Deep brain stimulation for psychostimulant use disorders.

Safe and effective therapeutics for psychostimulant use disorders remain elusive. Deep brain stimulation (DBS), which is FDA-approved for other indications, is a promising candidate for treating severe substance use disorders. We examine the clinical and preclinical evidence for DBS of the nucleus accumbens as a possible therapeutic option for cocaine and methamphetamine use disorders. Limitations of the literature to date, including the lack of females included in studies evaluating the efficacy of DBS, and new strategies to optimize brain stimulation approaches are also discussed.

Treating cocaine and opioid use disorder with transcranial magnetic stimulation: A path forward.

Developing new, effective treatments for substance use disorders (SUDs), especially cocaine and opioid use disorders (CUD and OUD), are of immense importance. These are chronic, relapsing brain diseases characterized by dysregulated circuits manifesting from neuroplastic change brought on by repeated exposure to substances of abuse. A potential treatment is therapeutically inducing neuroplastic change in targeted dysregulated circuits. One such intervention, repetitive transcranial magnetic stimulation (rTMS) has gained traction over the past two decades as a method of noninvasively stimulatin

Neurobiology of cue-reactivity, craving, and inhibitory control in non-substance addictive behaviors.

Non-substance addictive behaviors may involve repetitive rewarding behaviors including gambling, gaming, sexual activities, internet-use, and buying-shopping. In addictions, there exists diminished control over participation in the behaviors and continuation of the behaviors despite experiencing negative consequences. Addictive participation in behaviors typically results in distress and functional impairment in personal, relational, occupational, educational, or other life domains. Increases in cue-reactivity and craving and decreases in inhibitory control may represent pivotal mechanisms in

Prevalence and correlates of substance use among psychiatric inpatients.

PURPOSE: This study aimed at examining correlates of substance use among patients diagnosed with psychiatric disorders in Jordan. METHODS: This study used a nonexperimental descriptive survey design. Data were collected through using DUSI-R scale obtained through a self-designed questionnaire. A total of (203) subjects satisfied the inclusion criteria. RESULTS: Most used substances prior to admission were caffeine (78.2%), cigarette (60.9%), and alcohol (22.2%), while the least were heroin (1.2%) and inhalants (0.08%). Patients diagnosed with schizophrenia had higher percentages among all ot

Comparing immediate- and delayed-onset posttraumatic stress disorder: Results from the National Epidemiologic Survey on Alcohol and Related Conditions-III (NESARC-III).

Delayed-onset posttraumatic stress disorder (PTSD) is defined as a period of 6 months or more between trauma exposure and episode onset. Due to the limited research and lack of epidemiological studies on this form of the disorder, we investigated its prevalence, clinical features, and psychiatric comorbidities in a nationally representative sample of U.S. adults. Using National Epidemiologic Survey on Alcohol and Related Conditions-III (NESARC-III) data collected from individuals who met the DSM-5 criteria for PTSD (N = 1,980), we compared delayed- and immediate-onset PTSD with regard to demog

Psychopathology in chronic cocaine abusers.

A group of 30 hospitalized cocaine abusers were studied, along with 124 hospitalized patients who were dependent upon opiates or central nervous system depressants. DSM-III diagnoses, family history data, demographic characteristics, and measures of current depressive symptomatology were compared in the two groups. Nineteen (63%) of the cocaine abusers met criteria for an Axis I diagnosis other than substance abuse; sixteen (53%) had affective disorder. These figures reflected a significantly higher prevalence rate of affective disorder among the cocaine abusers than among the opiate and depre

Psychopathology and suicide method in mental health care.

BACKGROUND: Not all suicide methods are evenly distributed among different psychiatric disorders. METHODS: In a nationwide sample of 505 suicides by persons in mental health care, the relationship between psychiatric diagnosis and suicide method was examined with chi(2) tests, logistic regression analyses and multinomial logistic regression analysis, including interactions with age, gender and treatment status. RESULTS: Psychotic disorders were associated with jumping from heights, and substance-related disorders were associated with self-poisoning. Depressive disorders were not associated w…

A New Tool to Tackle the Opioid Epidemic: Description, Utility, and Results from the Maine Diversion Alert Program.

STUDY OBJECTIVE: The opioid epidemic continues to impact the United States, and new strategies are needed to combat this epidemic. The objective of this study was to analyze 2015 drug arrest data from Maine's Diversion Alert (DA) program (diversionalert.org), compare arrest data between the first quarters of 2015 and 2016, and provide an analysis of the sex differences in the arrests as well as information about DA use. STUDY DESIGN: This was a population-based descriptive study using data from the Maine DA program. The study population consisted of persons arrested for prescription, nonpresc

Reasons for Benzodiazepine Use Among Persons Seeking Opioid Detoxification.

BACKGROUND: Over the past decade, patients admitted to addiction treatment programs have reported increasing rates of concurrent opioid and benzodiazepine (BZD) use. This drug combination places individuals at high risk for accidental overdose. Little is known about reasons for BZD use among individuals seeking treatment for opioid use disorders. METHODS: We surveyed consecutive persons initiating inpatient opioid detoxification and identified 176 out of 438 who reported BZD use in the past 30 days and/or had a positive toxicology. RESULTS: Forty percent of persons surveyed used a BZD in the

Experiments with cocaine and heroin addicts--are they predictive?

Treating cocaine addiction using dopamine replacement strategies, treats withdrawal but not relapse. Experiments with diverse pharmacological agents shows involvement of multiple pharmacologies and new approaches are emerging to treat the drug seeking behaviour and craving associated with relapse. Neuropathological studies are showing structural and connectivity changes in the brain of addicts which appear permanent, making control of learned behaviours associated with these changes extremely challenging. Heroin addiction is treated successfully with opiate replacement strategies but relapse a

Excluding a psychoactive substance use disorder in forensic psychiatric evaluations.

The forensic psychiatrist is sometimes asked to exclude that a person has a psychoactive substance use disorder, for example, in a security worker who has access to weapons, in a health care professional who may be alcohol/drug impaired, or in a parent, in a deprived child or custody hearing matter. After examining the data that are leading to the evaluation, these evaluations require corroborated background information to look for developmental and genetic antecedents that might be consistent with substance abuse and dependence; inquiry into the history of substance use; and an examination of
