[Sleep related breathing disorders].

Sleep related breathing disorders are common conditions and the management of patients with sleep apnea is an essential component of routine patient care. Daytime sleepiness is the leading symptom of sleep apnea but not mandatory. Especially patients with cardiac, pulmonary or metabolic comorbidities can benefit from treatment of sleep apnea and those patients should be considered for cardio-respiratory screening even with mild clinical symptoms. Continuous positive airway pressure therapy is still the most efficient treatment for sleep apnea and standard treatment for severe forms. With patie

Effects of breathing training on voluntary hypo- and hyperventilation in patients with panic disorder and episodic anxiety.

Anxiety disorders are associated with respiratory abnormalities. Breathing training (BT) aimed at reversing these abnormalities may also alter the anxiogenic effects of biological challenges. Forty-five Panic Disorder (PD) patients, 39 Episodic Anxiety patients, and 20 non-anxious controls underwent voluntary hypoventilation and hyperventilation tests twice while psychophysiological measures were recorded. Patients were randomized to one of two BT therapies (Lowering vs. Raising pCO(2)) or to a waitlist. Before treatment panic patients had higher respiration rates and more tidal volume instabi

Developmental aspects of the upper airway: report from an NHLBI Workshop, March 5-6, 2009.

The upper airway serves three important functions: respiration, swallowing, and speech. During development it undergoes significant structural and functional changes that affect its size, shape, and mechanical properties. Abnormalities of the upper airway require prompt attention, because these often alter ventilatory patterns and gas exchange, particularly during sleep when upper airway motor tone and ventilatory drive are diminished. Recognizing the relationship of early life events to lung health and disease, the National Heart, Lung, and Blood Institute (NHLBI), with cofunding from the Off

[Bronchopulmonary dysplasia in ex-prematures approaching adulthood].

Many patients born prematurely who developed bronchopulmonary dysplasia (BPD) as neonates are now approaching adulthood. Adults with BPD are at increased risk for respiratory difficulties including respiratory and lung function disorders. Respiratory symptoms include wheezing, cough and dyspnoea. In adult BPD patients lung function is impaired, notably by bronchus obstruction, hyperreactivity and reduced diffusion capacity; exercise capacity is also diminished. The pathophysiology of BPD is not identical to that of asthma and standard treatment for asthma is therefore not effective. Premature

[Polysomnography in the prevention of crib death].

The authors stress the importance of polysomnography--a new electrophysiologic method--in the prevention of SIDS (cot death). SIDS is the most important and frequent cause of infant mortality between 1 and 12 months of age in western countries (nowadays between 1-2/1000!). In Hungary the frequency is not so high. In the last few years the incidence declined after the "back to sleep" campaigns, but to reach further success, it is very important to seek the so called "risk" babies. The unique cause of cot death is not yet understood exactly, but some instability in respiration (mostly during the

[Prolonged artificial ventilation of the lungs in nervous system diseases].

Artificial ventilation of the lungs (AVL) has been carried out for many years in 25 patients with nervous diseases. Spontaneous respiration ceased because of peripheral neuron involvement in malignant myasthenia (5 cases), chronic polyneuropathy (1), paralysis of motor muscles after poliomyelitis (1), amyotrophic lateral sclerosis (11), spinopontocerebellar atrophy (1), and because of the central neuron involvement in 5 patients, 3 of these with the apnea syndrome during sleeping caused by syringobulbomyelia, atrophic process in the cerebellum and stem, and Arnold-Chiari's syndrome, and 2 with

Physiologic instability in panic disorder and generalized anxiety disorder.

BACKGROUND: Because panic attacks can be accompanied by surges in physiologic activation, we tested the hypothesis that panic disorder is characterized by fluctuations of physiologic variables in the absence of external triggers. METHODS: Sixteen patients with panic disorder, 15 with generalized anxiety disorder, and 19 normal control subjects were asked to sit quietly for 30 min. Electrodermal, cardiovascular, and respiratory measures were analyzed using complex demodulation to quantify variability in physiologic indices. RESULTS: Both patient groups reported equally more anxiety and cardia

Revision of Energy Metabolism Adaptations in High-Level Athletes: From Physical Performance Enhancement to Potential Therapeutic Targets in Mental Disorders.

High-level athletic performance requires the implementation of personalized strategies based on the analysis of metabolic pathways involved in energy production: phosphagen, glycolytic, and oxidative pathways. In this context, mitochondria play an essential role as the central regulator of energy production, being closely linked to these three pathways. Exercise boosts cellular respiration, which can also be optimized by nutritional interventions and targeted supplementation, promoting mitochondrial biogenesis, reducing oxidative stress and increasing ATP production. These metabolic adaptation

Low-oxygen storage modulates fruit flavor formation: Metabolic and regulatory insights.

Controlled atmosphere prolongs fruit storage period by reducing oxygen availability, thereby suppressing respiration, ethylene production, softening, and physiological disorders. However, low oxygen can compromise flavor, a critical attribute for consumer acceptance. This review summarizes current knowledge of how hypoxic storage influences sugar and acid metabolism, volatile biosynthesis, and associated molecular mechanisms. Low oxygen promotes sucrose breakdown and increases glucose and fructose accumulation, while suppressing acid degradation. However, volatile production is reduced through

Molecular pathogenesis and gene therapy-based intervention of GTPBP3-related mitochondrial disease.

Mitochondrial transfer RNA (mt-tRNA) modification determines organelle translation and function. GTPBP3 and MTO1 catalyze 5-taurinomethyluridine (τm5U) modification at wobble uridine of five mt-tRNAs. τm5U hypomodification causes mitochondrial encephalomyopathy, but the underlying pathogenesis and intervention strategy due to GTPBP3 mutations are lacking. In this study, we identify two genetic variants (c.689 A > C (p.Q230P) and c.1120 A > G (p.N374D)) of GTPBP3 in a Chinese proband with metabolic disorders and multisystem dysfunction. Mechanistically, Q230P and N374D mutations induce protein

Ergothioneine controls mitochondrial function and exercise performance via direct activation of MPST.

Ergothioneine (EGT) is a diet-derived, atypical amino acid that accumulates to high levels in human tissues. Reduced EGT levels have been linked to age-related disorders, including neurodegenerative and cardiovascular diseases, while EGT supplementation is protective in a broad range of disease and aging models. Despite these promising data, the direct and physiologically relevant molecular target of EGT has remained elusive. Here, we use a systematic approach to identify how mitochondria remodel their metabolome in response to exercise training. From these data, we find that EGT accumulates i

PML is crucial for neural stem cell differentiation, stress tolerance and mitochondrial integrity.

The tumor suppressor promyelocytic leukemia protein (PML) has important roles in brain development; however, the molecular and cellular pathways regulated by PML in neuronal cells remain largely unknown. To address this issue, we analyzed gene expression changes caused by loss of PML in neural stem cells. Our findings revealed that PML-deficient cells exhibited increased mTOR (mammalian target of rapamycin) pathway activation and protein translation, as well as impaired autophagy and proteasome activity, resulting in increased formation of aggregates and stress-induced death. Loss of PML disru

Disruption of the PIKfyve complex unveils an adaptive mechanism to promote lysosomal repair and mitochondrial homeostasis.

Lysosomes are essential organelles that regulate cellular homeostasis through complex membrane interactions. Phosphoinositide lipids play critical roles in orchestrating these functions by recruiting specific proteins to organelle membranes. The PIKfyve/Fig4/Vac14 complex regulates PI(3,5)P₂ metabolism, and intriguingly, while loss-of-function mutations cause neurodegeneration, acute PIKfyve inhibition shows therapeutic potential in neurodegenerative disorders. We demonstrate that PIKfyve/Fig4/Vac14 dysfunction triggers a compensatory response where reduced mTORC1 activity leads to ULK1-depend

The Neural Palette of Heme: Altered Heme Homeostasis Underlies Defective Neurotransmission, Increased Oxidative Stress, and Disease Pathogenesis.

Heme, a complex iron-containing molecule, is traditionally recognized for its pivotal role in oxygen transport and cellular respiration. However, emerging research has illuminated its multifaceted functions in the nervous system, extending beyond its canonical roles. This review delves into the diverse roles of heme in the nervous system, highlighting its involvement in neural development, neurotransmission, and neuroprotection. We discuss the molecular mechanisms by which heme modulates neuronal activity and synaptic plasticity, emphasizing its influence on ion channels and neurotransmitter r

The mitochondrial calcium uniporter engages UCP1 to form a thermoporter that promotes thermogenesis.

Uncoupling protein 1 (UCP1)-mediated adaptive thermogenesis protects mammals against hypothermia and metabolic dysregulation. Whether and how mitochondrial calcium regulates this process remains unclear. Here, we show that mitochondrial calcium uniporter (MCU) recruits UCP1 through essential MCU regulator (EMRE) to form an MCU-EMRE-UCP1 complex upon adrenergic stimulation. This complex formation increases mitochondrial calcium uptake to accelerate the tricarboxylic acid cycle and supply more protons that promote uncoupled respiration, functioning as a thermogenic uniporter. Mitochondrial calci

Systematic Comparison of Respiratory Signals for the Automated Detection of Sleep Apnea.

Sleep apnea is one of the most common sleep disorders. It is characterized by the cessation of breathing during sleep due to airway blockages (obstructive sleep apnea) or disturbances in the signals from the brain (central sleep apnea). The gold standard for diagnosing sleep apnea is performing an overnight polysomnography recording which contains, among others, a wide array of respiratory signals. Respiration information can also be extracted from other physiological signals such as an electrocardiogram or from a bio-impedance measurement on the chest. Studies have shown that algorithms can b

Vagal function indexed by respiratory sinus arrhythmia and cholinergic anti-inflammatory pathway.

The autonomic nervous system, in particular vagal function, plays an important role in a wide range of somatic and mental disorders. Cardiac vagal function can be indexed by the respiratory sinus arrhythmia (RSA) - oscillations in heart rate linked to respiration mediated predominantly by fluctuations of vagus nerve efferent traffic originating in the nucleus ambiguus. Moreover, the neurocardiac vagal modulation has been shown to be related to physiological adaptability/flexibity and emotional regulation. Thus, greater vagal withdrawal during stressors and subsequent recovery should be indicat

Respiratory, autonomic, and experiential responses to repeated inhalations of 20% CO₂ enriched air in panic disorder, social phobia, and healthy controls.

Inhalation of carbon dioxide (CO₂) enriched air triggers anxiety in panic disorder (PD) patients, which is often interpreted as a sign of biological vulnerability. However, most studies have not measured respiration in these tasks. We compared patients with PD (n=20) and social phobia (SP, n=19) to healthy controls (n=18) during eight inhalations of 20% CO₂, preceded and followed by two inhalations of room air, while continuously measuring subjective anxiety and dyspnea as well as autonomic and respiratory variables. PD patients showed increased reactivity and delayed recovery during CO₂ inhal

PURPOSE OF REVIEW: To review some management issues in myasthenic disorders that are not part of the immune interventions. These are mainly related to pharmacotherapy of these conditions and side effects of various medications, but also referred to acute management of respiration in myasthenic crises. RECENT FINDINGS: Antisense compound inhibiting acetylcholine esterase has positive therapeutic effects during short-term administration, but further studies are needed to confirm its chronic effects.
