From Aug. 1977 to June 1988, vagotomy had been performed in 238 patients with duodenal ulcer (DU). According to the clinical signs and the result of gastric acid secretion test, (GAST) parietal cell vagotomy (PCV) and selective vagotomy plus antrectomy (SV+A) were performed in 100 and 138 cases respectively. Surgical procedures for the surgical management of duodenal ulcer had evolved through many stages, over the years. It started with gastroenterostomy alone, then followed by subtotal gastrectomy, vagotomy alone, vagotomy and gastro-jejunostomy, vagotomy and pyloroplasty, and highly selective vagotomy - all which started as open surgical procedures. The results of surgical treatment of 96 patients with giant duodenal ulcer, to whom selective proximal vagotomy (SPV), duodenoplasty (DP), vagotomy with pylorus destroying operation (PDO), gastric resection were performed, were analyzed. Prophylactic methods for an acute postoperative pancreatitis (APP) occurrence before, during and after the operation performance were elaborated. It was established, that the frequency of APP occurrence after SPV and DP is lower than after gastric resection and vagotomy with PDO.

A 10-year result of surgical treatment of 187 patients with an uncomplicated duodenal ulcer has been analysed. Gastric resection was performed in 92 patients, the different types of vagotomy--in 95. After gastric resection, a peptic ulcer of gastroenteroanastomosis occurred in 1 patient, after vagotomy--in 28. The author considers vagotomy to be insufficiently effective in the treatment of an uncomplicated duodenal ulcer. It is not expedient to extend the indications for surgical treatment of the patients with uncomplicated duodenal ulcer.

The results of reconstruction of the pylorus in 12 patients with disabling diarrhoea and/or dumping after vagotomy and pyloroplasty are reported. We have demonstrated that at doses lower than those used by others in dogs, atropine consistently inhibited food-stimulated gastrin release irrespective of vagal innervation of the stomach. Gastrin release induced by food placed directly into the stomach was studied in four gastric fistula dogs with intact vagi and in three other similar dogs with fundic vagotomy. During a period of 10 years, 10 cases of marginal ulcer (MU) after surgery for duodenal ulcer were evaluated retrospectively. The most common cause of MU was inadequate gastric resection plus incomplete vagotomy; the second common cause was incomplete vagotomy. In one case, the MU could be ascribed to malignant gastrinoma.
