Circulating biomarkers of left ventricular hypertrophy in pediatric coarctation of the aorta.

OBJECTIVE: Patients undergoing surgical repair of aortic coarctation have a 50% risk of pathologic left ventricular remodeling (increased left ventricular mass or relative wall thickness). Endothelin 1, ST2, galectin 3, norepinephrine and B-natriuretic peptide are biomarkers that have been associated with pathologic LV change in adult populations but their predictive value following pediatric coarctation repair are not known. HYPOTHESIS: Biomarker levels at coarctation repair will predict persistent left ventricular remodeling at 1-year follow up. DESIGN: Prospective, cohort study of 27 pati…

The theory of hypertensive heart disease and heart failure: revisiting the evidence and pathophysiology.

Arterial hypertension is a major epidemiological risk factor for left ventricular hypertrophy (LVH) and heart failure (HF). Within this framework, the theory of hypertensive heart disease (HHD) has historically been proposed as the conceptual substrate for HF with preserved ejection fraction (HFpEF). The aim of this state-of-the-art review is to re-examine whether secondary hypertrophic remodelling due to chronic pressure overload represents an intrinsically dysfunctional myocardial state prone to HF, beyond the well recognized role of severe excess afterload as a haemodynamic precipitant of d

Prevention and treatment of hypertensive left ventricular hypertrophy.

PURPOSE OF REVIEW: Left ventricular (LV) hypertrophy (LVH) is a well recognized target organ adaptation to longstanding uncontrolled hypertension and other cardiovascular risk factors. It is also a strong and independent predictor of many cardiovascular disorders. RECENT FINDINGS: This focused review explores the current concepts in screening, diagnosis, prevention, and treatment of LVH in patients with hypertension. Currently, the primary screening and diagnostic tools for LVH are ECG and 2D echocardiography. Implementing machine learning in the diagnostic modalities can improve sensitivity

Renshen decoction improves myocardial energy metabolism in heart failure by regulating exosomal miR-30e-5p.

BACKGROUND: Abnormal intercellular communication mediated by exosome (EXOs)-miRNAs in cardiac cells is the core pathogenic mechanism that induces myocardial energy metabolism disorders and leads to ventricular remodeling in heart failure (HF). Renshen Decoction (RSD), a traditional formula, exerts significant cardioprotective effects. However, whether it inhibits ventricular remodeling in HF by regulating EXOs-miRNA-mediated intercellular communication remains unclear. OBJECTIVE: To explore the mechanism by which RSD improves myocardial energy metabolism disorders and inhibits ventricular rem…

Left ventricle reverse remodeling in chronic aortic regurgitation patients with dilated ventricle after aortic valve replacement.

BACKGROUND: Aortic valve replacement (AVR) for chronic aortic regurgitation (AR) with a severe dilated left ventricle and dysfunction leads to left ventricle remodeling. But there are rarely reports on the left ventricle reverse remodeling (LVRR) after AVR. This study aimed to investigate the LVRR and outcomes in chronic AR patients with severe dilated left ventricle and dysfunction after AVR. METHODS: We retrospectively analyzed the clinical datum of chronic aortic regurgitation patients who underwent isolated AVR. The LVRR was defined as an increase in left ventricular ejection fraction (LV

Right Ventricle Remodeling and Function in Scleroderma Patients.

Scleroderma, known also as systemic sclerosis (SSc), is a severe disease associated with high mortality rates, and right ventricular (RV) remodeling and dysfunction, along with pulmonary artery hypertension (PAH), are among the most important internal organ manifestations of this disease. PAH has a higher prevalence in patients with SSc compared to the general population and represents a significant predictor of mortality in SSc. In patients with SSc, the morphological remodeling and alteration of RV function begin even before the setting of PAH and lead to development of a specific adaptive p

Inflamed adipose tissue: A culprit underlying obesity and heart failure with preserved ejection fraction.

The incidence of heart failure with preserved ejection fraction is increasing in patients with obesity, diabetes, hypertension, and in the aging population. However, there is a lack of adequate clinical treatment. Patients with obesity-related heart failure with preserved ejection fraction display unique pathophysiological and phenotypic characteristics, suggesting that obesity could be one of its specific phenotypes. There has been an increasing recognition that overnutrition in obesity causes adipose tissue expansion and local and systemic inflammation, which consequently exacerbates cardiac

Influence of Supraphysiologic Biomaterial Stiffness on Ventricular Mechanics and Myocardial Infarct Reinforcement.

Injectable intramyocardial biomaterials have promise to limit adverse ventricular remodeling through mechanical and biologic mechanisms. While some success has been observed by injecting materials to regenerate new tissue, optimal biomaterial stiffness to thicken and stiffen infarcted myocardium to limit adverse remodeling has not been determined. In this work, we present an in-vivo study of the impact of biomaterial stiffness over a wide range of stiffness moduli on ventricular mechanics. We utilized injectable methacrylated polyethylene glycol (PEG) hydrogels fabricated at 3 different mechan…

[Pulmonary hypertension caused by left heart disease].

Increased resistance to pulmonary venous drainage is the main mechanism in pulmonary hypertension (PH) developing due to left heart disease. This condition may occur as a result of various diseases affecting left ventricle, left atrium, mitral or aortic valves. Pulmonary hypertension is the common and well-recognized complication of left ventricular systolic dysfunction and pulmonary arterial hypertension accompanying chronic heart failure is related to increased mortality. Treatment should be tailored according to the underlying disease.

Left atrial reverse remodeling predicts long-term survival after cardiac resynchronization therapy.

BACKGROUND: Left ventricular (LV) reverse remodeling has been identified as a strong predictor of long-term survival in patients receiving CRT. Interestingly, CRT induces reverse remodeling in the left atrium (LA) as well. It is currently unknown to what extent LA reverse remodeling is correlated to long-term survival after CRT. This study aims to assess the long-term prognostic value of left atrium (LA) reverse remodeling in patients undergoing cardiac resynchronization therapy (CRT). METHODS: Baseline and 3-months follow-up echocardiograms after CRT implantation were prospectively assessed

Inflammatory Mechanisms of Cardiovascular Remodeling.

Inflammation and fibrosis play an important role in the development and progression of cardiovascular diseases. Acute coronary syndrome (ACS) is caused by rupture of inflamed atherosclerotic plaque and subsequent atherothrombosis. Recent studies have shown that inflammatory markers such as C-reactive protein (CRP) can predict ACS development and have demonstrated the effectiveness of new therapeutic approaches targeting inflammation. Studies have also shown that an enhanced inflammatory response after myocardial infarction (MI) is associated with cardiac rupture, ventricular aneurysm formation
