Valuing the health states associated with Chlamydia trachomatis infections and their sequelae: a systematic review of economic evaluations and primary studies.

OBJECTIVES: Economic evaluations of interventions to prevent and control sexually transmitted infections such as Chlamydia trachomatis are increasingly required to present their outcomes in terms of quality-adjusted life-years using preference-based measurements of relevant health states. The objectives of this study were to critically evaluate how published cost-effectiveness studies have conceptualized and valued health states associated with chlamydia and to examine the primary evidence available to inform health state utility values (HSUVs). METHODS: A systematic review was conducted, wit

Evidence that adult life risk factors influence the expression of familial propensity to breast cancer.

In the context of a study in Athens comprising 692 cases and 1,261 controls, we have evaluated the effect on breast cancer risk of the joint action of first-degree relative family history and established adult life risk factors. We created a risk score by assigning the value of 1 to women at high risk with respect to any of these risk factors and 0 otherwise, and summing these values, using weights equal to the excess odds ratio. The odds ratio for a tertile increment in the risk score was 1.5 [95% confidence interval (CI) = 1.3-1.8] among women without family history, and 2.3 (95% CI = 1.1-5.

Hope, meaning, and growth following the September 11, 2001, terrorist attacks.

Positive psychologists found the increase of seven character strengths that encompass the so-called theological virtues, including hope and spirituality, in Americans after the September 11, 2001, attacks. Little is known about how they may affect post-September 11, 2001, mental health. Using multivariate analysis, this study investigated the relationship of hope and spiritual meaning with depression and anxiety in a sample of 457 students 3 months after September 11, 2001. Both characters contributed to lower levels of symptoms. In qualitative analysis, of 313 answers to an open-ended questio

[Study of calcium and magnesium in cerebrospinal fluid and its' relation to different neurological diseases (author's transl)].

Calcium and magnesium have been measured in cerebrospinal fluid by atomic absorption spectrophotometry in children. The normal values on 194 C.S.F., obtaining for the calcium x: 5.24 mg. % and s: +/- 0.378 mg. % [50--56 % lower than serum values] and for magnesium x: 2.64 mg. % and s: +/- 0.155 mg. % [19--33 % higher than serum values] are found. Higher values of calcium at birth and on the first year of life and no differences with magnesium are noted. Applying the t-test, between normal values obtained and the different pathological entities, authors find singificant differences on the level

Upper Extremity Peripheral Nerve Ultrasonography, as a Diagnostic Aid in Amyotrophic Lateral Sclerosis.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a life-threatening progressive motor neuron disease whose diagnosis is challenging because of lacking specific diagnostic means. The current study aims to assess the value of upper extremity peripheral nerves ultrasonography in ALS detection. MATERIALS AND METHODS: In this case-control study, 30 ALS subjects were assessed regarding the cross-sectional area (CSA) of the proximal (at distal part of arm or the proximal of elbow) and distal (at wrist level) median and ulnar nerves, assessed via ultrasonography. Similarly, 30 age- and gender-match

How much do patients benefit in quality of life after surgery for cervical spondylotic myelopathy? A Spine CORe™ analysis of QOD data.

OBJECTIVE: Cervical spondylotic myelopathy (CSM) is the leading cause of spinal cord dysfunction, leading to worsening disability and poor quality of life. The long-term benefits of surgery for patient quality of life remain incompletely defined. Given the current emphasis on value-based care, this study aimed to identify 1) how surgery improves quality of life and 2) what factors are associated with quality of life improvement in patients operated on for CSM. METHODS: The Spine CORe™ study group performed a post hoc analysis of the prospectively collected, 14-site Quality Outcomes Database C

An Exceptional Case of Diabetic Ketoacidosis.

We present a case of diabetic ketoacidosis, known as one of the most serious metabolic complications of diabetes. We were confronted with rapid neurological deterioration and unseen glycaemic values, which reached almost 110 mmol/L, subsequently resulting in hyperkalaemia and life-threatening dysrhythmias. This is the first reported live case with such high values of blood glucose and a favourable outcome.

Dose-dependent changes in real-life affective well-being in healthy community-based individuals with mild to moderate childhood trauma exposure.

BACKGROUND: Childhood trauma exposures (CTEs) are frequent, well-established risk factor for the development of psychopathology. However, knowledge of the effects of CTEs in healthy individuals in a real life context, which is crucial for early detection and prevention of mental disorders, is incomplete. Here, we use ecological momentary assessment (EMA) to investigate CTE load-dependent changes in daily-life affective well-being and psychosocial risk profile in n = 351 healthy, clinically asymptomatic, adults from the community with mild to moderate CTE. FINDINGS: EMA revealed significant CT

Constructing High-stakes Surgical Decisions: It's Better to Die Trying.

OBJECTIVE: To explore high-stakes surgical decision making from the perspective of seniors and surgeons. BACKGROUND: A majority of older chronically ill patients would decline a low-risk procedure if the outcome was severe functional impairment. However, 25% of Medicare beneficiaries have surgery in their last 3 months of life, which may be inconsistent with their preferences. How patients make decisions to have surgery may contribute to this problem of unwanted care. METHODS: We convened 4 focus groups at senior centers and 2 groups of surgeons in Madison and Milwaukee, Wisconsin, where we

Decreased half-life time of plasma von Willebrand factor collagen binding activity in essential thrombocythaemia: normalization after cytoreduction of the increased platelet count.

Patients with essential thrombocythaemia (ET) exhibit a decrease of large von Willebrand factor (VWF) multimers in plasma, which is inversely related to the platelet count. In the present study we investigated whether the decrease of large VWF multimers in plasma with increasing platelet counts is the consequence of increased turnover of large VWF multimers in vivo. To that end we measured the half-life times of endogenously released VWF:Ag and VWF:CBA (collagen binding activity) after intravenous administration of desmopressin (DDAVP) to nine ET patients and nine control subjects (N). In addi

Zidovudine for the prevention of vertical HIV transmission: a decision analytic approach.

The purpose of this study was to quantify the benefits of maternal-neonatal zidovudine (ZDV) administration for the prevention of vertical human immunodeficiency virus (HIV) transmission against the potential risks of drug-induced complications in uninfected children. A decision analysis model was created with use of a Markov cohort simulation, for evaluating both survival and quality of life for two hypothetical cohorts of HIV-exposed neonates: one with in utero and neonatal exposure to preventive ZDV therapy and the other not exposed. The model included the probability of congenital HIV infe

Can health-related quality of life predict survival in adults with cystic fibrosis?

RATIONALE: Advances in the management of cystic fibrosis have led to a significant improvement in survival, although marked differences between individuals are still observed. The value of patient-reported health-related quality of life scores in predicting survival in adults with cystic fibrosis is unknown. OBJECTIVES: To evaluate whether patient-reported health-related quality of life could predict survival in cystic fibrosis. METHODS: From 1996 to 1997 a consecutive series of 223 patients were recruited to evaluate the Cystic Fibrosis Quality of Life Questionnaire. Demographic (age, sex),

Cholestatic jaundice in the newborn.

Neonatal cholestasis or direct hyperbilirubinemia in the first 3 months of life is nearly always more pathologic than physiologic. This article reviews the extensive differential diagnosis of cholestatic jaundice and the approach to diagnosis. A sense of urgency is underscored by the value of early recognition and intervention in metabolic, iatrogenic, and anatomic disorders. The long-term management goals are reviewed, including indications for hepatic transplantation.

Understanding the Influence of Culture on End-of-Life, Palliative, and Hospice Care: A Narrative Review.

Cultural beliefs and values significantly shape end-of-life care decisions, including palliative and hospice care approaches. These cultural influences affect how patients and families view death, pain management, and medical decision-making. This narrative review examines how cultural factors impact care delivery for patients with terminal diagnoses, with particular attention to improving patient-provider communication and trust across diverse populations. A search of the current literature was conducted from March 1, 2022, to May 1, 2022, through the following databases: PubMed, ScienceDirec
