BACKGROUND: Head-down tackling has been associated with higher rates of head and neck injuries and less successful tackles compared with head-up tackling in American football. In rugby, head and neck injuries have been associated with tackling, with the tackler's head positioned in front of the ball carrier.
PURPOSE: To assess the success and risks of tackling techniques used in the English Rugby Premiership matches.
STUDY DESIGN: Cross-sectional study; Level of evidence, 3.
METHODS: Three reviewers analyzed 1000 consecutive solo defensive tackling attempts during the 2022 to 2023 season in

INTRODUCTION: The Security Force Assistance Brigades (SFABs) are specialized units designed to strengthen allied and partnered nations through advising, supporting, liaising, and assessing in support of U.S. national security interests and combatant commanders' war fighting objectives. As the 1st SFAB was the pioneer unit, descriptive analysis of the musculoskeletal injures and body regions occurring before, during, and after deployment was previously unavailable, limiting the ability of embedded holistic health and fitness teams to proactively address the unit's musculoskeletal needs and medi

OBJECTIVE: Motor vehicle crash (MVC)-related spinal injuries result in significant morbidity and mortality in children. The objective was to identify MVC-related injury causation scenarios for spinal injuries in restrained children.
METHODS: This was a case series of occupants in MVCs from the Crash Injury Research and Engineering Network (CIREN) data set. Occupants aged 0-17 years old with at least one Abbreviated Injury Scale (AIS) 2+ severity spinal injury in vehicles model year 1990+ that did not experience a rollover were included. Unrestrained occupants, those not using the shoulder por

Arthroscopy of the shoulder is well established as a diagnostic procedure. It carries a greater degree of accuracy than conventional imaging techniques and in addition allows the assessment of instability and the degree of mechanical derangement inside the joint. Arthroscopic surgical procedures in the shoulder are in the developing stage. Arthroscopic acromioplasty for impingement syndrome has been shown to be a safe, effective procedure and probably better than conventional surgery. In the older patient, arthroscopic debridement of the rotator cuff is very effective at relieving pain. Arthro

The authors have classified tendinopathies into two categories. The majority of the tendinopathies are in the primary category in which lesions arise in the substance of the tendon. The authors emphasize that not all tendinopathies are degenerative in nature. Some primary tendinopathies are reactive, such as calcifying tendinitis.

Posterior dislocations of the glenohumeral joint are extremely rare (2-4% of all shoulder dislocations) and often associated with bone or ligamentary injuries. Though the reverse Hill-Sachs lesion is a common injury associated with posterior shoulder dislocation, there have been only few articles describing specific treatments for this type of humeral head defect. This article describes the successful operative treatment of an acute locked posterior shoulder dislocation by reconstructing the articular surface of the humeral head with the use of autologous bone graft taken from the iliac crest.

Upper extremity injuries tend to take more time to recover from, and this is even more so in a throwing athlete. Specific injuries of the shoulder and elbow that are common in the throwing athlete are discussed and how advancements in diagnostic and surgical technology have allowed a quicker, more reliable return for a throwing athlete to their previous level of activity.

Athletes, especially in contact and collision sports, are at risk for anterior and, less commonly, posterior glenohumeral instability. Studies have demonstrated that both types of instability can be successfully treated with surgery, using either open or arthroscopic techniques. For athletes with anterior instability, open stabilization has a slightly lower recurrence rate than arthroscopic stabilization. For posterior instability, arthroscopic techniques facilitate a higher rate of return to sport than open techniques. Throwing athletes have been shown to have a lower rate of return to sport

Competitive athletes are subject to several neurovascular conditions that can affect the upper extremity. These conditions are relatively rare and can be difficult to recognize but are quite important because they can seriously limit athletic performance and may even have limb-threatening consequences. The purpose of this article is to review how to identify and differentiate the major neurovascular conditions affecting the upper extremity in the athlete-patient. We also wish to highlight current concepts and treatment strategies for these problems that can help avoid serious complications and

The outstanding progress achieved in MRI and arthroscopic techniques has forced orthopedic surgeons to consider the anatomy of the shoulder joint in detail. The superior labrum complex shows a broad range of variations, which is the reason for some difficulties regarding diagnostic conclusions as well as therapeutic considerations. In particular, the differentiation between the normal and pathological adherence of the labrum is a serious clinical problem. The glenoid labrum consists of circularly arranged collagenous fibers attached to the osseous margin of the glenoid by a fibrocartilaginous

In the young athlete, anterior shoulder dislocations are common injuries that usually result in recurrent instability, and often require surgical treatment. Non-operative treatment remains the initial recommended course for most conditions. Operative treatment has advanced to more anatomical repairs, both open and arthroscopic. The purpose of this paper is to review the evaluation and treatment of anterior shoulder instability, to include acute dislocations, acute subluxations and recurrent instability.

The use of a rotation osteotomy of the humerus in 10 patients with disabling loss of lateral rotation at the shoulder due to brachial plexus birth injuries is described. The operative technique and complications are discussed. Before operation only one patient was independent, but after operation nine of them were. Restoration of lateral rotation at the shoulder can improve the function, the appearance and the independence of these handicapped patients.

BACKGROUND: Scapula fractures occur in the setting of high-energy trauma. Operative treatment of complex intra-articular scapular fractures can result in adequate surgical and functional outcomes.
MATERIALS AND METHODS: Twenty-seven patients with complex, displaced intra-articular scapular fractures with or without involvement of the scapular body, were treated operatively in a single trauma center between 2010 and 2021. Associated injuries such as ipsilateral upper limb fractures and nerve injuries were identified in the majority of the patients. Fixation with anatomical medial and lateral b

Acromioclavicular joint injuries are common and account for up to 12% of all shoulder girdle injuries and have been reported as being as high as 50% in sports-related shoulder injuries. While the majority of acromioclavicular joint dislocations can be treated non-operatively, there are certain injury configurations, which can include high-grade dislocations in overhead athletes, where surgery may be indicated. The surgical management of acromioclavicular joint instability has moved towards recreating the action of the coracoclavicular ligaments by resuspending the clavicle on to the coracoid. …

Flexion-distraction injuries to the lumbar spine frequently occur with concomitant abdominal viscus injury. We encountered 16 patients with flexion-distraction lumbar spine injuries. Half of them also suffered abdominal viscus rupture. Lap safety belts were most frequently associated with these injuries; however, the incorrect, underarm use of the shoulder harness was common in our series. The diagnosis of the spinal injury was frequently delayed when abdominal viscus injury occurred together with a flexion-distraction spinal injury. Thorough physical and radiographic examination of the spine

CONTEXT: Upper extremity injuries in baseball pitchers cause significant time loss from competing and decreased quality of life. Although shoulder range of motion (ROM) is reported as a key factor to prevent potential injury, it remains unclear how limited glenohumeral ROM affects pitching biomechanics which may contribute to upper extremity injuries.
OBJECTIVE: To investigate how pitchers with decreased total arc glenohumeral ROM of the throwing arm differed in upper extremity pitching kinematics and kinetics as well as ball velocity compared with pitchers with greater levels of glenohumeral

Rather than providing an encyclopedic review of extremity injuries, this article reviews selected serious injuries of the extremities that can be missed in the emergency department, either because they are relatively uncommon or because they are subtle in their clinical and radiographic presentation. They include injuries to the scapula, the shoulder, the forearm, the femur and hip, the knee, the tibia (which is the most common long bone fracture), and the ankle and foot. Their various causes include sports injuries, falls, and motor vehicle accidents. Several of these injuries can result in e
