A prospective study of shoulder pain in primary care: prevalence of imaged pathology and response to guided diagnostic blocks.

BACKGROUND: The prevalence of imaged pathology in primary care has received little attention and the relevance of identified pathology to symptoms remains unclear. This paper reports the prevalence of imaged pathology and the association between pathology and response to diagnostic blocks into the subacromial bursa (SAB), acromioclavicular joint (ACJ) and glenohumeral joint (GHJ). METHODS: Consecutive patients with shoulder pain recruited from primary care underwent standardised x-ray, diagnostic ultrasound scan and diagnostic injections of local anaesthetic into the SAB and ACJ. Subjects who…

Shoulder MRI refinements: differentiation of rotator cuff tear from artifacts and tendonosis, and reassessment of normal findings.

One of the difficulties with rotator cuff imaging lies in the normal variability of the tendon's signal. There may be intermediate signal present within the tendon because of magic-angle phenomenon, muscle and tendon fiber interdigitation, or tendinopathy related to degenerative changes or overuse injury. Partial and complete rotator cuff tears should be distinguishable from these causes of intermediate signal if water signal is reliably identified. This article reviews the important issue of distinguishing between rotator cuff tear and other causes of high signal in the rotator cuff, includin

Relationship between the lateral acromion angle and postoperative persistent pain of distal clavicle fracture treated with clavicle hook plate.

BACKGROUND: The clavicular hook plate is an accepted surgical procedure for distal clavicle fractures. The relationship of the characteristics of the hook plate, acromioclavicular joint and acromion morphology, and clinical outcome has remained poorly understood. We reviewed the clinical records of patients who had distal clavicle fractures with different lateral acromion angles treated using a clavicle hook plate and evaluated their clinical outcomes with respect to shoulder pain and acromial morphology. METHODS: We retrospectively reviewed 102 patients with distal clavicle fractures treated…

Symptomatic coracoclavicular joint: incidence, clinical significance and available management options.

PURPOSE: Coracoclavicular joint (CCJ) is a rare anomalous joint occasionally found between the coracoid process of scapula and the conoid tubercle of clavicle. The articulation has been extensively studied by means of anatomical, osteological and radiological investigations. Most cases are discovered incidentally, with the symptomatic variety remaining an exceptional rarity. Our aim was to review all reported symptomatic CCJ to increase the level of evidence and formulate a treatment algorithm to aid clinicians in management planning. METHODS: A thorough literature search was performed, and d

Radiographically undetected coracoid fractures diagnosed by sonography. Report of seven cases.

Coracoid process fractures are rare and can be overlooked on routine radiographs due to the anatomical orientation of the CP and superimposition of adjacent bone structures. Early fracture detection reduces morbidity and allows for proper treatment. Sonography of the shoulder is usually performed for evaluation of peri-articular soft tissues. To the best of our knowledge, sonographic diagnosis of CP fracture has not been reported in the literature. We present a retrospective review of seven cases of sonographically detected coracoid fractures which were undiagnosed at previous standard radiogr

Ultrasound-guided musculoskeletal interventional procedures around the shoulder.

Ultrasound is a fast, accessible, reliable, and radiation-free imaging modality routinely used to assess the soft tissues around the shoulder. It enables to identify a wide range of pathological conditions. Furthermore, most ultrasound-guided musculoskeletal interventional procedures around the shoulder produce better results in terms of accuracy and clinical efficacy than those performed in a blinded fashion. Indeed, intra-articular and peri-articular interventional procedures can be easily performed under continuous ultrasound monitoring to ensure the correct position of the needle and to de

Functional Results of Unstable (Type 2) Distal Clavicle Fractures Treated with Superior Anterior Locking Plate.

BACKGROUND: The treatment of distal clavicle fracture is always a challenge, as it is mostly unstable and has higher rate of delayed union, malunion, non-union and associated acromioclavicular arthritis. So the management of these fractures remains controversial. The purpose of this study is to evaluate the functional results of Type 2 distal end clavicle fractures treated with superior anterior locking plate. METHODS: From June 2011 to August 2015 a retrospective study of12 male patients (mean age of 41.3 years) 11 with unilateral and 1 with bilateral distal clavicle fractures treated with s

Dexamethasone for pain after outpatient shoulder surgery: a randomised, double-blind, placebo-controlled trial.

BACKGROUND: Dexamethasone has analgesic properties when given intravenously before surgery, but the optimal dose has not been determined. We hypothesised that a dose of 40 mg dexamethasone would improve analgesia after outpatient shoulder surgery compared with 8 mg. METHODS: A randomised, double-blind, placebo-controlled clinical trial was conducted at Horsens Regional Hospital, Denmark. Patients scheduled for arthroscopic subacromial decompression and/or acromioclavicular joint resection as an outpatient procedure (n = 101) were randomised to receive intravenous dexamethasone 40 mg (D40), 8 …

Involvement of the acromion in cases of distal clavicular osteolysis.

OBJECTIVE: Distal clavicular osteolysis (DCO) is a commonly encountered cause of shoulder pain resulting from repetitive overuse or antecedent trauma, classically described in young male weightlifters. We propose a variant of DCO in which osteolysis spans the acromioclavicular joint, involving both the anterior acromion and the distal clavicle. MATERIALS AND METHODS: A retrospective PACS query identified patients with DCO on shoulder MRIs performed at ≥ 1.5 T over a 1-year period. After inclusion/exclusion criteria were applied, each case was reviewed in a blinded fashion to assess for additi…

Postoperative Vision Loss in Shoulder Surgery: A Case Series and Review of the Literature.

Postoperative vision loss (POVL) can be devastating and permanent. We present three unique cases of POVL after shoulder surgery, and we provide recommendations on patient positioning and eye protection perioperatively based on our experience and a review of the literature. In Case 1, our patient underwent an uncomplicated right arthroscopic rotator cuff repair and acromioclavicular (AC) joint resection in the beach-chair (BC) position. He developed bilateral vision loss (right eye greater than the left eye) in the postoperative period, with visual field deficits noted on Humphrey visual field
