Extraperitoneal incision of the bladder is recommended for difficult gynecologic-oncologic operations. Cystostomy facilitates dissection of the bladder and helps to prevent injury of the distal ureter. Six cases are reported (2 cervical carcinomas, 2 ovarian carcinomas, 1 vaginal carcinoma, 1 sarcoma) to illustrate the indication and technique of cystostomy.

PURPOSE: To improve the results of current bladder neck reconstruction a modification was developed to provide tapering, circumferential compression and suspension of the bladder neck.
MATERIALS AND METHODS: Nine female and 15 male patients with spinal dysraphism and incontinence, despite intermittent catheterization and pharmacotherapy, and low outlet resistance underwent surgery. The procedure involves wrapping a pedicle strip of anterior bladder wall around the bladder neck and suspending it to the pubic symphysis.
RESULTS: All 9 girls are completely continent, whereas 10 boys are dry and

Urinary bladder distention is a rarely reported cause of iliac vein compression. We report a case of bilateral lower extremity and scrotal edema resulting from marked bladder distention. The bladder was readily palpable and a 4,100 cm3 postvoid residual urine volume was found. No other cause of lower extremity and scrotal edema was found. The patient improved after bladder drainage and transurethral prostatectomy (TURP) for moderate benign prostatic hypertrophy.

We report the second infant of nonconsanguineous parents with epidermolysis bullosa junctionalis associated with urinary bladder exstrophy, epispadias, anteriorized anus, and bilateral inguinal hernias. The family history also included the death of a maternal cousin due to epidermolysis bullosa. Our diagnosis was based on electron microscopy and immunofluorescence evidence. This patient is reported because of the rarity of this constellation of findings.

Among 535 cases of urothelial tumors (upper urinary tract and bladder tumors) from 1970 to 1988, 40 had upper urinary tract tumor associated with bladder tumor. A case of hypothalamic diabetes insipidus accompanied by nonobstructive dilatation of the bladder and upper urinary tract is reported. Treatment with dDAVP resulted in decrease of the urine volume and improvement of dilatation of the urinary tract.

Transurethral resection of the prostate, or other methods to decrease outlet resistance usually leads to relief of symptoms in patients with bladder outlet obstruction (BOO). If symptoms of underactivity persist after normalization of outflow conditions, treatment options are limited.
