Prevention of catheter-related bladder discomfort - pudendal nerve block with ropivacaine versus intravenous tramadol: study protocol for a randomized controlled trial.

BACKGROUND: Catheter-related bladder discomfort (CRBD) is a common distressing symptom complex during the postoperative period, especially after urologic procedures with a relatively greater size urinary catheter. In this study, we will enroll male patients undergoing elective prostate surgery with urinary catheterization under general anesthesia, and we will compare the efficacy of pudendal nerve block (PNB) and intravenous tramadol in CRBD prevention. METHODS/DESIGN: This trial is a prospective, randomized controlled trial that will test the superiority of bilateral PNB with 0.33 % ropivaca…

Adverse Events Following Diagnostic Urethral Catheterization in the Pediatric Emergency Department.

OBJECTIVES: The purpose of this study was to assess adverse events associated with diagnostic urethral catheterization (UC) in young children and to determine their impact on the patient and their family. METHODS: This was a prospective cohort study conducted in the emergency department of a tertiary-care pediatric hospital. All 3- to 24-month-old children with fever who had a diagnostic UC were eligible. Parents who consented to participate were contacted by phone within 7 to 10 days after the UC to answer a standardized questionnaire inquiring about complications. The primary outcome was th

Neglected Trivial Trauma - A Cause for Pseudo-acute Kidney Injury.

Rupture of the urinary bladder and extravasation of urine into the peritoneal cavity leading to urinary ascites is an uncommon event, usually caused by blunt trauma to the abdomen. A high index of suspicion is required for early accurate diagnosis, which avoids unnecessary investigations and interventions. The disappearance of ascites following indwelling Foley's catheterization and high peritoneal fluid urea and creatinine compared to serum values are keys for diagnosis. Sometimes, the diagnosis may be delayed as the features are mistaken for intrinsic renal disease. Here, we report a case of

Urinary tract infection in patients with hip fractures.

We found that 23% of 435 patients treated for a femoral neck fracture in our department also were treated for a urinary tract infection during their hospital stay. The most common pathogen was Escherichia coli, sensitive for mecillinam in 98% of the cases. The most frequently used antimicrobial agent was a broad-spectrum antibiotic, fluoroquinolon, although the most reasonable choice would have been a non broad-spectrum agent such as mecillinam. Catheterization was not a predisposing factor for urinary tract infection, but a poor medical condition and female sex were. We did not find a higher

Complications of Foley catheters--is infection the greatest risk?

PURPOSE: Foley catheters cause a variety of harms, including infection, pain and trauma. Although symptomatic urinary tract infection and asymptomatic bacteriuria are frequently discussed, genitourinary trauma receives comparatively little attention. MATERIALS AND METHODS: A dedicated Foley catheter nurse prospectively reviewed the medical records of inpatients with a Foley catheter at the Minneapolis Veterans Affairs Medical Center from August 21, 2008 to December 31, 2009. Daily surveillance included Foley catheter related bacteriuria and trauma. Data were analyzed as the number of event da

Intermittent self-catheterization training and effects on treatment adherence and infection.

BACKGROUND: Intermittent self-catheterization (CISC) is the preferred treatment for patients with bladder dysfunction due to spinal cord injuries or multiple sclerosis (MS). However, the learning phase plays a crucial role in the still frequent drop-out. AIM: To examine whether the timing of training affects the treatment compliance and the prevalence of urinary tract infections in patients with neurogenic urinary retention. DESIGN: This is a non-randomized observational study. SETTING: The study was carried out from January 2017 to December 2019 in outpatient settings at the Bari Polyclini

Gas containing renal stone - case report.

Gas-containing stones are extremely rare. The formation of renal stones, including the presence of free gas within the stone, is overall poorly understood. The presence of free gas in the collecting system of the urinary tract indicates emphysematous pyelonephritis, in the absence of prior surgical intervention, gastrointestinal fistula, or urinary bladder catheterization. Here, we present the tenth case of gas-containing renal stones, as detected by computed tomography, and discuss the main diagnostic features to differentiate gas-containing stones from emphysematous pyelonephritis.

[Activity of antiseptic catheter lubricants against methicillin-resistant Staphylococcus aureus (MRSA)].

In recent years, general and urinary tract infections caused by methicillin-resistant Staphylococcus aureus (MRSA) have became an increasing problem worldwide. Catheterization of the urinary bladder is one of the most significant risk factors for MRSA contamination of the urine. In vitro tests have documented the antimicrobial activity of the catheter lubricants Instillagel and Endosgel against MRSA specimens within 5 min. These findings support recommending the use of these lubricants in high-risk patients in hospitals and old people's homes for prophylaxis of MRSA bacteriuria.

Urological complications in gynaecological diseases. Review of 18 cases requiring diversion or reconstruction of the urinary tract.

In 18 gynaecological patients requiring diversion or reconstruction of the urinary tract, the relationship between the urological treatments and their influences on the renal function based on DIP were studied. Double-J catheterization and percutaneous nephrostomy drainage with Fr 8 catheter are easily obstructed due to infection of the urinary tract, so a thicker catheter should be used to maintain drainage. Ureterocutaneostomy is considered to be a suitable method to divert urine in cases of poor general condition and poor prognosis in order to preserve renal function for a longer time.

[Double-balloon urethrography in the female].

76 women with pains in the lower efferent urinary tract were urethrographically examined by the double balloon method according to Davis and Cian. The morphological changes registered were correlated with anamnestic, clinical and urodynamic findings and the following indications were derived for this X-ray examination of the female urethra: differential diagnosis between suburethral tumour and urethral diverticulum, recidive infections of the urinary tract, urethral syndrome, uromanometrically proved functional disturbances in the region of the bladder and urethra, painful catheterism and susp

Quality of life and the prevalence of urinary incontinence after surgical treatment for gynecologic cancer: a questionnaire survey.

BACKGROUND: Although there have been a number of reports on urinary voiding symptoms associated with surgical interventions for gynecologic cancer and post-voiding symptoms, there have been few reports on urinary storage symptoms such as urinary incontinence (UI) and overactive bladder (OAB). The purpose of this study was to examine the rates and impact on quality of life (QOL) of urinary storage symptoms after gynecologic cancer surgery. METHODS: A questionnaire survey, including Japanese-language versions of the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF),

The urethral diverticulum of the bull.

The exact location of the urethral diverticulum of the bull was determined. It was found to lie 80-100 mm caudal to the colliculus seminalis, at the transition from the pelvic to the penile urethra. The presence of the diverticulum makes catheterization of the urinary bladder via the penile urethra very difficult.

Robotic-assisted laparoscopic ureteral re-implant (RALUR): Can post-operative urinary retention be predicted?

OBJECTIVE: Urinary retention following robotic-assisted laparoscopic extravesical ureteral reimplantation (RALUR) is proposed to be due to traction or injury of the pelvic parasympathetic nerve plexus during distal ureteral dissection. Nerve-sparing techniques have been employed to avoid injury to the pelvic plexus, either directly or indirectly. This single-center study assessed postoperative urinary retention rates after extravesical RALUR and investigated whether demographic or operative factors could predict this occurrence. METHODS: All RALUR cases entered into an Institutional Review Bo

Purple Urine Bag Syndrome.

Purple urine bag syndrome (PUBS) is a rare disorder seen in elderly persons, wherein the urinary bag and the tubing turn in to purple colour. It is usually seen in patients who are on urinary catheters for a long time. Purple coloured urine occurs due to the accumulation of indigo and indirubin, which are the end products of tryptophan metabolism due to the action of sulfatases and phosphatases formed by bacteria like Providencia, Citrobacter, Enterobacter, Klebsiella etc. We present this interesting phenomenon of purple urine in a young male who was on prolonged urinary catheterization. The u…

Escitalopram-associated acute urinary retention.

New-onset urinary retention can typically be explained by the use of the routine normally suspected medications (e.g., anticholinergics, antihistamines). However, selective serotonin-reuptake inhibitors are not typically presumed as the cause of acute urinary retention (AUR). The following case describes the introduction of escitalopram in a patient and the subsequent development of AUR. Medical causes of urinary retention had been ruled out, and ipratropium was initially suspected to be the cause of urinary difficulties and was discontinued. However, the retention persisted four days after su

Enterococcus faecalis overcomes foreign body-mediated inflammation to establish urinary tract infections.

Urinary catheterization elicits major histological and immunological changes that render the bladder susceptible to microbial invasion, colonization, and dissemination. However, it is not understood how catheters induce these changes, how these changes act to promote infection, or whether they may have any protective benefit. In the present study, we examined how catheter-associated inflammation impacts infection by Enterococcus faecalis, a leading cause of catheter-associated urinary tract infection (CAUTI), a source of significant societal and clinical challenges. Using a recently optimized …
