Spontaneous rupture of the liver, a rare but grave complication of pregnancy merits attention of an obstetrician. It is important to stress that proper antenatal care, prompt hospitalization with effective treatment at first alarming sign of preeclampsia and early induction of labour when medically indicated may prevent such a grave complication. We have described a case of malignant schwannoma (MS) originating from the posterior mediastinum with subsequent pathologic rupture of the thoracic aorta. The patient presented with acute retrosternal pain radiating to the back, and she died on the day of admission. Necropsy revealed a large hemothorax and a tumor mass involving the thoracic aorta. The histologic features of the lesion fulfilled the criteria for delineated MS. We found no similar cases of thoracic MS with consequent aorta rupture.

A case of plasma cell leukemia complicated by spontaneous rupture of the spleen is presented. Plasma cell leukemia occurs in less than 2% of patients with myeloma and is associated with an increased incidence of splenomegaly due to infiltration by malignant plasmacytes. Sontaneous splenic rupture is known to occur in patients with acute and chronic leukemia, but has been reported only once previously in a patient with plasma cell leukemia. Etiologic factors and the need for prompt diagnosis and management are discussed.

Posterior midventricular rupture occurred in 3 patients after insertion of a mitral prosthesis. Iatrogenic surgical trauma was not implicated. There were five clinical factors common to each case: a woman with mitral stenosis; a left ventricle of relatively normal size; the use of intermitent cold cardioplegia; the insertion of a porcine heterograft valve; and transient postoperative hypertension. Rupture in such cases may be caused by hyperdynamic left ventricular contraction against the strut of the prosthesis. Causes of ventricular perforation are discussed.

Leukemic reticuloendotheliosis, or "hairy cell leukemia," is a neoplasm of the reticuloendothelial system. It is characterized by the presence of many "hairy cells" in blood, marrow, lymph nodes, and spleen; by anemia, leukopenia, thrombocytopenia, and often, by massive splenomegaly. The reports of rupture of an aneurysm during carotid angiography are very few but the actual incidence is doubtless higher than recorded. The case presented is documented with the demonstration of a large middle cerebral artery aneurysm with an intratemporal hematoma, and, in sequence, the rupture of the aneurysm during the height of the injection and the subsequent extensive leakage of the contrast medium. The factors which can lead to rupture of an aneurysm during the angiographic procedure are discussed.
