OBJECTIVES: We evaluated late-term results of surgical repair and physical rehabilitation of isolated zone 3 extensor tendon injuries.
METHODS: Sixteen patients (13 males, 3 females; mean age 28 years; range 11 to 57 years) underwent surgical repair for isolated simple central slip injuries of zone 3 extensor tendon. Injuries involved the second, third, fourth, and fifth fingers in seven, three, two, and four patients, respectively. Primary tendon repair was performed in 14 patients. Two patients were treated after one week and two months following primary injury, respectively. Tendons were r

BACKGROUND: Effective soft tissue graft fixation to the tibial tunnel in all-inside anterior cruciate ligament reconstructions has been reported to be a problem and may lead to retrograde pullout at ultimate load testing.
HYPOTHESIS: A combined retrograde bioabsorbable screw and cortical-cancellous suture button suspension apparatus would gain stiffness from the button and strength from the screw, thus providing for a larger pullout ultimate load, yield load, and stiffness when compared with either fixation alone in an all-inside anterior cruciate ligament reconstruction.

STUDY DESIGN: An analysis of consecutive pediatric patients treated surgically for high-grade spondylolisthesis by one of three surgical procedures with emphasis on complications and functional outcomes.
OBJECTIVE: Complications, radiographic results and patient-assessed function, pain, and satisfaction were assessed among three surgical procedures.
SUMMARY OF BACKGROUND DATA: The existing literature is in disagreement about whether it is better to fuse without instrumented reduction or to use instrumentation and reduce high-grade dysplastic spondylolisthesis.
METHODS: Thirty-two patients h

PURPOSE: There is a growing interest in developing new animal models to better understand the postoperative response of hernia meshes, which are key to improving device evaluation and clinical outcomes. This study presents an animal program using a porcine model to evaluate mesh response at the early postoperative period through in vivo imaging and and ex vivo pressure-based bench testing, comparing a lightweight flatsheet (LWF), a heavyweight flatsheet (HWF) and a self-fixating mesh (SFM).
METHODS: Partially radiopaque meshes were implanted in vivo in twelve pigs. Two surgical models were co

PURPOSE: Olecranon fracture is a common upper limb fracture, and several surgical approaches have been advocated for its fixation. To overcome the complications associated with common techniques, we present a novel shape-memory alloy concentrator, an alternative for tension band compression, to fix olecranon fracture.
METHODS: Fifty-seven patients (26 men and 31 women) with olecranon fracture, with a mean age of 45 years, were included in this study. Each patient had undergone open reduction and internal fixation using the Nitinol (Ni-Ti) arched shape-memory connector (ASC). The clinical asse

Distal femoral fractures have typically a bimodal occurrence: in young people due to a high-energy trauma and in older people related to a low-energy trauma. These fractures are associated to a very high morbidity and mortality in elderly. Distal femoral fractures might be treated with plates, intramedullary nails, external fixations, and prosthesis. However, difficulties in fracture healing and the rate of complications are important clinical issues. The purpose of this retrospective review was to present our experience in treatment of distal femoral fracture in a sample of older people in or

OBJECTIVE: Proximal attachment failure, often leading to graft migration, is a severe complication of endovascular aneurysm repair (EVAR). Aortic cuffs have been used to treat proximal attachment failure with mixed results. The Zenith Renu AAA Ancillary Graft (Cook Inc, Bloomington, Ind) is available in two configurations: converter and main body extension. Both provide proximal extension with active fixation for the treatment of pre-existing endovascular grafts with failed or failing proximal fixation or seal in patients who are not surgical candidates. We prospectively compared the outcomes

INTRODUCTION AND OBJECTIVES: In cases where adjacent segment disease (ASD) develops following lumbar fusion surgery, various surgical approaches can be employed. In such cases, removal of the instrumentation can positively impact lumbar pain syndrome. One frequently used method is short-segment fixation-decompression (SSFD). One of minimally invasive methods is microsurgical bilateral decompression via unilateral approach (MBDU). This study aims to determine which method is more effective and safe in cases that developed ASD following fusion surgery by comparing SSFD and MBDU after instrumenta

We present a case of a 25-year-old male patient suffering from severe prolonged pain after uneventful pectus excavatum repair that could be treated successfully by paravertebral nerve radiofrequency thermoablation. The patient was scheduled for a minimally invasive Nuss pectus excavatum repair. Surgical correction was performed under general anesthesia in combination with a thoracic peridural catheter. The immediate postoperative course was uneventful; however, the patient developed severe prolonged bilateral chest wall pain across segments T8 and T9. After failure of conservative treatment op

BACKGROUND: Implant removal is necessary in up to 25% of patients with plate osteosynthesis after proximal humeral fracture. Our new technique of arthroscopic implant removal offers all the advantages of minimally invasive surgery. This study outlines the first results after arthroscopic implant removal in comparison with those of open implant removal.
MATERIAL AND METHODS: Twenty patients [median age 64 (30-82) years] had arthroscopic and nine patients [median age 53 (34-76) years] had open implant removal. Median 9.5 months after implant removal subjective patient satisfaction, Constant-Mur

OBJECTIVE: Percutaneous reduction by distraction and subsequent percutaneous screw fixation to restore calcaneal and posterior talocalcaneal facet anatomy. The aim of this technique is to improve functional outcome and to diminish the rate of secondary posttraumatic arthrosis compared to conservative treatment and, secondly, to reduce infectious complications compared to open reduction and internal fixation (ORIF).
INDICATIONS: Sanders type II-IV displaced intraarticular calcaneal fractures.
CONTRAINDICATIONS: Isolated centrally depressed fragment. Patients who are expected to be noncomplian

During the skeletal development of the axis and foot of the child physiological specialties can occur. Those are thick fatty sole, increased valgus in the ankle joint, tibial varus as a toddler with first varus, later on valgus in the knee, increased laxity of the ligaments, increased anteversion of the femoral neck with "kneeing in" and "toeing in", and with knees first in flexion, later on hyperextension. The knowledge of those specialties is important to be able to diagnose and treat deformities of the foot during childhood. Included are club foot, flexible flatfoot, vertical talus, pes add

We retrospectively reviewed 57 patients with L4--L5 degenerative spondylolisthesis (L4--L5 DS) who underwent posterior decompression and posterolateral fusion of L4--L5 without (Group A) or with (Group B) transpedicular screw instrumentation at least 2 years earlier. The clinical results and fusion rate were similar between Groups A and B, that is, a 72.4% satisfactory outcome with a fusion rate of 82.8% in Group A versus 82.1% satisfactory outcome with a 92.8% fusion rate in Group B. Screw instrumentation reduced postoperative low back pain and resulted in a lordotic slip angle of L4--L5. How

For the treatment of chronic sepsis of the hip in paraplegic patients, we adopted three measures: (1) a Girdlestone procedure, (2) transposition of the vastus lateralis muscle into the void that was left by the removal of the femoral head and neck and the acetabular wall, and (3) external fixation to prevent unrestrained motion of the femoral shaft, which might damage the transposed muscle. The hip joint was spanned by a posterior pelvic-femoral skeletal external fixator. Nine patients, all of whom had thoracic-level paraplegia, were treated in this manner. The fixator was kept in place for th

BACKGROUND: Soft tissues (wound dehiscence, skin necrosis) and septic (wound infection, osteomyelitis) complications have been historically recognized as the most frequent complications in surgical treatment of high-energy proximal tibia fractures (PTFs). Staged management with a temporary external fixator is a commonly accepted strategy to prevent these complications. Nonetheless, there is a lack of evidence about when and how definitive external or internal definitive fixation should be chosen, and which variables are more relevant in determining soft tissues and septic complications risk. BACKGROUND: Interspinous fixation devices (IFDs) provide indirect decompression and stabilization for lumbar spinal stenosis (LSS) and are potentially less invasive than open decompression, but long-term comparative data are limited.
OBJECTIVE: To evaluate the long-term clinical efficacy, safety, and perioperative outcomes of indirect decompression and fusion with an IFD versus standard open surgical decompression in patients with LSS.
METHODS: 48 patients were randomly assigned to IFD or decompression. Primary endpoints were changes in Oswestry Disability Index (ODI), Visual Analogue Scale

Distraction osteogenesis is a powerful stimulus for new bone (and tissue) generation (anabolism). Biomechanical stimulation by distraction of the regenerate region results in a high rate of tissue and bone production. However, catabolism (bone resorption) can also occur, the process potentially accentuated in a stress-shielded environment of an external fixator. Regenerate insufficiency can result in regenerate bending or fracture after frame removal. Experimental evidence has demonstrated that bisphosphonates may mediate improved local limb bone mineral density (BMD) and regenerate strength i
