Chronic pelvic pain in urogynecological practice: a personal view.

Women with chronic pelvic pain are frequently referred to gynaecologists as over 50% also have genitourinary or irritable bowel symptoms or both. When no specific pathology is found and the pathophysiology is unclear, the aetiology is probably an interplay of neuropathic, inflammatory and functional factors. The principles of medical and surgical treatment are discussed and should be based on evidence-based medicine.

Accessory navicular bone: when ankle pain does not originate from the ankle.

A young girl suffering from ankle pain occurring after gymnastics classes was referred to the rheumatology department by an orthopedic surgeon because a rheumatological condition was suspected to cause her symptoms. MRI was useful in pointing to the correct diagnosis of accessory navicular bone (AN). The morphological classification of ANs is discussed and the imaging modalities for diagnosis are presented.

[Treatment of trigeminal neuralgia using controlled differential selective thermorhizotomy].

Personal experience in 35 cases of controlled, partial, selective thermorhizotomy of the trigeminus is referred to in a discussion of its indisputable advantages (abolition of pain, preservation of tactile sensibility, simple method) by comparison with other techniques, and its theoretical basis: selective lesion of the "delta" A and C fibres by exploiting the vulnerability gradient of the nerve fibres to heat.

[Short-term therapy of painful muscular disorders. Results of a multicenter double-blind test of 2 new suppository preparations with and without codeine].

A report is given about a multicentric double-blind test for proof of effectiveness of two new suppository preparations with and without codeine against comparable remedies. Dolo Visano Supp. sine codeino showed a slight superiority over the reference preparation. This was proved above all for the influence upon pain and muscular overstrain. The better tolerability was marked. Dolo Visano Supp. (with codeine) showed advantages against the reference preparation. It was used in cases of severe pain, and in 88% it had a very good effect, whereas for the reference preparation this applied only in

Possible pathogenic mechanism of gluteal pain in lumbar disc hernia.

Recent reported results by Fang et al. published in BMC Musculoskeletal Disorders have added to the weight of evidence supporting associatio … i (intraspinal portion), which produce gluteal pain by irritating superior/medial cluneal nerve and referred pain from facet joints and sacroiliac joints. In addition, the presence of proximal sciatica could also induce gluteal pain. Lastly, fibers in the superior or inferior gluteal nerve could be compressed/irritated in LDH, inducing LDH-related gluteal pain. …

Intertester reliability of a low back pain classification system.

STUDY DESIGN: This prospective study of intertester reliability examined pairs of therapists' ability to agree independently on a patient's low back pain diagnosis. OBJECTIVE: To determine the intertester reliability of a low back pain classification system among experienced and novice clinicians. BACKGROUND: Many of the disparate categorization schemes for patients with low back pain are purely nominal, assigning designations based on the presumptive source of the problem without providing any practical guide for rehabilitation. A useful classification scheme reliably groups patients into t

General health factors and denture function in patients with burning mouth syndrome and matched control subjects.

A total of 30 denture-wearing patients with burning mouth syndrome (BMS) referred to a Pain Clinic Unit and 26 age- and sex-matched control subjects were examined and compared with respect to general health factors and denture function. The study demonstrated a significantly higher frequency of multiple chronic diseases, psychosocial stress factors, and tenderness/pain in masticatory, neck, shoulder, and suprahyoid muscles in patients with BMS. Denture function differed also between the two groups as patients with BMS had significantly less daily use of dentures, reduced tongue space, incorrec

Magnetic resonance imaging of the temporomandibular joint. Increased T2 signal in the retrodiskal tissue of painful joints.

The cause of pain associated with TMJ disk displacement and internal derangement is unclear. In clinical work with MR imaging we have noted increased T2 signal from the retrodiskal tissue in some patients with TMJ pain and dysfunction. The purpose of this study was to analyze a possible association between pain and the T2 signal intensity from the retrodiskal tissue on MR images. The study was based on 48 joints in 33 patients referred for MR imaging of the TMJ. The T2 signal from the retrodiskal tissue was measured on the MR scanner and was correlated to the degree of pain. The results showed

Determination of potential risk characteristics for cauda equina compression in emergency department patients presenting with atraumatic back pain: a 4-year retrospective cohort analysis within a tertiary referral neurosciences centre.

OBJECTIVE: Atraumatic back pain is an increasingly common presentation to the ED. A minority of these cases will have significant structural pathology, resulting in acute cauda equina compression (CEC). Although clinicians often look for 'red flags' to identify potential CEC, the prognostic accuracy of these presenting symptoms and clinical examination findings is unclear. We sought to evaluate the accuracy of individual clinical features in a large cohort of ED patients with atraumatic backpain and reference standard imaging, for the diagnosis of CEC. METHODS: A retrospective case note revie

Combination of exercise training and erythropoietin prevents cancer-induced muscle alterations.

Intra-abdominal abscesses are localized collections of pus confined in the peritoneal cavity by an inflammatory barrier. They are generally … on of a pelvic abscess, but brought up the theory that the etiology of the symptoms could be due to referred pain to the hips from the abscess. Further studies are required to investigate the percentage of pelvic abscess patients who primarily present with a component of hip pain.

A minimally invasive lumbar decompression procedure after an interspinous spacer device implantation: an uncommon order of treatment with a successful outcome.

A patient with lumbar spinal stenosis (LSS) was referred to our clinic due to refractory low back pain, radicular pain, and neurogenic claudication despite conservative treatment with medical management. Imaging of the lumbar spine revealed spinal canal and foraminal stenosis. Multiple epidural steroid injections (ESI) were performed which did not resolve her condition. We offered her an Implantation of an Interspinous Spacer Device (ISD) since her primary symptoms were predominantly characteristic of radicular pain. Her radicular symptoms improved but experienced worsening of her lower back p
