Preoperative lymphoscintigraphy for detection of sentinel lymph node in patients with gastric cancer--initial experience.

BACKGROUND: Preoperative lymphoscintigraphy may be informative when assessing nodal status in patients with early gastric cancer, especially for surgeons who are scheduled to introduce or have just introduced SLN biopsy. METHODS: A total of 80 patients with clinical T1N0M0 gastric cancer were injected with technetium-99 m tin-colloid and then underwent preoperative lymphoscintigraphy. The detection rate of SLN per lymphatic basin was determined on the basis of the results of confirmatory lymph node dissection. RESULTS: By means of lymphoscintigraphy, 71 of 106 basins were found (67%; 95% con

The impact of factors beyond Breslow depth on predicting sentinel lymph node positivity in melanoma.

BACKGROUND: In addition to Breslow depth, the authors previously described how increasing mitotic rate and decreasing age predicted sentinel lymph node (SLN) metastases in patients with melanoma. The objectives of the current study were to verify those previous results and to create a prediction model for the better selection of which patients with melanoma should undergo SLN biopsy. METHODS: The authors reviewed 1130 consecutive patients with melanoma in a prospective database who underwent successful SLN biopsy. After eliminating patients aged <16 years and patients who had melanomas that m…

Evaluation of intraoperative scrape cytology for sentinel lymph node biopsy in patients with breast cancer.

The techniques for intraoperative evaluation of sentinel lymph nodes (SLNs) vary. The most common methods include frozen section, imprint cytology/touch preparation cytology, and scrape cytology (SC). The purpose of this study was to evaluate whether there is concordance between the intraoperative SC and the final pathology of SLNs in patients with breast cancer. From October 2001 to June 2005, sentinel lymph node biopsies were attempted in 181 patients with breast cancer using a combination of blue dye and radioisotope. A lymph node was considered an SLN when it was stained with blue dye, had…

Effect of lymphoscintigraphy drainage patterns on sentinel lymph node biopsy in patients with breast cancer.

BACKGROUND: Sentinel lymph node (SLN) biopsy examination is an accepted method of staging breast cancer patients. SLN biopsy examination in patients with drainage to the internal mammary chain (IMC) nodes is controversial. METHODS: A prospective study of SLN biopsy examination followed by axillary dissection was analyzed to determine how surgeons manage patients with IMC drainage and the rates of axillary SLN identification and positivity in these cases. RESULTS: Lymphoscintigraphy was performed in 2196 (53.2%) of the 4131 patients in this study. IMC drainage was noted in 80 patients (3.6%).…

Micrometastasis in the sentinel lymph node of breast cancer does not mandate completion axillary dissection.

OBJECTIVE: To determine if micrometastatic disease in the sentinel lymph node is a predictor of non-sentinel lymph node (non-SLN) involvement in breast cancer. SUMMARY BACKGROUND DATA: Sentinel lymph node biopsy (SLN) is an accepted alternative to axillary dissection in staging breast cancer. If the SLN contains metastatic foci, the standard recommendation is completion axillary node dissection (CAD). However, a large subset of patients with metastasis limited to the SLN is unnecessarily subjected to the morbidity of CAD. METHODS: A retrospective review of prospectively gathered breast cance…

Sentinel lymph node mapping and biopsy for ductal carcinoma in situ and other controversial indications.

This review discusses the currently available literature regarding three controversial indications for sentinel node mapping for breast cancer patients. For women with ductal carcinoma in situ (DCIS), the use of sentinel lymph node mapping (SLNM) should be limited to women having a mastectomy. For patients with multifocal breast carcinoma, SLNM is accurate when a retroareolar injection technique is employed in the procedure. When treatment plans for node-negative patients call for neoadjuvant chemotherapy, accurate sentinel node mapping can be performed prior to the administration of chemother…

Prognostic value of sentinel lymph node biopsy in the pathologic staging of colorectal cancer patients.

BACKGROUND AND OBJECTIVES: Over the last decade, lymphatic mapping and sentinel lymph node (sN) biopsy have greatly increased the possibility of identifying nodal metastasis in clinically node-negative patients with melanoma and breast cancer, thus improving the accuracy of pathologic staging. Recently, sN biopsy has been applied also in colorectal cancer. This prospective study aimed to assess its feasibility and accuracy in predicting regional lymph nodes metastases in colorectal cancer patients as well as the impact on treatment decision-making. MATERIALS AND METHODS: Lymphatic mapping was…

Sentinel lymph node mapping for thick (>or=4-mm) melanoma: should we be doing it?

BACKGROUND: Thick (>or=4-mm) primary melanomas are believed to be associated with a high incidence of occult distant metastases. The use of sentinel lymph node (SLN) mapping and biopsy in the treatment lesions has been questioned. METHODS: A retrospective review of a computerized database identified 114 patients who underwent successful SLN mapping and biopsy from January 1, 1994, to December 31, 1999. Records were reviewed for clinicopathologic features of the patients and their tumors. Survival curves were constructed from Kaplan-Meier estimates and analyzed with log-rank tests and Cox prop…

[Staging surgery with minimal axillary invasion in breast cancer. Value of the sentinel lymph node].

UNLABELLED: Traditionally, axillary dissection has been the best way to staging patients with breast cancer, nevertheless in the early stages (I and II), without palpable axillary nodes (NO) half will not have nodal metastasis and will have been unnecessarily exposed to the morbidity of the procedure. The sentinel node (SN) has showed to be a predictor of the histologic state of the other axillary nodes, its identification allows to adequate staging of NO breast cancer patients avoiding the axillary dissection. OBJECTIVE: To get to know the SN value as predictor of axillary nodal metastasis.

Evaluation of axillary status in breast cancer. CT-based determination of sentinel lymph node size.

PURPOSE: To determine whether the location and size of sentinel lymph nodes (SLN) on CT are predictive of the axillary lymph node status in patients with breast cancer. MATERIAL AND METHODS: Forty patients with confirmed breast cancer underwent 5-mm CT of both breasts and axillae and the most inferior lymph node in the affected axilla was designated the SLN. Based on CT assessment of the axillary lymph node status, 22 (55%) patients then underwent dye- and gamma probe (DGP)-guided SLN biopsy followed by axillary dissection; 18 (45%) underwent dissection without prior SLN biopsy. The localizat…

Intraoperative sentinel lymph node mapping in non-small-cell lung cancer improves detection of micrometastases.

PURPOSE: Lymph node metastases are the most significant prognostic factor in localized non-small-cell lung cancer (NSCLC). Nodal micrometastases may not be detected with current standard histologic methods. We performed intraoperative technetium-99m ((99m)Tc) sentinel lymph node (SN) mapping in patients with resectable NSCLC. This study aimed to identify the first station of nodal drainage of operable lung cancers. Serial section histology and immunohistochemistry were used to validate the SN and to identify the presence of micrometastatic disease. PATIENTS AND METHODS: One hundred patients w…

Methylene blue dye as an alternative to isosulfan blue dye for sentinel lymph node localization.

Isosulfan blue dye has been used with increasing frequency in localizing sentinel lymph nodes in breast cancer patients. Few alternative types of dye have been investigated. In a prospective study of 30 patients, methylene blue dye was used instead of isosulfan blue dye to localize the sentinel lymph node. The methylene blue dye localization technique was successful in 90% of patients. These results are similar to those for isosulfan blue dye. This study describes methylene blue dye localization as a successful alternative to isosulfan dye in identifying the sentinel node in breast cancer pati…

Sentinel lymph node biopsy for breast cancer: the role of previous biopsy on patient eligibility.

Several reports have demonstrated the accurate prediction of axillary nodal status with radiolocalization and selective resection of sentinel lymph nodes (SLNs) in patients with breast cancer (BC). Because of concerns over lymphatic disruption, several authors have proposed that prior excisional breast biopsy is a contraindication for SLN biopsy. Clear unfiltered 99mtechnetium-sulfur colloid (1.0 mCi) was injected around the perimeter of the breast lesion (palpable and nonpalpable) or prior biopsy site. Resection of the radiolocalized SLN was then performed. Axillary lymph node dissection was …

Sentinel lymph node biopsy and axillary dissection in breast cancer: results in a large series.

BACKGROUND: Axillary lymph node dissection is an established component of the surgical treatment of breast cancer, and is an important procedure in cancer staging; however, it is associated with unpleasant side effects. We have investigated a radioactive tracer-guided procedure that facilitates identification, removal, and pathologic examination of the sentinel lymph node (i.e., the lymph node first receiving lymphatic fluid from the area of the breast containing the tumor) to predict the status of the axilla and to assess the safety of foregoing axillary dissection if the sentinel lymph node …

Omitting the sentinel lymph node biopsy: a region's perspective following recommendations from the SOUND trial.

INTRODUCTION: Sentinel lymph node biopsy (SLNB) remains the standard of care for axillary staging in early breast cancer (BC). However, its associated morbidity has prompted interest in de-escalating axillary surgery. The SOUND and INSEMA trials demonstrated that patients with small tumours and negative preoperative axillary imaging could safely omit SLNB without compromising outcomes. METHODS: This multicentre retrospective study evaluated whether SLNB influenced treatment decisions in our region in patients aged 70 years and above with small, invasive BC (<2cm) and negative preoperative axi…

Nanoplatforms for Sentinel and Tumor-Draining Lymph Node Mapping: From Visualization to Pathway-Informed Staging.

Accurate identification of sentinel lymph nodes (SLNs) and tumor-draining lymph nodes (TDLNs) is central to oncologic staging, treatment stratification, and surgical decision-making. However, conventional radiotracer- and dye-based approaches remain limited by suboptimal signal specificity, inflexible workflows, and reduced reliability in anatomically complex or therapy-altered lymphatic networks. Nanoparticle-based lymphatic tracers address these limitations by enabling controllable lymphatic transport, stable intranodal retention, and multimodal signal generation, thereby expanding the preci…
