Geometric Tear Configuration Influences Intraoperative Reparability after Arthroscopic Rotator Cuff Repair.

BACKGROUND: Rotator cuff (RC) tear morphology is thought to influence reparability and healing potential, yet few clinical studies have compared outcomes across distinct tear patterns. The purpose of this study was to determine whether different RC tear patterns affect reparability and retear rate following arthroscopic repair. METHODS: A retrospective analysis was conducted on 134 patients who underwent arthroscopic repair for posterosuperior RC tears between 2018 and 2022. Tears were classified arthroscopically into 5 geometric patterns-crescent, anterior L-shaped, posterior L-shaped, U-sha

Combined anterior latissimus dorsi and teres major transfer and superior capsular reconstruction using autogenous biceps tendon for anterosuperior irreparable rotator cuff tears: technical note.

This technical note describes the surgical technique of combining anterior latissimus dorsi and teres major (LDTM) tendon transfer with biceps superior capsular reconstruction (SCR) for the treatment of anterosuperior irreparable rotator cuff tears (AIRCTs). Anterosuperior irreparable rotator cuff tears, characterized by irreparable tears in the subscapularis and supraspinatus tendons, can lead to significant shoulder dysfunction. The described procedure aims to restore glenohumeral stability, prevent humeral head migration, and optimize shoulder mechanics. The LDTM tendon transfer replicates

Platelet Concentration Does Not Influence Clinical Efficacy and Retear Rates of Rotator Cuff Repair With Platelet-Rich Plasma: A Systematic Review and Meta-analysis With Meta-Regression.

BACKGROUND: Platelet-rich plasma (PRP) is emerging as a popular augmentation technique in the context of rotator cuff repair (RCR) to strengthen the repair construct and promote healing. Previous studies examining the efficacy of PRP augmentation during RCR have described heterogeneous results. PURPOSE: To determine whether platelet concentration of a PRP injection as an adjunct of RCR impacts clinical outcomes and retear rates. STUDY DESIGN: Systematic review and meta-analysis; Level of evidence, 2. METHODS: A systematic review was performed by searching the PubMed, Cochrane Library, and E

A retrospective study on the pathology and short-term outcomes of arthroscopic repair for intratendinous, bursal-side, and articular-side tears: comparison after conversion to full-thickness tears.

Partial-thickness rotator cuff tears (PTRCTs) are classified into intratendinous tears (ITT), bursal-side tears (BST), and articular-side tears (AST). ITT is relatively rare, and many aspects of its clinical presentation remain unknown. Moreover, there are few reports on arthroscopic rotator cuff repair (ARCR) of ITTs. To the best of our knowledge, few reports have compared the treatment outcomes of ARCR with those of BST, AST, and ITT. Patients who were diagnosed with ITT based on intraoperative findings and who could be followed up for at least 1 year postoperatively were included in this st

Progressive Rotator Cuff Tears Alter Strain Across Footprint and Tear Borders: Subregion-Specific Loading Reveals Size-Dependent Mechanical Risk Zones.

Rotator cuff (RC) tears frequently involve the supraspinatus (SSP) muscle, which consists of anterior and posterior subregions with distinct functional roles. As tear size progresses, it remains unclear how force from these subregions transmits to the RC tendon stump. This study examined strain distribution across SSP tendon stump regions during isolated and subregion-specific muscle loading under increasing tear sizes. Eight fresh-frozen male cadaveric shoulders (mean age: 63 years) were tested. The humerus was fixed at 45° abduction, and the SSP muscle (whole, anterior, or posterior) was loa

Quantitative assessments of ultrasound-based rotator cuff muscle quality.

BACKGROUND: Fatty infiltration and muscle atrophy strongly influence clinical outcomes in patients with rotator cuff tears. Emerging quantitative techniques integrated with brightness-mode ultrasound and shear wave elastography (SWE) have been used to evaluate soft tissue morphology and mechanical properties. OBJECTIVE: To evaluate integrative B-mode ultrasound and SWE as a potential tool to quantify rotator cuff muscle quality. METHODS: Twenty participants (a total of 32 shoulders) with and without rotator cuff tears were included in this study. Shear wave speed (SWS) was measured in SWE-mo

Surgical vs. non-surgical therapy for partial tears of the rotator cuff: a systematic review and meta-analysis of pooled studies with indirect comparison.

BACKGROUND: Rotator cuff disease (RCD) constitutes the most common cause of shoulder pain, with partial-thickness rotator cuff tears (PT-RCTs) representing a substantial subset that may progress over time. Although both surgical and non-surgical interventions are employed in clinical practice, a consensus regarding the optimal management of symptomatic PT-RCTs is lacking, and a comprehensive synthesis of available evidence has not yet been conducted. METHODS: A systematic review was conducted according to PRISMA guidelines using MEDLINE, EMBASE, CENTRAL, ClinicalTrials.gov, and WHO ICTRP. Stu

Translational Research on Orthobiologics in the Treatment of Rotator Cuff Disease: From the Laboratory to the Operating Room.

Rotator cuff disease is one of the most common human tendinopathies and can lead to significant shoulder dysfunction. Despite efforts to improve symptoms in patients with rotator cuff tears and healing rates after rotator cuff repair, high rates of failed healing and persistent shoulder morbidity exist. Increasing interest has been placed on the utilization of orthobiologics-scaffolds, cell-based augmentation, platelet right plasma (platelet-rich plasma), and small molecule-based strategies-in the management of rotator cuff disease and the augmentation of rotator cuff repairs. This is a comple

The effects of tears in infraspinatus on other rotator cuff constituents.

The rotator cuff tear effects on glenohumeral joint tissues, such as superior labrum anterior-posterior (SLAP) lesions, have been studied experimentally or numerically in various cases. In relation to these studies, and as a novel feature of our study, infraspinatus (INF) muscle tear effects on other muscle force variations and stress and strain increases on glenoid labrum (GL), glenoid cartilage (GC) tissues, and a SLAP pathology were investigated. The ITK-SNAP Software (ISS) was used to segment the humerus and glenoid bone. The surface entities were segmented and exported to SolidWorks 2019,

Hedgehog Activation for Enhanced Rotator Cuff Tendon-to-Bone Healing.

BACKGROUND: Rotator cuff repair is a common orthopaedic procedure, yet the rate of failure to heal after surgery is high. Repair site rupture is due to poor tendon-to-bone healing and lack of regeneration of the native fibrocartilaginous enthesis. During development, the enthesis is formed and mineralized by a pool of progenitors activated by hedgehog signaling. Furthermore, hedgehog signaling drives regenerative enthesis healing in young animals, in contrast to older animals, in which enthesis injuries heal via fibrovascular scar and without participation of hedgehog signaling. HYPOTHESIS: H

An evaluation of shear wave elastographic characteristics of the supraspinatus tendon after rotator cuff repair.

BACKGROUND: The main complication of rotator cuff repair is retear, which is most common in older patients and patients with greater tear sizes. However, it is unknown why these factors are associated with increased rates of retear. The aim of this study was to determine whether the factors associated with rotator cuff retear (age, tear size, sex, history of trauma, and duration of symptoms) are also associated with decreased mechanical stiffness of the supraspinatus tendon after repair, as assessed by shear wave elastography. METHODS: This was a prospective study of 50 patients undergoing pr

Timing of retears after arthroscopic rotator cuff repair and associated factors: a retrospective analysis.

BACKGROUND: Retear after arthroscopic rotator cuff repair (ARCR) remains a complication of important concern. Few reports have evaluated retear timing and its associated patient characteristics in large cohorts. This study aimed to investigate retear timing and patient characteristics and factors associated with this parameter. METHODS: Of the 638 consecutive shoulders that underwent ARCR from August 2009 to November 2019, shoulders with retear complication within 1 year of surgery were included. Retears were defined as type IV or V of the Sugaya's classification, and magnetic resonance imagi

What are patients asking and reading online? An analysis of online patient searches for rotator cuff repair.

BACKGROUND: Patients undergoing rotator cuff surgery often search the internet for information regarding the procedure. One popular source, Google, compiles frequently asked questions and links to websites that may provide answers. This study provides an analysis of the most frequently searched questions associated with rotator cuff surgery. We hypothesize that there will be distinct search patterns associated with online queries about rotator cuff surgery that could provide unique insights into patient concerns. METHODS: A set of search terms were entered into Google Web Search using a clean

Management of partial-thickness rotator cuff tears with autologous adipose-derived regenerative cells is safe and more effective than injection of corticosteroid.

Symptomatic, partial-thickness rotator cuff tears (sPTRCT) are problematic. This study tested the hypothesis that management of sPTRCT with injection of fresh, uncultured, unmodified, autologous, adipose-derived regenerative cells (UA-ADRCs) is safe and more effective than injection of corticosteroid even in the long run. To this end, subjects who had completed a former randomized controlled trial were enrolled in the present study. At baseline these subjects had not responded to physical therapy treatments for at least 6 weeks, and were randomly assigned to receive respectively a single injec

An assessment of factors associated with early postoperative stiffness in arthroscopic rotator cuff repair.

BACKGROUND: Postoperative stiffness affects up to 20% of patients following arthroscopic rotator cuff repair. Recent evidence indicates that early postoperative stiffness may associated with lower retear rates. This study aimed to identify the independent predictors of early postoperative stiffness. METHODS: A retrospective cohort study of 1526 rotator cuff repairs was performed. Range of motion at 6-week postop was used to identify stiff patients. Stiffness was defined as external rotation <27.5o or forward flexion <145o . Multiple logistic regression analysis was used to identify the indepe

External to internal glenohumeral strength ratio in non-traumatic rotator cuff pathologies.

BACKGROUND AND STUDY AIMS: A correct agonist -antagonist strength relationship for shoulder external and internal rotation is necessary for functional stability of the shoulder. This strength relationship is described by the ratio of external to internal strength (ER/IR).The aim of this stydy is to produce comparative data as regards the ER/IR ratio in subjects with different non-traumatic rotator cuff diseases. DESIGN AND SETTING: A cross-sectional study in an outpatient clinic in a tertiary care university hospital. METHODS: In 55 subjects with rotator cuff disease (confirmed by physical e
