Sugar-based colloidal nanocarriers for topical meglumine antimoniate application to cutaneous leishmaniasis treatment: Ex vivo cutaneous retention and in vivo evaluation.

The leishmaniases are a group of diseases caused by protozoan parasites from Leishmania species. Effectiveness therapies for cutaneous leishmaniasis (CL), the most common form, are still needed to be developed since the available drugs such as meglumine antimoniate (MA) present severe adverse reactions. Here, we develop and characterize maltodextrin polymeric colloidal nanocarriers containing MA (PCN-MA) for topical CL treatment. PCN-MA is composed of 5 to 8% maltodextrin, 0.3% NaCl, 1% MA in 21% of water as aqueous-internal phase, containing or no 3% Kolliphor® P-188, and 10% SF1540 dispersed

Treatment of New World cutaneous and mucosal leishmaniases.

The most extensive investigations of treatment of New World cutaneous leishmaniasis have been performed against L. panamensis disease in Colombia, and the relative value of regimens shown there may be instructive for disease from other areas. In Colombia, a 90-95% cure rate was achieved with three different drug regimens: The standard regimen of pentavalent antimony (20 mg/ kg/day for 20 days parenterally) A short course of pentamidine (3 mg/kg every other day for four injections intramuscularly The marketed combination of topical paromomycin (15%)-MBCl (12%) for 10 days, plus antimony (20 mg/

The key to success in blocking lateral cutaneous branches with re-modified thoracoabdominal nerves block through perichondrial approach: a newly discovered space between the endothoracic fascia, diaphragm, and costodiaphragmatic recess.

PURPOSE: This study aimed to determine whether the administration of a modified thoracoabdominal nerves block through perichondrial approach (M-TAPA) could result in the blockade of the lateral cutaneous branches. This study focused on a newly discovered anatomical space/plane adjacent to the M-TAPA plane, which we termed "space between the endothoracic fascia, diaphragm, and costodiaphragmatic recess: SEDIC." METHODS: Thirteen sides of nine formalin-embalmed cadavers were macroscopically dissected to investigate the anatomical spaces related to the effects of M-TAPA. Furthermore, ten adult v

Successful treatment of cutaneous leishmaniasis with intralesional meglumine antimoniate: A case series.

Cutaneous leishmaniasis (CL) is a high-morbidity, vector-borne disease endemic to Colombia. Unlike conventional systemic antileishmanial therapy, intralesional meglumine antimoniate administration has fewer adverse effects and can be as effective and safe. We describe 12 patients treated with intralesional meglumine antimoniate: seven with primary and five with recurrent lesions. The majority (11/12) met all cure criteria after 1-7 sessions of meglumine antimoniate administration (1-5 mL). Adverse effects comprised mainly of local pain and edema. Intralesional meglumine antimoniate administrat

EXPERIMENTAL STUDIES ON THE ADMINISTRATION OF SALVARSAN BY MOUTH TO ANIMALS AND MAN.

Salvarsan in capsule and in solution in doses varying from 0.1 to 0.6 of a gram was administered by mouth to seven patients suffering from syphilis. Five patients were given full doses of 0.6 of a gram. One patient (case 3) received in the aggregate within four weeks 2.4 grams without loss of weight or any disturbing effects. An ulcerating gumma of the knee was practically healed after a period of five weeks. This was the only patient of the seven, however, in whom notable improvement in the cutaneous manifestations followed the administration of the drug. This ulceration would probably have h

Acute generalized exanthematous pustulosis sine pustules.

Acute generalized exanthematous pustulosis (AGEP) is a type of severe cutaneous adverse reaction, typically presenting with sterile pustules in predominantly flexural areas. However, AGEP may present without clinically evident pustules, making histopathologic investigation particularly important in instances of complex or unclear medication administration history.

Treatment management for rosacea: current pharmacological and non-pharmacological options.

INTRODUCTION: Rosacea is a chronic skin condition classified into four subtypes: erythematotelangiectatic, papulopustular, phymatous, and ocular involvement. The physical symptoms and recurrent nature can impact patient quality of life. Effective treatment requires a phenotype-directed strategy that addresses both clinical features and patient concerns. AREAS COVERED: This review summarizes current treatment guidelines and therapeutic options for rosacea, categorized into topical agents, oral medications, procedural interventions, and lifestyle modifications. A literature search was conducted

Reactivity of tixocortol pivalate-positive patients in intradermal and oral provocation tests.

Pivalone/tixocortol pivalate commonly yields positive reactions in the patch test series. The clinical relevance of these positive reactions was investigated in more detail. In the standard patch test series 5.6% (73 of 1306) of the patients were positive to corticosteroids, 5.2% to 0.1% tixocortol pivalate in ethanol (Pivalone nasal spray diluted 1:10) and 2.3% to 1% hydrocortisone butyrate in ethanol. Some of the patients were tested in parallel with Pivalone and 1% tixocortol pivalate in petrolatum. The former test reagent yielded some false-positive reactions, whereas with the latter, some

[The combined therapy of chronic pyoderma taking into account body immunological reactivity and staphylococcal antibiotic resistance].

Summarizing the results of treatment in 23 patients the authors propose an effective two-staged method to treat chronic pyoderma. This method is based on administration of synthetic peptide of thymus--"thymogen", which is used to correct the immune status and increase sensitivity of staphylococci to antibiotics at the zones of cutaneous affection. The second stage of this method consists in administration of antibiotics in combination with stimulators of phagocytosis and humoral immunity. The results prove the high efficiency of thymogen application in complex management of chronic pyoderma: u

Efficacy of additional i.v. immunoglobulin to steroid therapy in Stevens-Johnson syndrome and toxic epidermal necrolysis.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare and life-threatening cutaneous adverse drug reactions. While there is no established therapy for SJS/TEN, systemic corticosteroids, plasma exchange and i.v. immunoglobulin (IVIG) have been used as treatment. The efficacy of IVIG is still controversial because total doses of IVIG used vary greatly from one study to another. The aim of this study was to evaluate the efficacy of IVIG, administrated for 5 days consecutively, in an open-label, multicenter, single-arm study in patients with SJS or TEN. IVIG (400 mg/kg per d

Clinical efficacy of intramuscular meglumine antimoniate alone and in combination with intralesional meglumine antimoniate in the treatment of old world cutaneous leishmaniasis.

Treatment of cutaneous leishmaniasis is often difficult. Even though most cutaneous lesions will heal spontaneously, their duration cannot be predicted in an individual case. In general, only large, multiple or diffuse lesions of the face, head and neck need to be considered for therapeutic intervention. Pentavalent antimonials (sodium stibogluconate and meglumine antimoniate) administered intralesionally or parenterally (IM or IV) are the mainstays of systemic therapy despite the toxicity associated with their use. The objective of this study was to compare the clinical efficacy of intramuscu

Update on the use of antibiotics in cutaneous surgery.

BACKGROUND: Antibiotics are often prescribed in perioperative settings, including dermatologic surgery. Given the continued evolution in the breadth and complexity of cutaneous procedures performed and inevitable localized or distant infections that occasionally occur, it seems prudent to periodically evaluate findings and recommendations from the literature regarding the use of antibiotics in cutaneous surgery. MATERIALS AND METHODS: Literature review from English-language sources from the past 30 years, especially focusing on sources from the past 5 to 10 years. Data were examined for a var

Cutaneous toxic effects associated with vemurafenib and inhibition of the BRAF pathway.

BACKGROUND: The development of a novel BRAF inhibitor, vemurafenib, has been associated with impressive tumor regression in patients with BRAF-positive stage IV melanoma. In the phase 3 clinical trials, dermatologic toxic effects associated with vemurafenib were described, namely, the development of eruptive squamous cell carcinomas. Herein, 3 cases are presented that highlight the development of squamous cell carcinomas and other cutaneous sequelae that have not been previously reported and are reminiscent of those observed with administration of the multikinase inhibitor sorafenib tosylate.

The activity of 2-substituted quinoline alkaloids in BALB/c mice infected with Leishmania donovani.

Potent antileishmanial activity has recently been described in vivo when certain 2-substituted quinoline alkaloids are administered to mice with cutaneous leishmaniasis. We now report the antileishmanial activity of four 2-substituted quinoline alkaloids, namely chimanine D or 2-(1',2'-trans-epoxypropyl) quinoline (I), 2-n-propylquinoline (II), 2-styrylquinoline (III) and 2-(2'-hydroxypropyl) quinoline (IV), for experimental treatment of visceral leishmaniasis in infected BALB/c mice. Subcutaneous treatment with chimanine D for 10 days at 0.54 mmol/kg per day resulted in 86.6% parasite suppres

The evaluation of penile microvascular endothelial function using laser speckle contrast imaging in healthy volunteers.

OBJECTIVE: The primary aim of this study was to evaluate the effectiveness of laser speckle contrast imaging (LSCI) coupled with transdermal iontophoretic delivery of acetylcholine (ACh) for the assessment of penile microvascular function. Additionally, we tested systemic microvascular function using both LSCI and ACh iontophoresis on the forearm. METHODS: We assessed cutaneous, endothelium-dependent microvascular reactivity on the penis and forearm of healthy volunteers (aged 56.6 ± 1.0 years, n = 26) at rest and 60 min following the oral administration of the phosphodiesterase type 5 inhibi

Recent advancements in nanoparticle-based topical drug delivery systems for psoriasis treatment.

Chronic autoimmune skin disorder known as psoriasis (PSO) is typified by the excessive proliferation of skin cells, which develops thick, red and scaly patches on the skin's surface. These patches may be uncomfortable for people with this illness due to their itching and soreness. Treatments for psoriasis try to lessen inflammation, ease symptoms and slow the proliferation of too many skin cells. Traditional treatment methods for psoriasis, including topical corticosteroids, systemic immunosuppressant and biologics, often struggle with issues like poor patient adherence, systemic toxicity, lim

Progress in topical nanoformulations against bacterial skin and soft tissue infections- current trends.

The accelerating rate of antibiotic resistance has always been one of the leading causes of increased skin and soft tissue infections (SSTIs) burden around the globe. Current treatments mainly focus on systemic antibiotics indicated for both uncomplicated and complicated SSTIs that act as a contributing factor secondary to widespread systemic exposure. Topical formulation of antibacterial agents or antibiotics are renowned for their targeted and localised action in the skin which appears as an intriguing clue to the resistance problem. Nevertheless, there are several deterrents associated with
