatric patients with these particular injuries, including male dominance, age around 9 years, fall

Does circumferential casting prevent fracture redisplacement in reduced distal radius fractures? A retrospective multicentre study.

BACKGROUND: This study evaluates whether a circumferential cast compared to a plaster splint leads to less fracture redisplacement in reduced extra-articular distal radius fractures (DRFs). METHODS: This retrospective multicentre study was performed in four hospitals (two teaching hospitals and two academic hospitals). Adult patients with a displaced extra-articular DRF, treated with closed reduction, were included. Patients were included from a 5-year period (January 2012-January 2017). According to the hospital protocol, fractures were immobilized with a below elbow circumferential cast (CC…

Distal ulna hook plate fixation for unstable distal ulna fracture associated with distal radius fracture.

The significance of distal ulna fractures is often undermined, which can result in inadequate treatment compared with fractures of the radius, the ulna's larger counterpart. However, little guidance exists in the current literature on how to manage distal ulna head or neck fractures and intra-articular ulna head fractures. Therefore, the purpose of this retrospective study was to evaluate the outcomes of distal ulna hook plate fixation for the treatment of an unstable distal ulna fracture associated with a distal radius fracture. Twenty-five patients with unstable distal ulna fractures who und

Unilateral, trifocal, diaphyseal fracture of the radius with ipsilateral mid-shaft ulna fracture in an adult: a case report.

INTRODUCTION: To the best of our knowledge, a trifocal, diaphyseal fracture of the radius associated with ipsilateral mid-shaft fracture of the ulna in an adult has not been reported in the literature to date. The AO classification system does not include such a fracture configuration. CASE PRESENTATION: We report a case of trifocal, diaphyseal fracture of the radius with a mid-diaphyseal fracture of the ulna in a 53-year-old Caucasian, British, right-hand dominant woman involved in a head-on collision with another vehicle. The management of this rare fracture configuration is described and a

Displaced intra-articular fractures of the distal radius: the effect of fracture displacement on contact stresses in a cadaver model.

Contact stresses in the wrist were measured after simulating displaced fractures of the lunate fossa in the distal radius of eight human cadaver arms. Osteotomies created displaced lunate fossa fractures of 0, 1, 2, and 3 mm. Contact stresses were measured with Fuji pressure-sensitive film after loads of 100 N were applied to the wrist through wrist flexor and extensor tendons. Mean contact stresses were significantly increased with step-offs of 1 mm or more. Maximum stresses and overloaded areas were significantly increased with step-offs of 2 mm or more. As the magnitude of the fracture disp

A Machine Learning Approach to Predicting Radiographic Outcomes of Nonsurgically Treated Distal Radius Fractures.

INTRODUCTION: Several statistical models have been developed to predict the stability of distal radius fractures after closed reduction, but their findings have not been consistently reproduced. We aimed to develop a machine learning (ML) model to predict radiographic outcomes of nonsurgically treated distal radius fractures based on pre-reduction and postreduction radiographic parameters and demographic variables. METHODS: Adults with displaced distal radius fractures at a single institution between 2012 and 2024 were identified through retrospective chart review. Inclusion criteria required…

Modified retrograde ESIN for diametaphyseal distal radius fractures in children: a retrospective cohort study.

BACKGROUND: Diametaphyseal distal radius fractures in children represent a therapeutic challenge because conventional elastic stable intramedullary nailing (ESIN) may provide insufficient distal fragment control, while alternative fixation methods are more invasive. This study evaluates a modified retrograde ESIN technique with a 90° distal bend ("crutch-bend technique") designed to improve distal fragment support and alignment. METHODS: In this retrospective single-center study, 43 pediatric patients with diametaphyseal distal radius fractures treated between 2022 and 2025 were analyzed. Pat…

Combined Dorsal Spanning and Volar Plating for Management of Highly Comminuted Distal Radius Fractures.

BACKGROUND: Comminuted intraarticular distal radius fractures remain a surgical challenge, with no consensus on the optimal fixation method. Conventional techniques such as volar plating or external fixation may be insufficient to restore anatomical alignment and articular congruity in highly comminuted cases. This study evaluated the clinical and radiographic outcomes of combined dorsal spanning plate (DSP) and volar locking plate (VLP) fixation for management of comminuted distal radius fractures. METHODS: This prospective cohort study was conducted at a level I university trauma center bet…

Risk factors for extensor pollicis longus tendon rupture following non-displaced distal radius fractures.

INTRODUCTION: Distal radius fractures (DRFs) are common, with an increasing incidence, particularly among the elderly. Rupture of the extensor pollicis longus (EPL) tendon, essential for thumb extension, is a notable complication, especially in non-displaced DRFs. Several mechanisms, such as local adhesion, ischemic atrophy, and tendon laceration, are associated with EPL tendon rupture. This multicenter retrospective study aims to identify risk factors for EPL tendon rupture in non-displaced DRFs. MATERIALS AND METHODS: The study reviewed 20 cases of EPL tendon rupture and 52 control cases fr…

Epidemiology, risk factor, and economic analysis of peripheral nerve injury following distal radius fractures.

PURPOSE: Distal radius fractures (DRFs) are a common orthopaedic injury with a bimodal age and gender distribution, comprising male children after high-energy trauma and older women after low-energy falls. Complications like malunion, ligament ruptures, and nerve injuries can significantly impact recovery. This study aimed to address the gap in population-based data by analyzing the prevalence and risk factors for peripheral nerve injuries associated with DRFs. METHODS: The National Inpatient Sample database was used to analyze DRFs patients with associated ulnar, median, or radial nerve inju…

The Biomechanical Behavior of Fixation Options in Short Segment Radius and Ulna Diaphyseal Fractures.

PURPOSE: Plate fixation of radius and ulna shaft fractures has evolved due to a more complete understanding of the biology and mechanics. Screw fixation across six cortices has been adopted as the standard, purported to provide the optimal biological and mechanical environment for healing; however, recent investigations have proposed that four cortices of fixation may be sufficient. These recommendations are intended for segments of optimal length; however, it is less clear how these options perform in short segment fixation. Our objective was the determine the biomechanical behavior of fixati
