[The postcholecystectomy syndrome in peptic ulcer].

Results are reported of examination and treatment of 26 patients with gastric and duodenal ulcer which was the cause of postcholecystectomy syndrome and was often not diagnosed before the operation. Roentgenography of the gastrointestinal tract and fibrogastroduodenoscopy helps to establish the diagnosis. Treatment in a gastroenterological clinic and health-resort treatment allow to improve long-term results in this category of patients.

[Leg ulcer complicated by secondary amyloidosis].

A 76-year-old woman with a chronic leg ulcer for the last thirty-seven years was hospitalized in our institution for chronic diarrhea and terminal kidney failure with proteinuria. The diagnosis of secondary amyloidosis due to persistent skin inflammation was confirmed by aspiration of subcutaneous abdominal fat and by kidney biopsy which showed AA type systemic amyloidosis. This appears to be a rare complication of chronic leg ulcers as there have been only eight publications covering eleven cases in the literature.

Corneal ulcer due to Listeria monocytogenes.

We present a patient with a corneal ulcer due to Listeria monocytogenes, which has not previously been considered to be a feature of human listeriosis. The ulcer responded to topical and subconjunctival gentamicin and cephaloridine. Subsequent management was complicated by the development of a fibrinous pupillary membrane leading to pupillary block requiring iridotomy and later vitrectomy with trabeculectomy. Listeria monocytogenes may be confused with diphtheroid contaminants seen in corneal ulcer scrapings and is probably underreported as a cause for microbial keratitis.

Peptic ulcer. A follow-up study.

One hundred and ten patients who had been entered into an ulcer healing study 2 to 4 years previously were recalled during an 8-week period for clinical assessment and endoscopy. Fifty-five gastric ulcer patients were followed-up after an average period of 21.2 months. Nine of these had undergone gastric surgery; of the remaining 46 patients, recurrent ulceration was found in 16 (35%). Fifty-five patients with duodenal ulcer disease were also followed-up after an average of 35.6 months. Five of these patients had undergone surgery and of the remaining 50, 25 (50%) had a recurrent duodenal ulce…

A thousand operations for ulcer disease.

A retrospective study of 1068 patients who had operations for peptic ulcer disease in the 12-year period from January 1, 1974, to January 1, 1986, permits these conclusions: The number of patients admitted to the Massachusetts General Hospital (MGH) has declined steadily in the years of this study--1974-1986. The average number of patients admitted with a diagnosis of peptic ulcer disease in precimetidine years--1974, 1975, and 1976--and in recent years--1982, 1983, and 1984--shows a decline of 39.3% in admissions. In the same periods, the average number of operations per year has declined fro…

[Surgical therapy of hemorrhaging gastroduodenal ulcer].

By using the literature important facts of the spontaneous course of ulcer bleeding are presented. Patient groups with a high risk for early rebleeding are characterized. The indication for operation is discussed regarding intensity and activity of bleeding and endoscopic findings. The operative technique of haemostasis in duodenal and gastric ulcer is described. The own cases are analysed with special respect to endoscopic haemostasis.

Surgical treatment of high gastric ulcer.

The purpose of this study has been to review the late results of surgical treatment of 244 patients with endoscopically proved benign chronic gastric ulcer located 5 cm or less from the cardia. In five patients, a total gastrectomy with esophagojejunostomy was performed. Proximal gastrectomy was used in 3 patients, mesogastrectomy in 5 patients, a partial Schoemaker's procedure in 73 patients, Pauchet's procedure in 70 patients, and Csendes' procedure in 23 patients. Nonresective procedures were employed in 67 patients and included the Kelling-Madlener procedure in 23 patients, pyloroplasty al…

The pathophysiology of peptic ulcer disease.

Heterogeneity is the most important consideration in the pathophysiology of peptic ulcer disease. Acute ulcers and erosions present clinically with gastrointestinal bleeding or perforation. If they heal there is no predictable recurrence. Factors concerned with mucosal defense are relatively more important than aggressive factors such as acid and pepsin. Local ischemia is the earliest recognizable gross lesion. The gastric mucosa is at least as vulnerable as the duodenal mucosa and probably more so. Most drug-induced ulcers occur in the stomach. Chronic or recurrent true peptic ulcers (penetra…

[Pathogenesis of the venous leg ulcer].

Based on different experimental findings, the following sequence of pathogenetically important factors for the development of venous ulcers is outlined: Valvular incompetence, reflux----chronic venous ambulatory hypertension----capillary dilatation and deformation----hyperemia----increased transendothelial passage (protein), microlymphangiopathy----protein deposition in the tissue. Deficient fibrinolysis----pericapillary fibrin cuffs----diffusion block----local hypoxia, tissue death. Although other mechanisms and complex factors may also play important additional roles, for the first time a re…

Varicose ulcer of the upper extremity.

Serious complications of local venous hypertension occur rarely in the upper extremity. These are most frequently the result of a dysfunctional arteriovenous fistula and may produce changes indistinguishable from those of chronic lower-extremity venous insufficiency. A combination of arterial and venous contrast imaging is essential for identifying major venous outflow obstruction and for planning appropriate surgical therapy.

Pathogenetic factors in peptic ulcer disease.

Peptic ulcers are believed to develop because of increased acid and pepsin secretion, a reduced mucosal defense mechanism, or a combination of these two abnormalities. Environmental factors such as use of nonsteroidal and steroidal anti-inflammatory drugs, smoking, and emotional stress may play a role in the pathogenesis of ulcer disease in some patients. The exact mechanisms whereby these factors cause ulcers is unknown, although it is likely that non-steroidal drugs cause ulcers by reducing mucosal defense mechanisms whereas emotional stress may lead to ulceration by increasing acid secretio…

[Somatostatin in severe peptic ulcer hemorrhage].

Somatostatin was compared with intensive antacid and thrombin in a randomised controlled study on 15 patients with severe haemorrhages of the upper digestive tract deriving from peptic ulcers and identified endoscopically in order to assess the efficacy of the two drugs. The results in both groups were similar but somatostatin appeared more effective than antacids and thrombin in terms of blood transfusions required and the average time it took to stop the bleeding. The insignificance of these results is in contrast with the data from similar studies using other drugs (anti-H2) and reported by…
