Review of human supraspinatus tendon mechanics. Part I: fatigue damage accumulation and failure.

Repetitive stress injuries to the rotator cuff, and particularly the supraspinatus tendon (SST), are highly prevalent and debilitating. These injuries typically occur through the application of cyclic load below the threshold necessary to cause acute tears, leading to accumulation of incremental damage that exceeds the body's ability to heal, resulting in decreased mechanical strength and increased risk of frank rupture at lower loads. Consistent progression of fatigue damage across multiple model systems suggests a generalized tendon response to overuse. This finding may allow for interventio

Treatment of periscapular tendinopathy with radiofrequency coblation: A case report.

Overuse injuries of the tendon - 'tendinopathy' - account for 30%-50% of all sporting injuries and a high proportion of orthopaedic referrals from primary care physicians. Tendinopathies often have a multifactorial aetiology and injury can be due to a combination of both acute and chronic trauma which contributes to loss of tissue integrity and eventual rupture. Our incomplete understanding of the mechanisms surrounding tendon pathophysiology continues to cause difficulties in treatments beyond loading regimes which can be unsuccessful in up to 30% of cases. We describe an uncommon case of ten

[Treatment of Acute and Chronic Ruptures of the Extensor Mechanism of the Knee Joints].

Ruptures of the extensor mechanism of the knee joint are rare but disabling injuries. Injuries occur on the basis of degeneration or acute trauma. The understanding of anatomy and biomechanical function plays an essential role for choosing the right diagnostic methods and initiating successful treatment. Besides clinical examination ultrasound is a cost efficient primary diagnostic measure. Additional MRI can be beneficial for partial tears or chronic ruptures. Non-operative treatment is suitable for minor partial ruptures with sustained function. The most common methods for reattachment of th

Free partial latissimus dorsi myocutaneous flap for coverage of severe achilles contracture in children.

Achilles tendon and soft-tissue contractures caused by trauma to the foot and ankle are complex injuries. An array of techniques, including local and distant flaps, has been used and described to reconstruct these challenging wounds. However, the management of these injuries in a growing child who develops abnormal gait due to equinus deformity has not been categorically reported. The latissimus dorsi myocutaneous flap has the advantage of a rich blood supply making specific partial muscle harvest possible. In this case series, we report on five paediatric patients with severe contracture of t

Clinical application of distal ulnar artery perforator flap in hand trauma.

The clinical value of distal ulnar artery perforator flap in composite defects of the hand is demonstrated in a case series of nine patients with severe injuries of the hand. Soft tissue loss with a mean diameter of 3 × 4 cm on the dorsum of the hand, volar and dorsal side of the wrist and palm was reconstructed with this flap. The mean size of flaps was 5.2 × 5.2 cm. Tendon and nerve injuries, and metacarpal fractures accompanied the soft tissue loss in seven patients. Subsequent reconstructive procedures were; Hunter rod placement and tendon grafting in two patients, nerve grafting in one, p

Ring Finger Flexor Digitorum Superficialis Hemi-Tendon Transfer for Fifth Finger Flexor Digitorum Profundus Reconstruction Technique: Preliminary Cadaveric Study and Case Report.

Delayed tendon injury has been historically treated with several techniques, including staged tendon graft and flexor digitorum profundus (FDP) transfers from neighboring rays. Nevertheless, the use of the flexor digitorum superficialis (FDS) hemi-tendon from neighboring fingers may provide the same results with less donor site morbidity. This article aims to present the safety and effectiveness of the fourth FDS hemi-tendon transfer technique to treat zone I-5th FDP injuries on cadaver specimens, and to describe its in vivo application. In 4 fresh-frozen forearms, the fifth FDP was cut at zon

Evaluating the Biomechanical Effects and Real-World Usability of a Novel Ankle Exo for Runners.

Achilles tendon overuse injuries are common for long-distance runners. Ankle exos (exoskeletons and exosuits) are wearable devices that can reduce Achilles tendon loading and could potentially aid in the rehabilitation or prevention of these injuries by helping to mitigate and control tissue loading. However, most ankle exos are confined to controlled lab testing and are not practical to use in real-world running. Here, we present the design of an unpowered ankle exo aimed at reducing the load on the Achilles tendon during running while also overcoming key usability challenges for runners outs

Editorial Commentary: Kartogenin Promotes Wounded Enthesis Regeneration.

Entheses are the insertion sites where tendons and ligaments attach to bone. Commonly, entheses are subject to overuse injuries, and tendon-to-bone healing is poor because the healing has occurred between 2 different tissues: hard tissue (bone) and soft tissue (tendon). It is necessary to form the zonal arrangement of the enthesis region in vivo after repair.

Loss of type III collagen has limited effect on tendon healing in young adult mice.

Tendon injuries account for considerable and escalating clinical burden. After injury, tendons heal poorly with persistent fibrovascular scar. Unlike healthy tendon which is type I collagen (COL1)-rich and highly-aligned, fibrovascular scar is type III collagen (COL3)-rich and disorganized; this compositional and organizational change has been implicated in the functional deficits resulting from tendon injury. Accordingly, COL3 is historically considered a primary driver of poor tendon healing. Given compelling evidence of COL3's role in regulating matrix structure and cell behavior, we sought…

Magnetic resonance imaging for the evaluation of acute posterolateral complex injuries of the knee.

Clinical evaluation of posterolateral complex injuries of the knee can be difficult. To determine if magnetic resonance imaging can assist in decision-making in the treatment of posterolateral complex injuries, six consecutive patients with acute posterolateral knee trauma were imaged preoperatively with standard magnetic resonance imaging sequences. Magnetic resonance imaging findings were then correlated with results of examination under anesthesia or open lateral reconstruction. There were five complete lateral complex injuries (grade III) and one partial tear. Magnetic resonance imaging wa

Peroneal tendoscopy: indications and techniques.

Endoscopy of the tendons (tendoscopy) of the foot and ankle has become more frequently performed and reported. New, smaller endoscopic cameras and instruments have made tendoscopy more accessible and useful for sports medicine and related surgeries. The peroneal tendons are particularly accessible for tendoscopy owing to their subcutaneous position along the lateral ankle. Tendoscopy of the peroneal tendon sheaths can be used for diagnosis and minimally invasive treatment of certain disorders. In cases where open procedures are indicated, surgical exposure can be limited by tendoscopic identif

[The incidence of hand trauma the COVID-19 epidemic setting].

OBJECTIVES: In France, a lockdown was enforced from March 17 to May 11, 2020. It was renewed with different modalities from April 3 to May 3, 2021. Our objective was to compare the epidemiology of hand and wrist trauma injuries during these periods to a control period, with the hypothesis of a decreased incidence of hand and wrist trauma. METHODS: Patients consulting at a trauma emergency center of a university hospital labeled SOS-Mains were included during lockdowns, and were compared with a control group who consulted during an equivalent period in 2019. We retrospectively collected demogr

Combined Open Sagittal Hood Repair and Metacarpophalangeal Joint Arthroscopy: A Comprehensive Approach to Complex Knuckle Injuries.

Isolated management of metacarpophalangeal (MCP) joint injuries or sagittal band disruptions often leads to incomplete symptom resolution and suboptimal functional outcomes, particularly in complex trauma. We present a combined surgical technique integrating MCP joint arthroscopy with open sagittal hood repair for traumatic extensor mechanism instability and intra-articular pathology. This paper describes a stepwise approach to address both intra-articular and soft tissue components of MCP joint injuries. Arthroscopy facilitates direct visualization, synovectomy, debridement, and capsular shri

MR imaging appearance of the extensor mechanism of the knee: functional anatomy and injury patterns.

Trauma to the extensor mechanism of the knee, a common clinical problem, can be accurately evaluated with magnetic resonance (MR) imaging. The extensor mechanism consists of the quadriceps muscle and tendon, patella, patellar tendon, and patellar retinacula. Injuries of these structures can be classified into partial and complete tears. Acute injuries are associated with edema, hemorrhage, and fluid collections; chronic injuries often demonstrate redundancy, atrophy, and retraction of the affected structures. MR imaging is useful in differentiating partial and complete tears and in evaluating

Pitfalls and complications of flexor tendon surgery.

In both new and old tendon injuries, errors and hidden dangers exist that may lead to later complications. These arise at all four phases of primary repair or secondary reconstruction, namely preoperative assessment, exposure and evaluation, repair or reconstruction, and, finally, postoperative care. Often, the resulting complications are the same following tendon suture or tendon graft, the most common being adhesion with joint contracture. The list of complications also includes rupture, bowstringing, skin necrosis, infection, recurvatum, the lumbrical plus finger, and the quadriga syndrome.

Hamstring injuries.

Hamstring injuries are a frequent injury in athletes. Proximal injuries are common, ranging from strain to complete tear. Strains are managed nonoperatively, with rest followed by progressive stretching and strengthening. Reinjury is a concern. High grade complete tears are better managed surgically, with reattachment to the injured tendon or ischial tuberosity. Distal hamstring injury is usually associated with other knee injuries, and isolated injury is rare.

Extensor tendons: can we design a simpler classification zone system?

We summarize the attention that has been drawn to and the thought process about the complexity of current classification zones of extensor tendons. A possible new, simpler classification was proposed by the lead author and discussed with the co-author. A simplified classification is presented with rationale, mainly based on the simplified treatment strategies used by the authors. We also discuss the possible drawbacks and call for investigations on this topic to make the current treatment strategies less complex. An updated system should be based on improved understandings of clinical treatmen
