The Effect of Intra-carpal Kirschner Wire Augmentation in Screw Fixation of Scaphoid - A Retrospective Cohort Study.

INTRODUCTION: Scaphoid fractures are most often treated with a single headless compression screw. However, intercarpal Kirschner wire (K-wire) might be added to improve stability and fracture outcomes. This study will determine if there is a difference in treatment outcome (union rate and time to union) between scaphoid fracture fixations using a single headless compression screw with and without augmentation using a intracarpal intramedullary K-wire. MATERIAL AND METHODS: We conducted a retrospective review of patients who underwent surgery for isolated scaphoid fractures over a 15 years per

The Etiology and Pathogenesis of Kienböck Disease.

Kienböck disease is a condition that typically occurs in the "at-risk" patient, in the "at-risk" aspect of the proximal condyle of the "at-risk" lunate. In the active male, repetitive loading causes the stress fracture that commences in the single layer proximal subchondral bone plate. The lunate fracture commences at the point the lunate cantilevers over the edge of the distal radius, and then takes on the shape of the radius. We postulate that the stress fracture violates the parallel veins of the venous subarticular plexus-leading to localized venous hypertension and subsequent ischemia and

Diagnosis of acute bone pain using isotope bone imaging.

Twenty-one patients with acute bone pain in areas other than the scaphoid were referred for isotope bone imaging directly from the A&E department. Of these scans 61% were positive. The various pathologies seen included stress fractures, osteomyelitis and bony metastases. Implications of the findings are discussed.

[Coincidental ipsilateral Colle's fracture and scaphoid fracture after low energy trauma].

Distal radius fractures are frequently seen in the emergency room. Scaphoid fractures are frequently seen as well. However, coincidental occurrence of these two types of fracture is quite rare. A 66-year-old healthy man was taken to the emergency room after a fall. He had pain in his right wrist, and an X-ray examination revealed a Colles' fracture with a 30 degree dorsal angulation and an undislocated transverse fracture of the scaphoid bone. The case illustrates that special attention should be given to the examination of the wrist and the first finger beam to exclude associated injuries.

Treatment of scaphoid non-unions with a corticocancellous graft and Herbert screw fixation: results at five years follow-up.

In a retrospective survey, 16 patients with 18 established nonunions of the scaphoid treated with a sandwich graft and Herbert screw fixation, were reviewed: 14 patients were very satisfied with the outcome. The mean DASH score was 17 (SD 18.4). Flexion was 76%, extension 79% and gripping force 85% of the contralateral side. Union was achieved in 13 scaphoid bones. There was a significant increase in carpal height. Despite these findings there was also an increase in osteoarthritis. However this was not related to the correction or absence of correction of the preoperatively existing DISI. The…

Effects of scaphoid fractures on the biomechanics of the wrist.

Scaphoid fractures are the most commonly fractured carpal bone. They most often occur from a fall on dorsiflexed, radially deviated wrist. The severity of the fall and position of the wrist influence the fracture pattern and the extent of ligamentous damage. Recent studies have correlated fracture pattern stability and any associated ligamentous disruption with the degree of alternation of wrist biomechanics. It is well documented that instability and altered kinematics can lead to nonunion and abnormal joint loads, which then can accelerate the onset of arthritis. Understanding these biomecha

Scaphoid fractures: a higher order analysis of clinical tests and application of clinical reasoning strategies.

INTRODUCTION: The purpose of this study was to identify clinical tests for scaphoid fractures and, using a higher order analysis, to determine their diagnostic accuracy. METHODS: A literature review of the databases CINAHL, Embase, Medline and PUBMED from 1980 to September 30, 2011 was conducted to obtain applicable literature on clinical tests used in identifying scaphoid fractures. Methodological quality was determined using the criteria for validity suggested by Sackett et al (2000). Using a random effects model, pooled positive likelihood ratios (PPLR) were established for any test evalua

Graft choice in the management of unstable scaphoid nonunion: a systematic review.

PURPOSE: To systematically compare outcomes between corticocancellous (CC) and cancellous-only (C-only) bone grafts for unstable scaphoid nonunions. METHODS: The English-language literature was searched using PubMed, the Cumulative Index to Nursing and Allied Health Literature, and the Cochrane Library with the following keywords: scaphoid, nonunion, malunion, unstable, collapse, humpback, dorsal intercalated segment instability, and deformity. Studies whose patient populations were characterized as possessing unstable scaphoid nonunion and/or collapse deformity or who met radiological defini

Resection of the distal pole of the scaphoid for scaphoid nonunion with radioscaphoid and intercarpal arthritis.

PURPOSE: The treatment of scaphoid nonunion with degenerative arthritis poses a clinical problem that is particularly challenging in cases of associated dorsal intercalated segmental instability collapse, radiocarpal and intercarpal degenerative changes, and poor scaphoid bone quality. The purpose of this study was to report our clinical experience performing a distal scaphoid resection for symptomatic scaphoid nonunion in patients with either radioscaphoid or intercarpal arthritis who have had multiple surgeries. METHODS: Nine patients with recalcitrant scaphoid nonunion and associated degen…

[Treatment of old scaphoid fracture with transposition of vascularized periosteal flap and internal fixation].

OBJECTIVE: To introduce an effective method of treating old scaphoid fracture. METHODS: From April 1995 to April 2002, 11 patients with old scaphoid fracture were treated with transposition of vascularized periosteal flap and internal fixation; if necessary, the radial styloid was removed. Out of 11 patients( 8 males and 3 females, aged from 17 to 46 years), the fracture sites were medial scaphoid in 7, the proximal one in 2 and the distal one in 2. The X-ray films showed separated fracture lines. The fracture lines were less than 1 mm in 4 patients and more than 1 mm in 3 patients. Sclerosis

Comparison of 2D and 3D navigation techniques for percutaneous screw insertion into the scaphoid: results of an experimental cadaver study.

Navigation in hand surgery is still in the process of development. Initial studies have demonstrated the feasibility of 2D and 3D navigation for the palmar approach in scaphoid fractures, but a comparison of the possibilities of 2D and 3D navigation for the dorsal approach is still lacking. The aim of the present work was to test navigation for the dorsal approach in the scaphoid using cadaver bones. After development of a special radiolucent resting splint for the dorsal approach, we performed 2D- and 3D-navigated scaphoid osteosynthesis in 12 fresh-frozen cadaver forearms using a headless co

Percutaneous screw fixation for scaphoid fracture: a comparison between the dorsal and the volar approaches.

PURPOSE: To evaluate the position of the screws and find the difference of clinical and radiologic outcome between the volar approach and the dorsal approach groups in percutaneous screw fixation for acute scaphoid fractures. METHODS: Forty-one consecutive patients with an acute scaphoid fracture, who had percutaneous fixation via either the volar approach or the dorsal approach, were evaluated at an average of 30 months after the surgery. The volar approach was used in 19 patients and the dorsal approach in 22 patients. By using a computerized digital image program, angles between the Herber

Stabilization of scaphoid type B2 fractures with one or two headless compression screws.

INTRODUCTION: Fractures of the scaphoid account for the most commonly injured carpal bone. Minimally displaced fractures of the waist will heal in 85-90% when using a below elbow cast. However, fractures with displacement have a higher risk for nonunion. Therefore, open reduction and fixation with headless compression screws (HCS) have become the preferred method of treatment. The aim of this study was to compare the radiological and clinical outcome of unstable scaphoid B2 type fractures, stabilized using one or two headless compression screws. PATIENTS AND METHODS: A total of 47 unstable sc

Managing Kienbock Disease's Surgical Treatment and Outcome Analysis: A Case Report and Review of Literature.

A 38-year-old man without a severe traumatic history reported to the outpatient department (OPD) with wrist pain at the dorsal aspect, mild swelling, stiffness, and restricted mobility at the left wrist joint. The patient had been experiencing these symptoms for a year. There was sharp tenderness, graded as 4 above the lunate bone, on examination of the left wrist joint. Advanced imaging, which is magnetic resonance imaging (MRI), and radiographs suggested that the patient had Kienbock's illness. Typically, the surgical approach for Kienbock's used is wrist fusion or proximal row carpectomy. Comparative study between open and arthroscopic techniques for scaphoid excision and four-corner arthrodesis.

This study presents our technique of arthroscopic scaphoid excision and four-corner arthrodesis and compares the clinical and radiological outcomes with those achieved with the open method. Twenty-seven patients (14 in arthroscopy group and 13 in open group) were included. Bone union was achieved in 13 of 14 patients in the arthroscopy group and in all 13 patients in the open group. In the open group, severe stiffness (flexion-extension arc was 10°) occurred in one patient after surgery. The mean postoperative flexion-extension arc was 75° and 51° in the arthroscopy group and open group, respe
