miRNA signatures in sera of patients with active pulmonary tuberculosis.

Several studies showed that assessing levels of specific circulating microRNAs (miRNAs) is a non-invasive, rapid, and accurate method for diagnosing diseases or detecting alterations in physiological conditions. We aimed to identify a serum miRNA signature to be used for the diagnosis of tuberculosis (TB). To account for variations due to the genetic makeup, we enrolled adults from two study settings in Europe and Africa. The following categories of subjects were considered: healthy (H), active pulmonary TB (PTB), active pulmonary TB, HIV co-infected (PTB/HIV), latent TB infection (LTBI), othe

[Clinicoradiological diagnosis of pulmonary tuberculosis that understood the life of a patient].

This review discusses the clinicoradiological diagnosis of pulmonary tuberculosis. To make a differential diagnosis between pneumonia and mycobacterial infections, it is very important to analyze the radiological findings of inflammatory lung diseases based on normal anatomical structures. If clinicoradiological analyses could make these differentiations, the appropriate treatment strategy for respiratory infections could be established. To accomplish this, exact orientations of pulmonary lobulus, acinus, and miliary nodule are very important. Then, through analyzing chest CT findings and dist

The impact of nutritional deficit on mortality of in-patients with pulmonary tuberculosis.

SETTING: A metropolitan governmental medical centre, Seoul, Republic of Korea. OBJECTIVE: To elucidate the impact of the nutritional deficit assessed by the Nutritional Risk Score (NRS) on the outcomes of in-patients with pulmonary tuberculosis (PTB). DESIGN: All hospitalised patients with microbiologically confirmed PTB were enrolled. A four-point NRS included low body mass index (<18.5 kg/m(2)), hypoalbuminaemia (<30.0 g/l), hypocholesterolaemia (<2.33 mmol/l) and severe lymphocytopaenia (<7 x 10(5) cells/l). The primary outcome was overall in-hospital mortality. The degree of radiographic

Haemoptysis in a patient of achalasia cardia: pulmonary actinomycosis, not tuberculosis.

Actinomycosis is an infectious disease caused by anaerobic gram-positive, non-spore forming bacteria of the genus Actinomyces that affects the oropharynx, digestive tract, and genitalia. Thoracic actinomycosis may affect the respiratory tract and the pleura, even extending to the chest wall. Pulmonary actinomycosis occurs in immunocompetent persons during the fourth and fifth decades of life, with greater prevalence in men, and is generally due to Actinomyces israelii or A meyeri. It is frequently misdiagnosed as primary or metastatic lung cancer or conventional lung infections (eg, tuberculos

[Surgical treatment for pulmonary tuberculosis with Mycobacterium resistance to drugs].

The results of surgical treatment were analyzed in 481 patients who had been preoperatively found to have drug resistance. Patients with fibrocavernous, cirrhotic tuberculosis, chronic pleural empyema, and cavernous pneumonia constituted the vast majority (83.2%). Chemotherapy was performed by individual schemes, by taking into account MBT susceptibility. Glutoxim and pentaglobin were used as pathogenetic therapy for immunity correction. Resection-type operations [n = 368 (68.9%)], thorocoplastic interventions [n = 78 (14.6%)], thoracostomy or cavernostomy [n = 35 (6.5%)], pleurectomy [n = 26

Effect of pulmonary tuberculosis on mortality in patients receiving HAART.

OBJECTIVE: To estimate the effect of ongoing treatment for pulmonary tuberculosis (PTB) at time of initiation of HAART on subsequent risk of death. DESIGN: Evaluation of an open cohort of 7512 patients who initiated HAART between April 2004 and March 2007 in the Themba Lethu Clinic in Johannesburg, South Africa. METHODS: Mortality hazard ratios were estimated using marginal structural Cox proportional hazards models to control for bias due to both confounding and loss to follow-up. Extensive sensitivity and secondary analyses were performed. RESULTS: Although the crude hazard ratio for mort

Extra-pulmonary tuberculosis in children: a study of 41 cases.

BACKGROUND: Extrapulmonary tuberculosis accounts for up to one third of all cases of tuberculosis and children show a higher predisposition to the development of extra-pulmonary tuberculosis. AIM: To review the clinical features of the extrapulmonary tuberculosis in children. METHODS: forty one children with extrapulmonary tuberculosis followed in the Children Hospital of Tunis between January 1995 and December 2007 were reviewed. RESULTS: Extrapulmonary tuberculosis constitutes 57.9% of all cases of tuberculosis. Male to female ratio was 0.7 and the mean age was 7.5 years. The most commonl

[The prevalence and course of arterial hypertension in patients with pulmonary tuberculosis].

The specific features of essential arterial hypertension (AH) were studied in patients with active pulmonary tuberculosis (PT) and in those with residual changes (RC) after prior PT. The prevalence of AH was defined in patients with PT in relation to age, gender, and clinical form of a tuberculous process. The study indicated that the prevalence of AH among patients with an active tuberculous process was less than in the general population and it amounted to 18.2%. It was more common in males aged less than 60 years and in females aged above 60 years. The prevalence of AH in fibrocavernous tub

[Differential diagnosis of tuberculosis of respiratory organs and pulmonary eosinophilia].

The reasons for misdiagnosis of tuberculosis in patients with pulmonary eosinophilia were analyzed, by using 6 clinical cases as an example. In most cases, the reasons for diagnostic errors are underestimation of elevated eosinophil levels and late afterexamination to verify the diagnosis. With general practitioners' increasing alertness, the inefficiency of antibiotics in patients with initially presumed pneumonia is frequently interpreted as an argument in favor of tuberculosis. Examining the cellular composition of bronchoalveolar lavage permits verification of the diagnosis of pulmonary eo

[A case of pulmonary tuberculosis complicated with an orthotopic liver transplantation].

The infectious disease is one of the most important complications related to the organ transplantation. Patients using immunosuppressive agents often present atypical tuberculosis and the treatment of such case is far more difficult in some cases due to the liver damage and/or the drug interaction. We report a case of pulmonary tuberculosis in a patient of 60-year-old man using tacrolimus after an orthotopic liver transplantation. He had liver transplanted orthotopically for the long-term history of chronic hepatitis B and subsequent liver failure on January 28, 2004. An abnormal shadow was fi

The early bactericidal activities of rifampin and rifapentine in pulmonary tuberculosis.

RATIONALE: Comparison of the early bactericidal activity (EBA) of rifapentine and its pharmacokinetics with those of rifampin to determine the cause of poor clinical response and regrowth between doses, leading to rifamycin monoresistance at relapse. OBJECTIVES: Determination of the dose size of rifapentine that gives sufficient drug exposure to prevent regrowth. METHODS: EBA study over initial 5 days of treatment of 123 patients, half at Durban and half at Cape Town, who received single rifapentine doses of 300, 600, 900, or 1,200 mg rifapentine or five daily doses of 150, 300, or 600 mg ri

[Diagnostic screening of social status in patients with pulmonary tuberculosis].

In the literature there is evidence for that the mood of a patient with pulmonary tuberculosis affects the treatment and course of the disease. The necessity of special training of physicians and the duration of an examination make personality tests applied by psychologists not easily accessible to phthisiologists. The author developed methods for integral assessment of social and psychological characteristics by using questionnaire survey. He drew up a special questionnaire given in the Supplement. The questionnaire was used to study 246 middle-aged patients with pulmonary tuberculosis: 141 m
