Endobronchial Valve Used as Salvage Therapy in a Mechanically Ventilated Patient with Intractable Life-Threatening Haemoptysis.

Treatment options for intractable life-threatening haemoptysis in mechanically ventilated patients with structural lung disease who do not respond to bronchial artery embolisation (BAE) and who are deemed unfit for surgery are limited. A 26-year-old HIV-positive male with a poorly preserved CD4 count and active pulmonary tuberculosis was intubated and mechanically ventilated for persistent life-threatening haemoptysis. Two attempts at BAE failed, and life-threatening haemoptysis recurred daily for 14 days despite antituberculous therapy. He was deemed unfit for surgery during that period. We p

Pneumocephalus in a patient with multiple stab wounds.

Pneumocephalus is a complication of trauma to the chest and many iatrogenic interventions. It may arise due to systemic air embolism or retrograde cerebral venous air embolism which is an extremely rare complication. We report a 26-years-old female patient who presented to the Emergency Department suffering of multiple stab wounds. She was in a state of shock and after first aid and evaluation she was operated successfully. In the early postoperative period generalized tonic clonic convulsions were observed following cardiopulmonary resuscitation due to sudden cardiovascular collapse. Brain co

[Aging and age-related changes of the vitreous body].

BACKGROUND AND PURPOSE: The vitreous body is the largest component of the eye. It is a colorless, gelatinous, highly hydrated matrix that fills the posterior segment of the eye between the lens, the ciliary body and the retina. Changes in vitreal structure that occur with aging, such as vitreous liquefaction and fiber aggregation (vitreous syneresis) are important in the pathogenesis of many vitreoretinal diseases. During senescence, the vitreous volume is reduced, the vitreous body collapses and the vitreal fibers are continuously thickened, become more tortuous and surrounded by liquefied vi

Pericardial mass in a patient with rheumatoid arthritis.

A 65-year-old man presented with long-standing rheumatoid arthritis (RA), severe fatigue and mild arthritis of metacarpophalaneal joints. Physical examination revealed S3, II/IV decrescendo diastolic murmur and 2+ LL oedema. Anticyclic citrullinated peptide antibodies were >250 units. Echocardiogram showed an 8 cm pericardial mass with no atrial or ventricular collapse and mild to moderate aortic regurgitation. Cardiac MRI defined the mass as a heterogeneous entity attached to the right, anterior and inferior heart borders, with compression on right cardiac structures and the left ventricle. C

An evaluation of scaffold safety at construction sites.

PROBLEM: This study evaluated common scaffold safety practices in construction. METHOD: A 150-point checklist was used to evaluate supported scaffold safety practices at 113 scaffolds in nine areas of the eastern United States. RESULTS: Thirty-six scaffolds (31.9%) were either in danger of collapse or missing planking, guardrails, or adequate access. There was a strong statistical correlation between structural flaws and fall protection hazards, and between proper scaffold safety practice and (a) competent persons with scaffold safety training, (b) use of separate scaffold erection contracto

Unusual bleeding of aneurysmal bone cyst in the upper thoracic spine.

Aneurysmal bone cyst (ABC) is a benign bone lesion and commonly affects young adolescents. It usually grows rapidly with hypervascularity. In the spine, it can cause extensive bone destruction and compress neural structures with eventual vertebral collapse. Operative management of such a lesion may be complicated by profuse hemorrhage. Herein, we report a 15-year-old boy who had an acute exacerbation of spinal cord compression because of an ABC in the upper thoracic region. With a two-staged operation that complicating with profuse intraoperative bleeding, decompressive curettage and stabiliza

Complete pelvic ring failure after posterior iliac bone graft harvesting.

STUDY DESIGN: Case report. OBJECTIVE: To present a rare complication of posterior iliac bone graft harvesting. SUMMARY OF BACKGROUND DATA: Although iliac bone graft harvesting is a common procedure in spinal surgery, it is not without complications. One such complication is fracture of the ilium that is seen most often in older women with osteopenia. METHOD: In this case a severe complication of posterior iliac bone graft harvesting was identified in a patient with steroid-induced osteoporosis that led to multiple pelvic fracture, nonunions, and eventual failure of the entire pelvic ring. The facial features of holoprosencephaly in anencephalic human specimens. II. Craniofacial anatomy.

The craniofacial anatomy of two anencephalic human specimens with the facial features of holoprosencephaly is reported. Many of the malformations have distinctly holoprosencephalic or anencephalic features. Absence of the entire ethmoid complex, observed in both specimens of the present study, has rarely been described and, to our knowledge, has never been confirmed histologically. The significance of agenesis of the ethmoid bone in the development of normal and holoprosencephalic facial features has not been considered previously. On the basis of several morphological variations in the two sp

Giving old drugs new life: Disulfiram alleviates microglial pyroptosis and disulfidptosis-like cytoskeletal/mitochondrial alterations in cerebral ischemia-reperfusion injury.

BACKGROUND: The cerebral ischemia-reperfusion injury (CIRI) induced by endovascular treatment represents a research challenge. Microglial pyroptosis plays a critical role in the neuroinflammatory damage associated with CIRI. Gasdermin D (GSDMD) is a key executor of pyroptosis, mediating plasma membrane rupture. Interestingly, research on pyroptosis has long focused on cell membrane rupture. We further hypothesize that cytoskeletal collapse during pyroptosis may facilitate cell rupture. This new perspective will contribute to a comprehensive understanding of the mechanisms of morphological chan

Mutant SOD1 expressed by oligodendrocytes aggregates in myelinic nanochannels and accelerates disease progression in familial ALS mice.

UNLABELLED: Amyotrophic lateral sclerosis (ALS) is a highly debilitating and fatal disease characterized by the progressive loss of motor neurons. Reduced oligodendroglial support has been implicated in ALS progression but remains mechanistically unexplained. Here, using a mutant superoxide dismutase 1 (SOD1-G37R) mouse model of familial ALS, Cre-mediated excision of the mutant SOD1 gene within the oligodendrocyte lineage prior to myelin compaction is shown to slow disease onset, improve motor performance, and prolong survival. In contrast, silencing mutant SOD1 expression within oligodendrocy

Repair and regeneration across the lifespan: an ontogenetic perspective.

The capacity for tissue repair and regeneration undergoes a profound and progressive decline across the human lifespan, representing a fundamental driver of aging and chronic disease. This review establishes a comprehensive ontogenetic framework by mapping the continuous biological transition from the flawless, scarless regenerative plasticity of embryonic development to the irreversible fibrotic scarring and organ failure characteristic of senescence. We synthesize the hierarchical collapse of reparative networks across multiple biological scales. Importantly, this ontogenetic decline should

A Perovskite Memdiode-Based Neuromorphic in Silico Surrogate Model for Emulating Predictive Coding Failure in Diabetic Neuropathy.

Halide perovskite memdiodes have coupled ionic-electronic dynamics and are promising candidates for artificial synapses in neuromorphic computing. We provide an in silico neuromorphic circuit that includes a comprehensive perovskite memdiode model and confirm its synaptic plasticity repertoire through simulation-based validation. The proposed model recapitulates analog long-term potentiation/depression (LTP/LTD), spike-timing-dependent plasticity (STDP), spike-rate-dependent plasticity (SRDP), and paired-pulse facilitation/depression (PPF/PPD). We also describe pulse-amplitude- and pulse-width

Proposal of Classification for Management of Molar Extraction Sockets and Decision Making for Immediate Implant Placement: A Technical Report.

In cases where adequate primary stability (PS) is established, the immediate implant placement (IIP) in molar sockets is considered a reliable treatment option that significantly reduces the overall treatment time. Several key factors contribute to the success of IIP in molar sockets, notably the presence of sufficient inter-radicular septa, which is essential for achieving the necessary PS in cases of elongated roots and also preventing buccal plate collapse. Furthermore, the properties of the buccal bone plate (BBP) may have a significant impact on the final morphology of the ridge following

Stem cell therapy combined with core decompression versus core decompression alone in the treatment of avascular necrosis of the femoral head: a systematic review and meta-analysis.

INTRODUCTION: Accumulated clinical trials had been focused on stem cell therapy in combination of core decompression (CD) in the treatment of avascular necrosis of the femoral head (ANFH). Nonetheless, the results were inconclusive. Here, we performed a systematic review and meta-analysis of previous randomized controlled trials (RCTs) and retrospective studies to assess whether combined stem cell augmentation with CD improved the outcomes of ANFH compared with CD alone. METHODS: The current study included 11 RCTs and 7 retrospective studies reporting the clinical outcomes of a total of 916 p

[Osteoporotic vertebral crush fractures with severe neurologic manifestations. Apropos of 6 cases].

Osteoporotic vertebral crush fractures with neurologic complications are rarely reported in the literature. We report six new cases particularly severe in which death occurred in two cases. The study group included four women and two men with a mean age of 75 years (range: 72-79). Vertebral collapse causing neurological deficit was T5, T9, T11 in two cases, L1 and L3. The mean number of vertebral collapses was three per patient (range: 1-9). Back pain appeared without traumatism 6 weeks before admission (range: 1-24). Neurological complications appeared 2.5 weeks after back pain (range: 1-8).

Bubble-Guided Self-Healing Superhydrophobic Surfaces with Dual-Mode In Situ Visual Monitoring.

Superhydrophobic surfaces exhibit great application potential ranging from sophisticated droplet manipulation to aerospace and other fields, owing to their unique properties such as liquid repellent, anti-corrosion, anti-icing, drag reduction, and so on. However, maintaining the stability of the solid-liquid interface remains a major challenge: the instability of the three-phase line caused by the collapse of the air layer during dynamic transport and the degradation of surface function caused by the retention of micro-nano contaminants seriously undermine its durability. In nature, rice leave

Surgical Correction of the Crooked Nose: A Procedure Proposal.

A crooked nose, resulting from the collapse of cartilaginous and/or bony support, often presents with depression of the nasal tip and middle third. This condition is frequently associated with septal deformities that impair breathing and cause damage to the upper lateral cartilages and bony framework. Traditional surgical techniques typically involve septal correction and the use of cartilage grafts to restore structure and projection, though such methods carry a risk of fibrosis and recurrent deformity. We present an alternative surgical approach that reconstructs the nasal pyramid without th

Mitochondria-targeted tertiary amine N-Oxide nanoparticles with photosensitizer conjugation for photopyroptotic cancer therapy.

Although photodynamic therapy has emerged as a promising strategy for cancer treatment, its efficacy is often restricted by the extremely short diffusion radius of reactive oxygen species (ROS). Concentrating ROS generation within vulnerable organelles, such as mitochondria, represents an effective avenue to overcome the limited immunogenicity often associated with conventional apoptosis and amplify photodynamic damage. To address this challenge, this study introduces a zwitterionic polymer, poly(2-(N-oxide-dimethylamino)ethyl methacrylate) (OPDMA), featuring a tertiary amine N-oxide (TANO) st
