Three young children with advanced cavitary pulmonary tuberculosis died between 1974 and 1978. Two of the patients had tuberculous meningitis as well. All diagnoses were verified at autopsy; however, all tuberculin skin tests were negative. These instances emphasize the difficulty of establishing the diagnosis of tuberculosis in young children. Tuberculosis should be considered in the differential diagnosis of cavitary chest disease in young children, even those with negative tuberculin skin tests.

To make a differential diagnosis, 274 patients were followed up. The prevalence of latent tuberculosis infection in a cohort of nurses new to a London hospital was 7.6% (13 of 171), using an interferon-gamma (IFN-gamma) release assay, and 16.2% (24 of 148), using the tuberculin skin test. On multivariate analysis, birth in a country with tuberculosis prevalence of more than 40 cases per 100,000 population was associated with positive results of both the IFN-gamma release assay and the tuberculin skin test.

We report an 81-year-old male who developed severe interstitial pneumonitis on maintenance intravesical Bacillus Calmette-Guerin (BCG) for in-situ carcinoma of the bladder. The patient was treated with steroids and anti-tuberculin therapy with complete response. While there is no established standard of care for the treatment of interstitial pneumonitis, recent reports describe success with combination of corticosteroids and anti-tuberculin medications. We elected to follow this precedent and treated our patient with corticosteroids and antituberculin therapy with good outcome.

Skin tests were conducted on 3 lepromatous, 3 dimorphous and 6 tuberculoid leprosy patients and 3 others not suffering from leprosy with lepromin, purified protein derivative from M. leprae of armadillo origin and tuberculin. Results show that a delayed hypersensitivity reaction could be produced with PPD in 72 hours on all Mitsuda positive cases, with one anomalous exception, without cross reaction to tuberculin. The results were promising from the point of view of substituting lepromin with PPD in usual tests.
