[Roentgen diagnosis of the posterior part of the base of the skull].

Since the cranial nerves and the base of the skull are spatially close to each other, certain complexes of symptoms and signs give a very accurate indication of the possible location of the lesion. Basing on such neurologic-topical structuralisation, lesions of the cerebrellopontine angle, the foramen jugulare, the clivus and of the foramen magnum are discussed systematically. The various techniques of radiological imaging or examination are assessed in accordance with their diagnostic ranking, and characteristic constellations of findings are described.

Metastasis to the base of the skull: clinical findings in 43 patients.

We studied 43 patients with metastases to the base of the skull to determine whether clinical symptoms localized the lesions accurately. We identified five clinical syndromes: orbital, parasellar, middle fossa, jugular foramen, and occipital condyle. The orbital and parasellar syndromes were characterized by frontal headache, diplopia, and first-division trigeminal sensory loss. Proptosis occurred with the orbital but not the parasellar syndrome. The middle-fossa syndrome was characterized by facial pain or numbness. The jugular foramen syndrome was characterized by hoarseness and dysphagia, w

[CT of the base of the skull in bacterial meningitis (author's transl)].

CT examinations of 42 cases of bacterial meningitis revealed in 38, 1% of the cases relevant inflammatory processes at the base of the skull which were of significant importance for a transmitted infection. Such infections were: Sinusitis frontalis, ethmoidalis, maxillaris and sphenoidalis, mastoiditis or petrositis, suppurating mucocele, impression fracture, and an intracranially penetrated foreign body. Excepting the identification of fine fractures, conventional x-ray films were diagnostically superior. Hence, especially in the acute stages, special projections can be omitted, if CT is effe

Lower cranial nerve palsies due to Pseudomonas osteomyelitis of the skull base.

A patient with pseudomonas osteomyelitis of the base of the left posterior fossa is reported. His clinical course was one of progressive paresis of the left 8th, 10th, and 11th cranial nerves. There have been three prior reports of osteomyelitis of the base of the skull not in contiguity with an infected paranasal sinus. Our patient subsequently developed osteomyelitis in the 3rd to 5th cervical vertebrae.

A Cadaveric Study Evaluating the Feasibility of the Amplatzer Septal Occluder for Skull Base Reconstruction in Endoscopic Endonasal Surgery: Can Occluders Offer a Viable Alternative for Skull Base Repair?

BACKGROUND AND OBJECTIVES: Effective reconstruction of skull base defects after endonasal endoscopic surgery is essential to prevent complications such as cerebrospinal fluid leakage and infection. Traditional methods may not always provide optimal outcomes for certain defects. This study evaluates the feasibility of using the Amplatzer Septal Occluder (ASO) as an adjunctive tool for skull base reconstruction in a cadaveric model. METHODS: Three fresh cadavers were used to simulate endonasal endoscopic approaches for resecting skull base pathologies. Defects measuring 8 × 8 mm and 10 × 10 mm …

Learning curve and technical nuances of endoscopic skull base reconstruction with nasoseptal flap to control high-flow cerebrospinal fluid leakage: reconstruction after endoscopic skull base surgery other than pituitary surgery.

PURPOSE: Although reconstruction techniques after endoscopic skull base surgery have been improved, there are difficulties in reconstructing the skull base with a nasoseptal flap (NSF), especially in the case of high-flow cerebrospinal fluid (CSF) leak. The aim of this study was to analyze risk factors for the development of postoperative CSF leaks in terms of less experienced surgeon practices. METHODS: Retrospective review of medical records was performed for 125 patients who underwent endoscopic skull base surgery for intradural pathology with intraoperative high-flow CSF leakage between O…

Shape-Memory Collagen/Silk-Fibroin Scaffold for Dura Sealing and Skull Base Regeneration.

When a skull base defect occurs, reconstruction is typically performed using a variety of autologous grafts, flaps, or synthetic materials. Failure to achieve appropriate management and closure can lead to serious consequences, such as cerebrospinal fluid (CSF) leakage and subsequent meningitis. Current grafts and synthetic patches often lack biological activity, mechanical integration, and sealing capacity. Here, we developed a multifunctional, injectable composite scaffold composed of collagen/silk-fibroin (S-F) and α-tricalcium phosphate (α-TCP), designed for simultaneous CSF sealing and ti…

Unlocking surgical precision: intraoperative MRI in endoscopic skull base surgery, a meta-analysis.

OBJECTIVE: Intraoperative magnetic resonance imaging (iMRI) is increasingly used in purely endoscopic endonasal skull base surgery (EESBS) to improve surgical accuracy. Given its high cost and logistical demands, quantifying its clinical utility is essential for evidence-based implementation. This meta-analysis was conducted to evaluate the clinical efficacy of iMRI in purely endoscopic endonasal skull base surgery. METHODS: PubMed, Scopus, and Web of Science databases were searched in accordance with PRISMA guidelines. The last search was conducted on November 7, 2024. Eligible studies were o…

Feasibility study of the FRACTURE sequence in evaluating pediatric skull base bone lesions.

OBJECTIVES: This study investigated the feasibility of a fast field echo resembling a CT using a restricted echo-spacing (FRACTURE) sequence as a radiation-free alternative to CT for evaluating pediatric skull base lesions. MATERIALS AND METHODS: In this prospective study (October 2023-December 2024), 40 pediatric patients (mean age, 7.7 ± 4.0 years) with suspected skull base lesions underwent both 256-slice CT and 3.0 T MRI, including a 3D FRACTURE sequence. Two pediatric radiologists independently assessed image quality for bone and soft-tissue visualization. Bone edge sharpness was objecti…

Understanding the Symptom Burden of Complex Skull Base Tumors From the Patient's Perspective.

BACKGROUND: Symptom burden of skull base tumor (SBT) patients, especially those undergoing multimodality treatment, is poorly understood. We aim to understand symptom burden in this patient group using the core module the MD Anderson Symptom Inventory (MDASI). METHODS: Twenty-nine patients with benign and malignant SBT underwent qualitative interviews to capture symptoms at diagnosis, during, and after treatment. An expert panel of 47 members (patients, caregivers, and providers) rated identified symptoms for relevance. RESULTS: Fifty-six individual symptoms were identified. Twenty-four symp…

[Endoscopic and Combined Skull Base Surgery - The Experience of Shaare Zedek Medical Center].

INTRODUCTION: The anterior skull base forms the bony floor of the anterior cranial fossa and serves as a barrier between the sinuses and nasal cavity and the brain. The most common pathologies in this area include tumors (benign and malignant), inflammatory and infectious diseases, congenital anomalies, and traumatic injuries. Anterior skull base surgeries are performed as part of a multidisciplinary team effort. In the past, these surgeries were performed using open approaches, but with the advancement of endoscopic technology, most surgeries are now performed using minimally invasive techniq…

[Endoscopic Endonasal Surgery for Skull Base Chordomas and Chondrosarcomas : Basic to Advanced Cases].

Skull base chordomas and chondrosarcomas are rare bone tumors predominantly found in the petroclival areas and involve the cranial nerves, internal carotid arteries, and brain stem. Their clinical aggressiveness and high risk of recurrence necessitate multimodal treatments, including extensive surgical resection and high-dose radiotherapy. Surgery is commonly the first step in their treatment, and ABCD rules should be respected; anatomical preservation of the innocent nasal anatomy for potential repeated surgeries in case of recurrence, block-by-block resection, coverage of the intradural resi…

The clinical utility of 7T MRI in skull base pathology - A narrative review.

INTRODUCTION: Ultra-high field 7 T MRI provides substantially higher signal-to-noise ratio than conventional 1.5 T/3T MRI, enabling superior depiction of neuroanatomy and tissue contrast. These advantages are particularly relevant at the skull base, where complex anatomy and intimate tumour neurovascular relationships may critically impact diagnosis and treatment. This narrative review evaluates current evidence for the use of 7 T MRI in skull base pathology, with focus on tumour detection, characterisation/delineation, and the assessment of compromised neurovascular structures. METHODS: A sy…
