Impact of obstructive sleep apnea on sleep cycles and spindle dynamics: A polysomnographic study.

Sleep spindles and the temporal organization of sleep are essential for restorative and cognitive functions, yet their disruption in obstructive sleep apnea (OSA) remains poorly understood. This study examined spindle density and duration across sleep cycles in 30 OSA patients and 22 controls using full-night polysomnography. While total sleep time and stage proportions were similar between groups, sleep efficiency and arousal index worsened with OSA severity. Spindle duration in N2 was significantly reduced in moderate OSA, particularly at frontal sites, with a Group × Cycle interaction indic

Sleep bruxism and pediatric obstructive sleep apnea: exploring their chronological overlap and clinical significance.

BACKGROUND: This study aimed to investigate the temporal relationship between rhythmic masticatory muscle activity (RMMA), characteristic of sleep bruxism (SB), and apnea-hypopnea (AH) events in children and adolescents diagnosed with obstructive sleep apnea (OSA). The primary research question was whether AH events temporally precede or follow RMMA events, helping to clarify potential shared mechanisms or causal links between SB and OSA. METHODS: A retrospective review of 72 pediatric PSG records (ages 6-17) diagnosed with OSA between 2017 and 2020 was conducted. RMMA episodes were identifie

Automatic scoring of drug-induced sleep endoscopy for obstructive sleep apnea using deep learning.

BACKGROUND: Treatment of obstructive sleep apnea is crucial for long term health and reduced economic burden. For those considered for surgery, drug-induced sleep endoscopy (DISE) is a method to characterize location and pattern of sleep-related upper airway collapse. According to the VOTE classification system, four upper airway sites of collapse are characterized: velum (V), oropharynx (O), tongue (T), and epiglottis (E). The degree of obstruction per site is classified as 0 (no obstruction), 1 (partial obstruction), or 2 (complete obstruction). Here we propose a deep learning approach for a

Rosacea, poor sleep quality, and obstructive sleep apnea: A commentary on potential interconnected features.

Although rosacea is classically considered a skin disorder, recent evidence shows that it is emerging as a systemic disease. The classical symptoms of burning, intense erythema, and flushing could be related to several systemic and metabolic comorbidities. We highlight the role of sleep disturbance as a possible trigger for rosacea, which could be explained by the inflammatory and stressful conditions that can be produced by poor sleep. In particular, we call attention to obstructive sleep apnea (OSA), a common multisystemic sleep disorder; it could be linked with rosacea in the context of the

Observational Study of Obstructive Sleep Apnea in Wake-Up Stroke: The SLEEP TIGHT Study.

BACKGROUND: Wake-up stroke (WUS) accounts for a quarter of all ischemic strokes. Its conspicuous occurrence during sleep suggests that WUS may be associated with obstructive sleep apnea (OSA). We investigated the potential association among WUS, OSA, and measures of sympathetic hyperactivity. METHODS: This is a cross-sectional analysis of data from the Sleep Apnea in Transient Ischemic Attack and Stroke (SLEEP TIGHT) study. Ischemic stroke patients were divided into WUS and non-WUS groups. Participants underwent polysomnography and ambulatory blood pressure monitoring. Collected data included

More severe hypoxemia is associated with better subjective sleep quality in obstructive sleep apnea.

BACKGROUND: Perceived sleep quality may play an important role in diagnosis and therapy for obstructive sleep apnea (OSA). However, few studies have assessed factors that are associated with perceived sleep quality in OSA patients. Hypoxemia depresses the central nervous system and attenuates the perceived respiratory load in asthmatic patients. This study aimed to investigate the factors related to perceived sleep quality, focusing on the role of hypoxemia. METHODS: Polysomnography studies of 156 OSA patients were reviewed. Traditional polysomnographic parameters, including parameters of oxy

Sleepiness and sleep in patients with both systolic heart failure and obstructive sleep apnea.

BACKGROUND: Adverse effects of obstructive sleep apnea (OSA), including sleep deprivation, can contribute to the progression of heart failure. The usual indication to diagnose and treat sleep apnea is subjective sleepiness. Previous studies suggest that patients with both heart failure and obstructive sleep apnea often do not complain of sleepiness, albeit their sleep time may be reduced. Therefore, we tested the hypothesis that patients with heart failure have less sleepiness and sleep less compared with subjects without heart failure for a given severity of OSA. METHODS: Sleepiness assessed

Determinants of heart rate variability in obstructive sleep apnea syndrome during wakefulness and sleep.

Heart rate variability (HRV) is mediated by at least three primary mechanisms: 1) vagal feedback from pulmonary stretch receptors (PSR), 2) central medullary coupling between respiratory and cardiovagal neurons (RCC), and 3) arterial baroreflex (ABR)-induced fluctuations. We employed a noninvasive experimental protocol in conjunction with a minimal model to determine how these sources of HRV are altered in obstructive sleep apnea syndrome (OSAS). Respiration, heart rate, and blood pressure were monitored in eight normal subjects and nine untreated OSAS patients in relaxed wakefulness and stage

[Evaluation of sleep fragmentation by EEG spectrography in patients with obstructive sleep apnea syndrome].

Sleep fragmentation is considered to have a central role in the pathophysiology of sleep-disordered breathing. However, the evaluation of sleep fragmentation can be very troublesome work. We sought to evaluate sleep fragmentation in 10 patients with obstructive sleep apnea syndrome by utilizing fast-fourier transforms to render their sleep encephalograms as spectrograms (horizontal axis: time, vertical axis: frequency, color: power). Detailed 5-minute spectrograms revealed relatively uniform patterns of change in EEG spectra during and after apnea episodes. In non-REM sleep, delta activity dur

Transient ischemic attacks and minor stroke during sleep. Relationship to obstructive sleep apnea syndrome.

BACKGROUND: It is reported that 13% to 44% of all cerebrovascular accidents (CVAs) occur during sleep. In addition to other well-known risk factors, snoring, sleep apnea, obesity, and daytime sleepiness have been shown to significantly increase the risk of stroke. We describe two cases that support the statistical relationship between snoring, sleep apnea, and CVA during sleep. CASE DESCRIPTIONS: In the first case, motor aphasia was noted in a 64-year-old, 5-ft, 1-in, 218-lb woman when she awakened from sleep at approximately 4 AM. This completely resolved within 3 hours. During her subsequen

Advances in the Diagnosis and Treatment of Obstructive Sleep Apnea in Women.

Obstructive sleep apnea (OSA) in women is often underdiagnosed due to various and different symptoms, significant delay of referrals, sex-specific polysomnographic patterns that are usually not detected by standard severity indices from the home sleep apnea test, and limitations of current screening tools. Up to 75% of women with OSA remain undiagnosed, with relevant clinical and socioeconomic consequences. Women often report daytime fatigue, insomnia, depression, anxiety, and poor sleep quality rather than excessive daytime sleepiness or snoring, which may lead to fewer sleep clinic referrals

Development and Validation of a Screening Equation for Obstructive Sleep Apnea.

Background: The high prevalence of obstructive sleep apnea (OSA), about 30% of people worldwide over 30 years old, underscores the crucial need for early screening. This study aimed to identify key predictive factors for OSA; use these factors to develop a screening equation for a population at high risk for OSA; and prospectively validate this equation's application. Methods: The study included two phases: a retrospective phase examining anthropometric data, the Epworth sleepiness scale (ESS), and the home sleep apnea test (HSAT) from 200 patients referred to the Respiratory Sleep Disorder Ce

Lipofuscin: The Missing Link in the Worsening of Obstructive Sleep Apnea.

OBJECTIVES: There is a lack of evidence linking oxidative stress to muscle dysfunction and the worsening of obstructive sleep apnea (OSA). Lipofuscin is a permanent intracellular deposit that accumulates with aging or oxidative stress. Its increase in skeletal muscles may lead to limitations in muscle function, such as weakness and fatigue. We aim to investigate whether lipofuscin deposits in the upper airway muscles of patients with severe OSA are increased compared to those in patients with primary snoring or very mild OSA. METHODS: We studied pharyngeal framework muscle tissue samples from

Physiologic Consequences of Upper Airway Obstruction in Sleep Apnea.

OSA is diagnosed and managed by a metric called the apnea-hypopnea index (AHI). The AHI quantifies the number of respiratory events (apnea or hypopnea), disregarding important information on the characteristics and physiologic consequences of respiratory events, including degrees of ventilatory deficit and associated hypoxemia, cardiac autonomic response, and cortical activity. The oversimplification of the disorder by the AHI is considered one of the reasons for divergent findings on the associations of OSA and cardiovascular disease (CVD) in observational and randomized controlled trial stud
