[Injuries and dysfunction of the posterior tibial tendon].

The function of the posterior tibial (PT) tendon is to stabilize the hindfoot against valgus and eversion forces. It functions as the primary invertor of the foot and assists the Achilles tendon in plantar flexion. The PT tendon is a stance phase muscle, firing from heel strike to shortly after heel lift-off. It decelerates subtalar joint pronation after heel contact. It functions as a powerful subtalar joint supinator and as a support of the medial longitudinal arch. The action of the tendon travels to the transverse tarsal joints, locking them and allowing the gastrocnemius to support heel r

Complications after flexor tendon injuries.

Management of flexor tendon injuries is one of the most demanding tasks in hand surgery. Despite substantial improvements in surgical technique and postoperative rehabilitation protocols, functional outcomes may still be somewhat unreliable. In the present article, the authors present complications encountered after flexor tendon repair and provide their preferred methods of prevention and treatment.

What is the significance of tendon suture purchase?

Repairs have been performed on porcine flexor tendons and subjected to tensile stress measurements to determine the effects and mechanism of core suture purchase (the length of the suture bite). Eighty-four pig trotter flexor profundus tendons were divided and repaired using four lengths of core suture purchase (1.33, 1, 0.66 and 0.33 cm) using a double modified Kessler repair (four strands, two knots) with a peripheral epitendinous suture. Tendon purchase was achieved by either bilateral equal purchase lengths or with one tendon purchase at a fixed depth of 1 cm. A separate group of tendons w…

Glucocorticoid injections in lesions of the achilles tendon.

BACKGROUND: A number of non-operative interventions are used to manage Achilles tendinopathy. In particular, local glucocorticoid injections have generated controversy. Although a number of case reports indicate symptomatic relief following glucocorticoid injection, one systematic review found little evidence to support their efficacy. Furthermore, local glucocorticoid injections may be associated with rupture of the Achilles tendon. This systematic review considered all available clinical trials measuring the effect of local glucocorticoid injections on symptom relief in patients with Achille

Fracture of an ossified tibialis anterior tendon.

Fractures of ossified tibialis anterior tendon have been successfully managed by the excision of the ossified tendon and transfer of the extensor hallucis longus tendon. A 64-year-old man sustained an injury during mountain hiking 2 weeks prior to presentation, falling down on his heels with his ankle fully plantarflexed. Two tender and bony hard masses were palpable along the course of the tibialis anterior tendon, one at the anteromedial aspect of the ankle and another at the dorsum of the talonavicular joint. Radiographs of the ankle demonstrated an approximately 2x1-cm ovoid-shaped bony ma

Complications of the treatment of Achilles tendon ruptures.

Since the first reports in the medical literature of treatment of the Achilles tendon, complications have been recognized from both non-operative and operative techniques. These include tendon rerupture, sural nerve morbidity, wound healing problems, changes in tendon morphology, venous thromboembolism, elongation of the tendon, complex regional pain syndrome, and compartment syndrome. This article delineates the incidence for each of these complications, with differing techniques, methods of avoiding these complications and treatment methods if they occur.

Distal tendinosis of the tibialis anterior tendon.

BACKGROUND: Disorders of the tibialis anterior (TA) tendon have infrequently been reported but spontaneous rupture of this tendon is well re … ened and had lost its normal fibrillary appearance. Longitudinal split tears were observed in eight tendons. Pathology was typical of a degenerative tendinosis. CONCLUSION: Distal TA tendinosis is a condition that seems to predominantly affect overweight elderly women. It often presents with nocturnal burning medial midfoot pain.

Powered AFO for Achilles tendon rupture.

This paper proposes a powered ankle foot orthosis (AFO) for the treatment of a ruptured Achilles tendon. Usually, conservative orthosis treatment requires about two months, and a motionless ankle degrades activities of daily living (ADL). It is difficult to go to school or work on foot, and a pair of crutches is needed to go up and down stairs. In order to improve the ADL, an electric powered AFO has been designed to improve the ability to walk with a fixed ankle joint. The sole of the proposed AFO is equipped with an electric actuator. The prototype actuator consists of Nd magnets and electro

Complications of distal biceps tendon repairs.

Surgical repair is the most reliable method of restoring flexion and supination strength of the elbow and forearm after acute rupture of the distal biceps tendon. Although there may be small measurable deficits in power, endurance, and terminal forearm rotation when carefully evaluated, most of the patients regain near normal upper extremity motion and function and can return to preinjury activities. There are currently 2 basic surgical approaches for distal biceps tendon repair, using 1 anterior incision or using 1 anterior and 1 lateral incision. Anterior repair alone has the advantage of a

Rehabilitating psoas tendonitis: a case report.

This case report describes the examination and physical therapy intervention for a woman with anterior hip pain whose medical diagnosis following magnetic resonance imaging (MRI) was bilateral labral tears and psoas tendinitis. Her physical therapy evaluation revealed findings consistent with psoas tendonitis. Utilizing theories of neuromuscular patterning and knowledge of normal muscle function, the patient was successfully treated in physical therapy following six physical therapy sessions, once a week for 6 weeks. The patient was found to have an overactive psoas muscle, as indicated by hip

Surgical treatment of peroneal tendon tears.

The focus of this article is the diagnosis and treatment of peroneal tendon tears. The article first describes mechanisms of injuries resulting in peroneal brevis and longus tears. Associated pathologies, such as ankle instability, hindfoot varus, hypertrophied peroneal tubercle, are discussed. Following sections on diagnosis and conservative treatment, the article describes operative treatment for isolated peroneus brevis tear, isolated peroneus longus tear, and tears of both the peroneus longus and brevis. The authors also discuss hamstring allograft reconstruction, the silicone rod techniqu

[Bilateral spontaneous patellar tendon rupture].

Ruptures of the patellar tendon are less often observed compared to those of the Achilles or quadriceps tendon. A bilateral rupture is a rarity. Steroid medications or a variety of systemic diseases are associated with spontaneous tendon ruptures. Less often traumatic rupture is the result of severe overloading of the extensor mechanism of the knee. We report on a 60-year-old female without an obvious trauma, systemic disease or medication who suffered a spontaneous bilateral patellar tendon rupture.

Less invasive Achilles tendon reconstruction.

BACKGROUND: The optimal management of chronic ruptures of the Achilles tendon is surgical reconstruction. Reconstruction of the Achilles tendon using peroneus brevis has been widely reported. Classically, these procedures involve relatively long surgical wounds in a relatively hypovascular area which is susceptible to wound breakdown. RESULTS: We describe our current method of peroneus brevis reconstruction for the Achilles tendon using two para-midline incisions. CONCLUSION: This technique allows reconstruction of the Achilles tendon using peroneus brevis preserving skin integrity over the

Oxygen free radicals and tendon healing.

Nitric oxide is a small free radical generated by a family of enzymes, the nitric oxide synthases (NOSs). In a series of experiments performed over the last 15 years, we showed that nitric oxide is induced by all 3 isoforms of NOS during tendon healing and that it plays a crucial beneficial role in restoring tendon function. In normal tendons, very little NOS activity was found, whereas in injured rat and human tendons, NOS activity was expressed in healing fibroblasts in a temporal fashion. In healing rat Achilles tendon fibroblasts, the first isoform to be expressed was endothelial NOS, foll…

Avulsion of the triceps tendon insertion in a cat.

Avulsion of the insertion of the triceps tendon, which had a pre-existing tendinopathy, is described in a cat. The tendon was re-attached to the olecranon and the repair was immobilised using a type la trans-articular external skeletal fixator. The treatment was successful, however, mild and intermittent lameness persisted. Although it is an uncommon condition in small animals, the diagnosis should be based on careful clinical and radiographic examination.

COMPLICATIONS OF JOINT, TENDON, AND MUSCLE INJECTIONS.

Prevention of complications is one of the most important aspects of patient care in pain management. The objective of this study is to review documented complications in medical literature that are associated with interventional pain management, specifically those associated with joint, tendon, and muscle injections. We conducted Medline research from 1966 to November 2006 using keywords complication, injection, radiofrequency, closed claim, facet, zygophyseal joint, sacroiliac joint, shoulder, hip, knee, carpel tunnel, bursa, and trigger point. We found over 35 relevant papers in forms of ori
