Bilateral acoustic neurofibromatosis with bilateral multicentric facial schwannomas.

A case report of a 20-year-old female with bilateral acoustic neurofibromatosis (NF-2) and bilateral facial schwannomas is presented. Multiple segmental schwannomas were found with clinically intact tissue between each tumoral enlargement in the right parotid region. Translocation (2;8) (p2.4:q2.1) was detected in this patient, and has not been reported in a schwannoma until now. The patient's family would not allow any major surgery to be performed.

Acoustic neurinomas with macrocysts treated with Gamma Knife radiosurgery.

Six cases of acoustic neurinomas with macrocystic components are presented. In three cases the cystic portion was within the tumor, while in the other three, the cyst was peritumoral, in the form of a cul-de-sac within the arachnoid, in other words it was not a true tumor cyst. The six tumors are from a series of 74 acoustic neurinomas treated by radiosurgery with a minimum follow-up of 18 months. In all cases, enlargement of the associated cyst was observed as early as 4 months after radiosurgery. Clinical signs and symptoms such as facial weakness, trigeminal symptoms, vertigo and dizziness

Epidermal growth factor receptor expression by acoustic neuromas.

Antibodies directed against epidermal growth factor receptor (EGFr) impede proliferation and induce differentiation of EGFr-positive cancers. To explore the effectiveness of anti-EGFr monoclonal antibodies on acoustic neuromas (ANs), we first sought to evaluate EGFr expression by ANs. The records of all patients with the diagnosis of AN at our institution from January 1989 to July 1994 were reviewed. Immunohistochemical analysis for EGFr was performed on formalin-fixed, paraffin-embedded archival surgical specimens. Skin, liver, and placenta were used as positive tissue controls. Purified rabb

Preliminary results of linear accelerator radiosurgery for acoustic schwannomas.

In this paper the authors evaluate the results of linear accelerator (LINAC)-based stereotactic radiosurgery for acoustic schwannomas. Fifty-six patients underwent LINAC-based stereotactic radiosurgery for acoustic schwannomas at the University of Florida between July 1988 and November 1994. Each patient was followed for a minimum of 1 year or until death; no patient was lost to follow up. One or more follow-up magnetic resonance images or computerized tomography scans were obtained in 52 of the 56 patients. Doses ranged between 10 and 22.5 Gy with 69.6% of patients receiving 12.5 to 15 Gy. Th

Iatrogenic impairment of hearing during surgery for acoustic neuroma.

Isolated or combined labyrinthine, neural, and vascular damage account for failure to preserve hearing during removal of acoustic neuromas. However, the specific mechanisms of auditory impairment remain unclear unless surgical maneuvers can be related to peri- and postoperative hearing on the basis of intraoperative monitoring of auditory function.Among the different auditory monitoring techniques, recording of cochlear nerve action potentials (CNAPs) from the intracranial portion of the nerve has proven particularly useful for identifying the mechanisms of iatrogenic auditory injury. The prese

[Hearing preservation and tinnitus following removal of acoustic neurinomas].

Thirty-five cases of unilateral acoustic neurinomas were analyzed with special reference to the postoperative eighth cranial nerve function. An additional three cases of bilateral acoustic neurinomas associated with neurofibromatosis were also analyzed. Out of a total of 40 neurinomas in all, 38 cases were retrospectively reviewed. The thirty-five cases of unilateral acoustic neurinomas were summarized as follows. The patients' age ranged from 23 to 69 years old. The tumor size varied as follows; 7 cases were confined to the internal acoustic meatus, 4 cases were 20 mm or less in their maximum

Microsurgery versus radiosurgery in the treatment of small acoustic neurinomas.

Microsurgical preservation of the facial nerve during removal of acoustic neurinomas can hardly be compared with microsurgery of the eighth cranial nerve. Many more anatomical and pathogenetic factors are involved that need careful consideration. In small neurinomas, of grades I and II, total extirpation of the tumour with preservation of both the facial nerve and segments of the vestibulocochlear nerve not directly involved by the tumour has become a safe and practical technique. In small acoustic neurinomas immediate facial nerve function could be preserved in 88% and "useful hearing" could

MR evaluation of acoustic schwannoma with fractional contrast doses.

OBJECTIVE: To investigate the utility of lower contrast medium doses for the detection and conspicuity of acoustic schwannomas. MATERIALS AND METHODS: The L/B (L, lesion; B, background) ratios or lesion contrast of 17 pathologically proven acoustic schwannomas studied with a standard dose (0.1 mmol/kg) of gadopentetate dimeglumine was measured. In addition, 22 patients with acoustic schwannomas were studied prospectively with fractional doses using the incremental dose technique. Each patient received an initial bolus injection of one-eight the standard dose (0.0125 mmol/kg) followed by an in

[Effects of stimulus rate on the ABR of acoustic tumor patients].

The effect of stimulus rate on the ABR of acoustic tumor (AT) patients was studied. We delivered 90 dB nHL click stimuli to 23 AT patients at stimulus rates of 9.5 Hz, 20 Hz, 40 Hz and 90 Hz, and recorded the ABR at each rate. Normal hearing subjects (42 ears) and sensorineural hearing loss patients (30 ears) were also studied for normative data. The following two parameters were examined: the interpeak latency difference between wave I and wave V (IPL I-V) at each stimulus rate, and the increase in IPL I-V (delta IPL I-V) when the stimulus rate was increased from 9.5 Hz. The values of both pa

Results of surgery following stereotactic irradiation for acoustic neuromas.

Stereotactic irradiation has been proposed as an alternative to surgical intervention for the treatment of acoustic neuromas. The authors present their experience with five patients previously treated with stereotactic irradiation who had progression of symptoms due to tumor enlargement. Intraoperatively, the tumors were severely scarred to the facial nerve. Histopathologic examination demonstrated viable cells typical of schwannomas. Two patients received auditory brainstem implants; neither received benefit from the implant. Three of the patients had House-Brackmann grade VI facial nerve fun

Impact of cranioplasty on headache after acoustic neuroma removal.

We reported previously the incidence of headache after the retrosigmoid removal of an acoustic neuroma as 23% at 3 months, declining to 9% at 2 years after surgery. In an attempt to reduce the incidence and the severity of these headaches, we made one change in our surgical procedure, which was to perform a cranioplasty with methyl methacrylate. Twenty-four patients underwent the cranioplasty and were followed for at least 3 months postoperatively. These patients were matched to 24 patients who did not undergo a cranioplasty. We found a 4% incidence of headache in the cranioplasty group and a

Patterns of evoked otoacoustic emissions associated with acoustic neuromas.

Evoked otoacoustic emissions (OAEs) are assumed to reflect healthy outer hair cell function. Over the past few years, evoked OAEs have been shown to be useful as indicators of cochlear hearing loss. Because basic studies have shown that OAEs are extremely sensitive to cochlear anoxia and hypoxia, as well as to the adverse effects of many inner ear diseases, it is possible that these objective tests can provide some insight into the fundamental basis of the hearing loss exhibited by patients with acoustic neuromas. The primary aim of the present study was to examine the effects of acoustic neur

Meatal facial nerve decompression in acoustic neuroma resection.

Delayed facial palsy occurs after approximately 15% of acoustic neuroma resections. Whereas in most cases facial nerve function recovers satisfactorily, in about 12% of cases delayed facial palsy may have a poor functional recovery. The fate of facial nerve function was studied in 55 consecutive translabyrinthine acoustic neuroma resections, 23 of whom underwent meatal facial nerve decompression (MFND) at the time of tumor resection. The facial nerve results also were reviewed in 19 middle cranial fossa resections, of whom 14 underwent MFND. There was a bimodal distribution of the delayed pals

[The value of clinical examination methods in diagnosis of acoustic neuroma].

The diagnostic value of routine clinical tests for predicting the presence of an acoustic neuroma (AN) was assessed retrospectively in a group of 391 patients having magnetic resonance imaging (MRI) scans because of a suspected retrolabyrinthine lesion. An AN was found by MRI only in 9% of these patients. The positive predictive value of unilateral sensorineural hearing loss in a pure-tone audiogram (PTA) and unilaterally impaired caloric response was 9% and 12%, respectively. In contrast, AN was found in 36% of patients with pathological brainstem evoked response audiometry (BE-RA). A normal

Synaptic alterations in acoustic cortex in Creutzfeldt-Jacob disease.

The acoustic cortex was studied in electron microscope in 10 cases of Creutzfeldt-Jacob disease. The most prominent finding was the tremendous loss of neurons associated with marked reactive astrocytosis. The so called Cajal-Retzius cell had completely disappeared in the first cortical layer. The neuronal synapses were rare and most of those that were preserved demonstrated marked morphological alterations such as extensive dilatation of the synaptic terminals, synaptic polymorphism, dilatation of the cisternae of the smooth endoplasmic reticulum in the presynaptic terminals and accumulation o
