Pediatric anesthesia in developing countries.

PURPOSE OF REVIEW: To highlight the problems faced in developing countries where healthcare resources are limited, with particular emphasis on pediatric anesthesia. RECENT FINDINGS: The fact that very few publications address pediatric anesthesia in the developing world is not surprising given that most anesthetics are provided by nonphysicians, nurses or unqualified personnel. In compiling this article information is drawn from pediatric surgical, anesthetic and related texts. In a recent survey more than 80% of anesthesia providers in a poor country acknowledged that with the limited resour

Hereditary angioedema: a case study.

Hereditary angioedema (HAE) is a condition that results from an autosomal dominant trait. It manifests as attacks of swelling involving the extremities, trunk, abdominal viscera, face, neck, or airway. The attacks may occur spontaneously, without any identifiable trigger, or may be the result of a specific trigger, such as a minor tissue trauma. The symptoms of an attack range from abdominal discomfort to a swelling of the upper extremities to a laryngeal edema that can lead to severe airway obstruction. Therapy for HAE can be divided into three distinct phases: treatment for acute attacks, sh

Adverse Childhood Experiences and Subjective Cognitive Decline Among Transgender Adults.

INTRODUCTION: Adverse childhood experiences (ACEs) are associated with an increased risk of developing chronic health conditions, including Alzheimer's disease and related dementias (ADRD) and subjective cognitive decline (SCD), self-reported confusion/memory loss, and an early clinical manifestation of ADRD. While ACEs and SCD have both been individually studied in transgender and nonbinary (TGN) adults, no study has examined the relationship between the two among this population. This study sought to establish the prevalence of ACEs and their association with SCD among TGN adults. DESIGN: C

Pediatric Orthopaedic Transfers to a Level I Trauma Center: After-Hours Burden and Resident Preparedness.

BACKGROUND: Level I pediatric trauma centers manage transferred orthopaedic injuries; however, interfacility transfer (IFT) patterns and educational implications for residents remain poorly characterized. METHODS: We performed a retrospective cohort study of pediatric ground/air emergency medical service transport IFTs to a level I pediatric trauma center for orthopaedic injuries from 2016 to 2021. Variables included demographics, International Classification of Diseases 10th Revision (ICD-10)-coded injuries, referring facility (emergency departments [EDs] and urgent care centers [UCCs]), ar

Head home: a prospective cohort study of a nurse-led paediatric head injury clinical decision tool at a district general hospital.

OBJECTIVES: To assess if a nurse-led application of a paediatric head injury clinical decision tool would be safe compared with current practice. METHODS: All paediatric (<17 years) patients with head injuries presenting to Frimley Park Emergency Department (ED), England from 1 May to 31 October 2018 were prospectively screened by a nurse using a mandated electronic 'Head Injury Discharge At Triage' questionnaire (HIDATq). We determined which patients underwent CT of brain and whether there was a clinically important intracranial injury or re-presentation to the ED. The negative predictive va

Seizure disorder in the men of a Veterans Administration nursing home.

This study examined the descriptive epidemiology of seizure disorder in 129 male residents of a Veterans Administration Nursing Home. Eighty-seven of the residents were institutionalized because of nonpsychiatric disorders (60 for chronic neurologic diseases, and 27 for other medical conditions). Forty-two were institutionalized because of a chronic psychosis (39 for schizophrenia, three for affective disorders). We determined for each resident an extensive clinical data base of 54 items including measures of hematologic, nutritional, metabolic and endocrine status, as well as continuing medic

Ergonomic risks in healthcare workers in acute care; the POSTURE framework.

INTRODUCTION: Healthcare workers performing fluoroscopy-guided procedures are at an elevated risk for work-related musculoskeletal disorders (WMSD) due to prolonged, maladaptive postures, further aggravated by the physical burden of lead aprons. Despite growing awareness, few studies have continuously assessed posture risk with task-level contributors to poor ergonomics in surgical settings. OBJECTIVES: To quantify the proportion of time that healthcare workers spend in medium-to high-risk postures during fluoroscopic procedures in Interventional Cardiology and Orthopaedic Trauma to identify

Optimal management strategy for incidental findings in trauma patients: an initiative for midlevel providers.

BACKGROUND: Increasing patient volume and residents' work hour restrictions have increased the workload at trauma centers. Further, comprehensive tertiary surveys after initial stabilization and appropriate follow-up plans for incidental findings are time consuming. Midlevel providers (MLP) can help streamline this process. We initiated a care plan in which MLPs conducted all tertiary surveys and coordinated follow-ups for incidental findings. METHODS: From November 2005 through May 2006, we implemented a MLP-driven initiative aimed at performing tertiary surveys within 48 hours of admission

Managing Vaginismus with Assisted Dilatation Multidisciplinary Protocol - Retrospective Cohort Study from Indonesia's Largest Dedicated Centre.

BACKGROUND: Vaginismus is a gynecologic disorder with persistent vaginal spasm whenever any penetration is attempted. This leads to a failure of penetration. Vaginismus' true prevalence is unknown and reporting heavily biased. A similar condition is happening in Indonesia, where our facility treats >200 new cases/year. No standardized vaginismus treatment protocol is available internationally, so this retrospective study evaluates our multidisciplinary assisted dilatation protocol and the obstetric outcomes of our patients. METHODS: This is a retrospective cohort study of all patients present

A practice research study concerning homeless service user involvement with a programme of social support work delivered in a specialized psychological trauma service.

ACCESSIBLE SUMMARY: Homeless persons are known to be highly vulnerable to psychological trauma, in events triggering periods of homelessness and the considerable social isolation and adversity suffered when homeless. This study provides an account of how mental health support work is experienced by homeless service users when it is informed by a person-centred, non-directive approach and implemented by trainee health and social care professionals under the auspices of a specialized psychological trauma service. The study draws upon material gathered from interviews with service users domiciled

The secrets women keep: intimate partner violence screening in the female trauma patient.

BACKGROUND: Although intimate partner violence (IPV) is the leading cause of serious injury and the second leading cause of death among reproductive age women in America, effective screening is difficult. Our institution currently screens for IPV during the floor intake assessment by having a registered nurse (RN) ask three unscripted questions about physical, verbal, and sexual abuse during a battery of 81 questions. The patients are frequently in pain, medicated, distraught, or intoxicated, and the RN is juggling multiple responsibilities. We also use a protocol-driven alcohol abuse screen o

War-Related Post-traumatic Stress Symptoms Among Healthcare Workers in Lebanon: A Cross-Sectional Study.

Introduction The 2023-2024 Israeli-Lebanese war has inflicted widespread trauma. On the front line, healthcare workers (HCWs) face psychological stressors, yet their mental health has received limited research. This study assesses the risk of post-traumatic stress symptoms (PTSS) development among Lebanese HCWs while exploring associations with sociodemographic and war-related factors. Methods A cross-sectional study was conducted at the Lebanese American University Medical Center-Rizk Hospital in Beirut, Lebanon, between December 2024 and June 2025. An anonymous, self-administered questionnai

Antenatal Rh Immune Globulin Dose, Timing, and Indications After Selected Procedures and Complications: A Scoping Review.

OBJECTIVE: Studies addressing RhD alloimmunization after antenatal procedures and complications are limited, and practice of RhD immune globulin (RhIG) administration after these events is variable. Our objective was to assess evidence for RhIG efficacy for RhD-negative pregnancies after selected antenatal events. DATA SOURCES: Literature was sourced from databases, including MEDLINE, EMBASE, Cochrane Evidence Based Medical Reviews, and Cumulative Index to Nursing and Allied Health Literature (1946 to August 2024). STUDY SELECTION: Studies included those focused on RhIG effectiveness after a

Predictors of Discharge Disposition Following Surgical Stabilization of Rib Fractures.

Introduction: Patients are commonly discharged to either home health care (HHC), skilled nursing facility (SNF), or inpatient rehabilitation facility (IRF) following surgical stabilization of rib fractures (SSRF) to assist with recovery. This study explores demographic and surgical factors that may be associated with discharge disposition to a transitional care setting (HHC, SNF, and IRF) relative to routine discharge (self-care) following SSRF. Methods: Patients who underwent SSRF between 2017 and 2021 were included using retrospective data from the National Trauma Data Bank. Patients were st

Reigniting Intensive Care Unit Liberation.

BACKGROUND: The Society of Critical Care Medicine has established guidelines to manage pain, sedation, delirium, immobility, family participation, and sleep disruption in the intensive care unit, a set of interventions known as the intensive care unit liberation (ABCDEF) bundle. Adherence to these guidelines has shown positive results. LOCAL PROBLEM: In the intensive care units of a level I trauma academic teaching hospital in central Texas, the rate of bedside nursing staff adherence to the ABCDEF bundle was only 67.1% in January 2022. The aim of this quality improvement project was to impro

Analysis of Unintentional Falls in Pediatric Population and Predictors of Morbidity.

INTRODUCTION: Unintentional falls are a leading cause of pediatric traumatic injury. This study evaluates clinical outcomes of fall-related injuries in children under the age of 10. METHODS: The National Trauma Database was queried for children who experienced an unintentional fall. Patients were stratified by age in two groups: 1-5 and 6-10 years old. The primary outcome was post discharge extension of care, defined as transfer to skilled nursing facility or rehabilitation center after discharge from the hospital. Descriptive statistics and a multivariable logistic regression analysis were u

The results of rapid source control laparotomy or open abdomen for acute diverticulitis.

INTRODUCTION: Rapid source control laparotomy (RSCL) for the management of non-traumatic intra-abdominal emergencies has increased over the past 25 years when it was advocated for trauma patients. Little data, however, support its widespread use. We hypothesize that the patients with RSCL will have poorer outcomes than those treated with primary fascial closure (PFC). METHODS: Patients operated for acute diverticulitis from 2014 to 2016 using The American College of Surgeons sponsored National Surgical Quality Improvement Program (NSQIP) data were reviewed. Two groups were identified: PFC, pa

Facial Fractures After Geriatric Ground-Level Falls Are a Marker of Functional Decline and Warrant Trauma Center Admission.

Ground-level falls (GLFs) are a frequent source of injury in the geriatric population. Facial fractures (FFs) are one subsequent injury that can occur and may be an important marker of functional decline. We conducted a retrospective analysis over a 6-year period of patients 65 years and older sustaining one or more FFs due to a GLF (n = 28). Demographics, comorbidities, FF patterns, concomitant injuries, procedures, and outcomes were analyzed. The mean age was 80.0 ± 8.2 years, 64% were male, 12 patients (43%) were on oral anticoagulants prior to injury, and mean injury severity score was 8.3
