Excising the Extra: A Scoping Review of Direct Excision Safety, Complications, and Prevention Strategies in Facial Aesthetic Plastic Surgery.

PURPOSE: Direct excision is a foundational technique. It has applications across the periorbital, nasal, and cervical regions. Our goal is to define the safety profile of direct excision and compile best practices for optimizing outcomes. To our knowledge, it has not been clearly characterized in the literature. METHODS: A structured literature search of PubMed, Embase, and Google Scholar identified studies published between 2011 and 2026 that evaluated the complications, safety, and outcomes of direct excision in facial aesthetic surgery. Following PRISMA guidelines, 11 studies were included

The Incidence of Retained Objects in Intraoperative X-rays for Missing Counts in Plastic Surgery: We Should Do Better.

BACKGROUND: In the event of incorrect surgical counts, obtaining X-rays to rule out retained surgical items (RSI) is standard practice. However, these safeguards also carry risk. This study investigates the actual incidence of RSI in plastic reconstructive surgery (PRS) cases as measured on intraoperative X-rays and its associated modifiable risk factors. METHODS: X-rays with indication of "foreign body" in PRS procedures from 2012 to 2022 were obtained. Reports with "incorrect surgical counts" and associated perioperative records were retrospectively analyzed to determine the incidence of re

Blepharoplasty Approach to the Orbital Rim: A Novel Approach for Select Patients Presenting for Gender Affirming Facial Plastic Surgery.

BACKGROUND: Gender affirming facial surgery is a valuable option for patients with gender dysphoria. Traditionally, procedures for the upper third of the face focus on hairline position and contour of bony prominences via a coronal or trichophytic incision. These traditional approaches require large incisions and extensive dissection, which is not desirable for many patients and potentially avoidable in specific situations. METHOD: We utilized a standard blepharoplasty approach to access the superior orbital rim in patients who desired isolated lateral orbital contouring as part of their gend

Wound healing in plastic surgery: does age matter? An American College of Surgeons National Surgical Quality Improvement Program study.

BACKGROUND: Increasing age has traditionally been associated with impairment in wound healing after operative interventions. This is based mostly on hearsay and anecdotal information. This idea fits with the authors’ understanding of biology in older organisms. This dictum has not been rigorously tested in clinical practice. METHODS: The American College of Surgeons National Surgical Quality Improvement Program database was retrospectively queried for all patients undergoing plastic surgery from 2005 to 2010. Variables extracted included basic demographics, comorbidities, previous steroid and

Analysis of the increasing prevalence of necrotising fasciitis referrals to a regional plastic surgery unit: a retrospective case series.

Necrotising Fasciitis is a destructive infection of the skin and subcutaneous tissues associated with significant mortality and morbidity. Survival from the condition often necessitates patient referral for appropriate reconstructive surgery and supportive medical management. The aim of our study was to identify emerging patterns, characteristics and outcomes of necrotising fasciitis in Northern Ireland. A retrospective analysis of all patients referred to the Regional Plastic Surgery Service in Belfast between 2007 and 2012 was performed. Forty-six patients were identified with clinical, intr

The self-lined superiorly based pull-through velopharyngoplasty: plastic surgery-speech pathology interaction in the management of velopharyngeal insufficiency.

We present a rationale and step-by-step description of a previously unpublished innovative surgical technique designed to overcome velopharyngeal insufficiency. This procedure maintains the anatomic integrity and physiologic function of the velopharynx by limiting interpalatal dissection and incorporates a method for flap attachment through the velum that is accessible, predictable, and versatile. The efficacy of this procedure in eliminating velopharyngeal insufficiency was evaluated by using rigorously controlled quantitative psychophysical scaling procedures of presurgical and postsurgical
