Surgical Technique: Brachioradialis to Extensor Carpi Radialis Longus and Brevis Nerve Transfers for Tetraplegia.

Improving upper extremity function in high cervical spinal cord injury (SCI) patients with tetraplegia is a challenging task owing to the limited expendable donor muscles and nerves that are available. Restoring active wrist extension for these patients is critical because it allows for tenodesis grasp. This is classically achieved with brachioradialis (BR) to extensor carpi radialis brevis (ECRB) tendon transfer, but outcomes are suboptimal because BR excursion is insufficient and its origin proximal to the elbow further limits the functionality of the tendon transfer, particularly in the abs…

Releasing the A3 pulley and leaving flexor superficialis intact increases pinch force following the Zancolli lasso procedures to prevent claw deformity in the intrinsic palsied finger.

Objective estimates of fingertip force magnitude following surgery to prevent digital metacarpophalangeal (MCP) hyperextension (clawing) in cases of paralysis of the hand's intrinsic muscles will assist clinicians in setting realistic expectations for post-operative pinch strength. We used a cadaveric/optimization approach to predict and confirm the maximal biomechanically possible fingertip force in the intrinsic palsied hand before and after two popular tendon transfer methods to the volar plate of the MCP joint. Both surgeries were also evaluated after release of the A3 pulley-a modificatio…

Wide-Awake Local Anesthesia No Tourniquet Surgery for Zone VII Extensor Tendon Reconstruction: Stages in Active Range of Motion From Intraoperative to Final Recovery.

PURPOSE: We investigated chronological changes in active range of motion (ROM) of the metacarpophalangeal (MCP) joint after zone VII extenso … bridge tendon grafting for tendon reconstruction. In cases with total excursion <30 mm, end-to-side tendon transfer was performed. Active ROM of the MCP joint was measured before surgery; during surgery; 1, 3, and 5 months after surgery, and at the final follow-up. RESULTS: Extension lag of the MCP joint was 49.2° before surgery, improving to 2.9° during surgery …

Arthroscopically Assisted Lower Trapezius Transfer Using Peroneus Longus Autograft for Irreparable Posterosuperior Rotator Cuff Tears.

BACKGROUND: Massive retracted rotator cuff tears are disabling in physically active patients. In patients with persistent pain in whom nonoperative treatment has failed, multiple surgical treatment options are available. Lower trapezius tendon transfer is a promising surgical procedure that can decrease pain, improve external rotation strength, and recreate more normal glenohumeral kinematics. In the present video article, we describe the surgical technique for successful arthroscopic ("scopy")-assisted lower trapezius transfer (SALTT) with use of an easily accessible peroneus longus autograft…

Closed rupture of the flexor tendons caused by carpal bone and joint disorders.

We analysed 21 patients with closed rupture of the flexor tendons caused by carpal bone and joint disorders. The tendon that ruptured depended on the location of the bone perforation into the carpal tunnel. Radiocarpal arthrography was performed in 13 patients and capsular perforation was demonstrated by contrast medium leakage into the carpal canal in 11 patients. This proved a useful diagnostic test. The flexor tendon(s) were reconstructed with free tendon graft in 17 patients, cross-over transfer of flexor tendons from adjacent digits in two and buddying to an adjacent flexor tendon in one

Acute bilateral extensor pollicis longus tendon rupture following bilateral displaced distal radius fracture: A case report.

Extensor pollicis longus tendon ruptures are rarely reported after traumatic events, especially after distal radius fractures. Its classical presentation is a late-onset and unilateral rupture after a non-displaced distal radius fracture. In this case report, we present the treatment of a patient with acute bilateral extensor pollicis longus tendon ruptures after bilateral displaced distal radius fractures. The patient was treated with bilateral open reduction and volar plate fixation via a volar Henry approach for distal radius fractures and bilateral extensor indicis proprius tendon transfer…

Reconstruction of the extensor central slip using a distally based flexor digitorum superficialis slip.

Several methods have been reported for reconstruction of the extensor central slip. We describe the successful use of a distally based slip of the flexor digitorum superficialis tendon for the reconstruction of an incompetent central slip of the extensor mechanism. The flexor digitorum superficialis tendon was transferred from volar to dorsal through the base of the middle phalanx and then woven through the extensor tendon over the proximal phalanx. The technique is straightforward and appears to be robust.

Functional latissimus dorsi island pedicle musculocutaneous flap to restore finger function.

Functional latissimus dorsi island pedicle musculocutaneous flaps were used to restore flexion or extension of the wrist and digits in four clinical cases. By retaining the fascial origin of the latissimus dorsi from the posterior crest of the ilium, the entire muscle was transferred without dividing its neurovascular pedicle and microneurovascular anastomoses. Its facial origin successfully reached the finger flexor or extensor muscles of the forearm. The latissimus dorsi muscle was sutured to the digital flexor tendons in three patients and connected to the extensor tendons in one patient. A

[Indications and results of operative treatment in birth-related brachial plexus injuries].

INTRODUCTION: A review of the literature reveals that under conventional treatment alone or in combination with secondary muscle/tendon transfer about 4 to 43 % of cases show incomplete recovery with severe functional and/or aesthetic impairment. When these patients undergo early microsurgical brachial plexus revision, a regeneration without any significant functional and/or aesthetic impairment can be achieved in 80 to 90 % of the cases. Moreover, microsurgical reconstruction of the brachial plexus does increase the possibilities of secondary muscle/tendon transfers. MATERIALS AND METHODS: O…

Palmaris Longus Tendon Grafting for Extensor Pollicis Longus Tendon Rupture bScrew Tip after 20 years.

INTRODUCTION: Spontaneous ruptures of extension pollicis longus tendon predominantly occur after undisplaced or minimally displaced distal radial fracture near Lister tubercle. Systemic inflammatory diseases and systemic or local steroid, mechanical causes like bony ridges, presence of bone plate or external fixator pin may precipitate this. Repetitive uses in certain occupation like cooking, cow milking, tailoring and direct trauma in kick boxer are also identified as cause. In this study it is caused by screw tip that also after 20 years. Instead of tendon transfer, interposition tendon graf…

Recurrent dislocation of the peroneal tendon.

Recurrent dislocation of the peroneal tendons is an important, although infrequent, cause of disability of the ankle. We used a simple procedure that corrects the basic deformity of a shallow peroneal groove, without the use of metallic fixation or transfer of a tendon or ligament. The groove for the tendons is deepened by removal of some inner fibular substance, while the smooth tenosynovial channel is maintained as an intact periosteal flap on the fibula. The technique was used successfully in ten cases.

Peroneus longus tendon lengthening as an adjuvant measure in cavus foot surgery.

Lengthening of the peroneus longus tendon is very important when performing cavus foot surgery. This tendon directly affects all three components (varus heel, increased arch height, and forefoot adduction) of the idiopathic cavus foot which can be accounted for by increased strength and activity of the peroneus muscle. The deforming force is removed or weakened according to the degree, duration and rigidity of the cavus foot by lengthening or transferring the tendon.
