Antimicrobial effect of biological surgical mesh added with vancomycin or silver nanoparticles for multidrug-resistant bacteria: experimental study in rats.

INTRODUCTION: Surgical site infection (SSI) and polypropylene mesh (PPM) infections are recurrent problems in abdominal hernia surgeries, highlighting the need for a new antimicrobial material for surgical repair. The aim of this study was to evaluate the in vivo antimicrobial effect of a new biological mesh made of decellularized bovine pericardium (BP), added with vancomycin (VAN) or silver nanoparticles (AgNPs), as prevention for SSI. METHODS: Thirty-five Wistar rats were divided into four groups: BP C+ (n=9) with BP without additions; PP C+ (n=8) with PPM; BP AgNPs (n=9) with BP added wit

Genomic characterization of vancomycin-resistant Enterococcus faecium and van-carrying mobile genetic elements in a tertiary hospital in northeastern China.

BACKGROUND: Vancomycin resistant Enterococcus faecium (VREfm) poses a significant healthcare challenge due to its multidrug resistance and genomic plasticity. Vancomycin resistance is commonly mediated by van gene clusters located on transposons, which are often associated with plasmids. METHODS: 19 VREfm isolates were collected from different departments of the First Hospital of Jilin University between 2019 and 2024. Whole-genome sequencing (WGS) was performed on the strains for comprehensive genomic analysis. Multilocus sequence typing (MLST) was used to determine the sequence types of the…

Limited utility of MALDI-TOF MS for identifying optrA-carrying vancomycin-resistant Enterococcus faecium during a hospital outbreak.

Enterococci are commensals of the human gut that opportunistically cause over 10% of hospital-acquired infections. Vancomycin-resistant enterococci (VRE) are often treated with linezolid; however, resistance to linezolid has emerged, most commonly mediated by the optrA gene. Bacterial strain typing is currently performed using whole-genome sequencing (WGS) at specialised laboratories. Matrix-assisted laser desorption-ionisation time-of-flight mass spectrometry (MALDI-TOF MS) is an established technology for microbiological species identification and has more recently been explored as a method

Prophylactic phage administration provides a time window for delayed treatment of vancomycin-resistant Enterococcus faecalis in a murine bacteremia model.

INTRODUCTION: Vancomycin-resistant Enterococcus faecalis (VRE) poses a significant challenge in clinical settings due to its resistance to multiple antibiotics. Phage therapy offers a promising alternative to address this resistance crisis. However, critical gaps remain regarding optimal dosing, therapeutic design, and treatment timing for phage therapy targeting VRE-induced bacteremia. METHODS: The biological and genomic characteristics of a novel lytic phage specific to VRE were investigated. Its in vitro bactericidal and antibiofilm activities were evaluated, along with its synergy with an

Development of a strain-specific PCR as a diagnostic tool for surveillance, detection, and monitoring of vancomycin-resistant Enterococcus faecium during outbreak.

INTRODUCTION: Vancomycin-resistant Enterococcus faecium (VREfm) poses a significant concern in healthcare settings, particularly during outbreaks. Traditional antibiotic susceptibility testing may fail to detect occult vancomycin resistance, and long culture times delay diagnosis. While whole genome sequencing (WGS) is the most accurate method for tracing infectious disease transmissions, its response times are not rapid enough to optimally support controlling of ongoing outbreaks. To address this limitation, we investigated the genomic diversity among outbreak isolates and developed outbreak-…

The use of a Poisson hidden Markov model for automated detection of hospital outbreaks with vancomycin-resistant enterococci in routine surveillance data.

BACKGROUND: Despite the low prevalence of infections due to vancomycin-resistant enterococci (VRE) in the Netherlands, VRE is a frequent source of hospital outbreaks. We investigated whether a Poisson hidden Markov model (PHMM) can detect in-hospital VRE outbreaks in routine data from the Dutch Infectious Diseases Surveillance Information System for Antimicrobial Resistance (ISIS-AR). METHODS: We performed a retrospective data linkage study from 2013 up to 2023, including data from 89 hospitals on VRE isolates from ISIS-AR. A PHMM was used to detect potential outbreaks based on weekly VRE cou

Co-spray drying of cannabidiol and vancomycin combination powder formulations for the treatment of methicillin-resistant Staphylococcus aureus respiratory infections.

Methicillin-resistant Staphylococcus aureus (MRSA) can cause life-threatening respiratory infections. Conventional oral and parenteral antibiotic treatments are often inefficient in targeting the lower respiratory tract and could cause systemic adverse effects. In this study, the 24-hour time-kill assay revealed that the co-delivery of cannabidiol (CBD) and vancomycin synergistically eradicated three clinical strains of MRSA. The combinations had rapid and sustained bactericidal activity over 24 h at the ½ x minimum inhibitory concentration (MIC). Four synergistic mass ratios of CBD and vancom

Novel, rapid, and reliable typing of vancomycin-resistant Enterococcus faecium CC17/ST80 strains using MALDI-TOF MS.

Vancomycin-resistant Enterococcus faecium (VREfm) is an important nosocomial pathogen. The recent emergence of the highly virulent clonal complex 17 (CC17) is posing a challenge for both therapeutic interventions and hospital infection control measures. Hence, prompt discrimination of CC17 VREfm from unrelated and less-virulent VREfm strains is essential for preventing its spread in hospitals and beyond. Between January 2022 and November 2024, 340 VREfm primary isolates have been identified in our lab and underwent genotyping by pulsed-field gel electrophoresis (PFGE) to survey a potential out

Formulation of pH-responsive nanoplexes based on an antimicrobial peptide and sodium alginate for targeted delivery of vancomycin against resistant bacteria.

The rapid spread of bacterial resistance to antibiotics necessitates the development of innovative strategies to enhance their efficacy. One promising approach is incorporating antimicrobial peptides (AMPs) to synergize antibiotics. Herein, we introduce pH-responsive nanoplexes of plant AMP and sodium alginate (Na-Alg) for the co-delivery of AMP and Vancomycin (VCM) against resistant bacteria. The optimal nanoplexes (VCM-Na-Alg/AMP) were characterized, revealing a particle size, polydispersity index, zeta potential, encapsulation efficiency, and loading capacity of 159.5 ± 1.150 nm, 0.149 ± 0.

Mutations in the riboflavin biosynthesis pathway confer resistance to furazolidone and abolish the synergistic interaction between furazolidone and vancomycin in Escherichia coli.

The combined application of furazolidone and vancomycin has previously been shown to be synergistic against Gram-negative pathogens, with great therapeutic promise. However, the emergence and mechanism of resistance to this antibiotic combination have not been characterized. To fill this gap, we here selected Escherichia coli progeny for growth on the furazolidone-vancomycin combination at the concentration where the parent was sensitive. We show that selected clones were associated with increased resistance to neither, only one drug, or both furazolidone and vancomycin, but in all cases were

Global burden, inequalities and projections of vancomycin-resistant Enterococcus faecium and Enterococcus faecalis, 1990-2050: A systematic global analysis.

BACKGROUND: Vancomycin-resistant Enterococcus faecium (VREfm) and Enterococcus faecalis (VREfs) are major antimicrobial resistance threats. Previous assessments rarely distinguished species, limiting understanding of their epidemiology and control. To address this gap, we analysed trends from 1990 to 2021 and projected the burden through 2050 in order to assess the global burden, inequalities, and future patterns of VREfm and VREfs. METHODS: We analysed deaths and disability-adjusted life years (DALYs) from Measuring Infectious Causes and Resistance Outcomes for Burden Estimation (MICROBE) ac

Nasal Colonizing Vancomycin-Resistant and Intermediate Staphylococcus aureus Among Admitted Patients: A Hospital Based Cross-Sectional Study.

BACKGROUND AND AIM: Staphylococcus aureus colonizing the nasal cavity poses a potential risk for infections. Vancomycin is a primary treatment for invasive infections caused by penicillin and methicillin-resistant S. aureus (MRSA). However, reports of vancomycin-resistant S. aureus (VRSA) have emerged, highlighting it as a high-priority pathogen requiring attention. There is limited information on the epidemiology of VRSA and vancomycin-intermediate S. aureus (VISA) from the Sidama regional state. This study aimed to determine the prevalence of VRSA and VISA among S. aureus colonizing patients

The virtue of training: extending phage host spectra against vancomycin-resistant Enterococcus faecium strains using the Appelmans method.

Phage therapy has (re)emerged as a serious possibility for combating multidrug-resistant bacterial infections, including those caused by vancomycin-resistant Enterococcus faecium strains. These opportunistic pathogens belong to a specific clonal complex 17, against which relatively few phages have been screened. We isolated a collection of 21 virulent phages growing on these vancomycin-resistant isolates. Each of these phages harbored a typical narrow plaquing host range, lysing at most 5 strains and covering together 10 strains of our panel of 14 clinical isolates. To enlarge the host spectru

Colonization of vancomycin-resistant Enterococcus faecium in human-derived colonic epithelium: unraveling the transcriptional dynamics of host-enterococcal interactions.

Enterococcus faecium is an opportunistic pathogen able to colonize the intestines of hospitalized patients. This initial colonization is an important step in the downstream pathogenesis, which includes outgrowth of the intestinal microbiota and potential infection of the host. The impact of intestinal overgrowth on host-enterococcal interactions is not well understood. We therefore applied a RNAseq approach in order to unravel the transcriptional dynamics of E. faecium upon co-culturing with human derived colonic epithelium. Co-cultures of colonic epithelium with a hospital-associated vancomyc
