Comparison of Assisted Hatching Techniques and Their Impact on Clinical Pregnancy Outcomes with Optimization Strategies in In Vitro Fertilization.

BACKGROUND: Infertility is a prevalent reproductive issue, often requiring assisted reproductive techniques (ART) such as in vitro fertilization (IVF). Implantation failure remains a major challenge, especially in women of advanced maternal age, those with previous IVF failures, or embryos with a thick zona pellucida. Assisted hatching (AH) is a technique designed to improve implantation rates by facilitating embryo hatching. However, its effectiveness remains uncertain, particularly concerning live birth rates. OBJECTIVES: This study aims to evaluate the impact of assisted hatching technique…

Scientific production on health inequalities using the Brazilian Information System on Live Births: a scoping review.

The Brazilian Information System on Live Births (SINASC) is widely used by health managers to plan and monitor maternal and child health programs. Scientific research using SINASC data is also essential to identify underexplored topics that warrant further investigation. This study aims to describe the profile of scientific output based on SINASC data and to determine gaps in the literature, with particular attention to studies addressing health inequalities. A scoping review was conducted, including original articles reporting primary outcomes derived from SINASC data. Five databases were sea

Two years of mid-trimester amniocentesis in Johannesburg.

During a 2-year period 438 mid-trimester amniocenteses were performed on women at risk of producing an abnormal infant. The commonest indications were advanced maternal age (61% of cases), neural tube defects (18%) and Down syndrome (11%). A 92,5% follow-up revealed a spontaneous abortion rate of 3% (1,5% if abortions within 8 days of the procedure were considered). Premature birth occurred in 4,4% of cases, neonatal death or stillbirth in 0,7%, and birth defects which were detectable by amniocentesis in 2%. Abnormalities were detected in 4,9% of cases, so that 95,1% of the patients were reasu…

Maternal Cardiac Arrest During Cesarean Section in the Setting of Severe Preeclampsia and Uncontrolled Type 1 Diabetes: A Case Report.

Maternal cardiac arrest is an uncommon but life-threatening complication of pregnancy. Several maternal, social, and obstetric factors have been associated with increased risk, including older maternal age, underlying medical comorbidities, and hypertensive disorders of pregnancy. We present a case of maternal cardiac arrest during cesarean section in a patient with preeclampsia with severe features and uncontrolled type 1 diabetes. This case raises awareness for maternal morbidity and suggests cardiovascular risk and preconception counseling in high-risk obstetrical patients.  A 31-year-old G

Beyond 42 Days: A National Cohort Study of Maternal and Late Maternal Deaths in Brazil from 2010 to 2023.

Maternal mortality is a serious public health problem and reflects social, ethnic, racial, and regional inequalities in access to and quality of obstetric care. Despite advances in the surveillance and investigation of maternal deaths in Brazil, late maternal deaths (occurring between 43 days and 1 year after birth) are still underestimated and underexplored. Therefore, the objective of this study was to analyze the distribution and factors associated with maternal deaths and late maternal deaths in Brazil between 2010 and 2023. This was a population-based, retrospective cohort study with a qu

Factors Influencing Preterm Birth in a Cohort of Predominantly Black Fathers: Preliminary Findings from the Fathers Matter Study.

BACKGROUND: Preterm birth remains a leading cause of infant mortality, with rates disproportionately high among Black women in the United States. While maternal risk factors are well-studied, paternal influences, particularly in Black families, remain underexplored. This study examined associations between paternal factors and preterm birth in a predominantly Black sample. METHODS: We analyzed data from 221 fathers in the Fathers Matter study, embedded within the Biosocial Impact on Black Births (BIBB) cohort study. Paternal sociodemographic, health, behavioral and psychosocial data were coll

The effect of paternal and maternal factors on the prognosis of live birth in couples with recurrent pregnancy loss.

INTRODUCTION: Currently, recurrent pregnancy loss (RPL) examinations focus on the woman, although paternal factors are also involved. Men in couples with RPL have higher sperm DNA fragmentation levels than fertile men, but the effect of sperm DNA damage on couple's later prognosis is unknown. Advanced maternal age and obesity are associated with RPL, but paternal lifestyle factors are less studied. Therefore, we aimed to study the associations of couples' lifestyle factors, causes of RPL, and sperm DNA fragmentation with their prognosis of future live birth. MATERIAL AND METHODS: This descrip…

The impact of sperm DNA fragmentation index levels on embryo development potential and clinical outcomes in infertile couples undergoing IVF/ICSI treatment.

The sperm DNA fragmentation (SDF) impact on embryo development, particularly regarding the fertilization methods and female age, remains inc … potential in a dose-dependent manner, particularly in cycles involving ICSI and among couples with advanced maternal age. However, when the analysis was restricted to fresh embryo transfer, high DFI levels did not result in worse reproductive outcomes.

Assisted reproductive technology represents a possible risk factor for development of epimutation-mediated imprinting disorders for mothers aged ≥ 30 years.

BACKGROUNDS: The proportion of assisted reproductive technology (ART)-conceived livebirths of patients with imprinting disorders (IDs) is higher than that of the general population. Whether this is due to ART or confounding effects of advanced parental age was not investigated. We examined the association of ART and parental ages at childbirth for the development of eight epimutation-mediated imprinting disorders (epi-IDs). RESULTS: We enrolled 136 patients with epi-IDs and obtained general population ART data from the Japanese robust nationwide registry. We compared the proportion of ART-con

Maternal Respiratory Syncytial Virus (RSV) Vaccination: Current Status and Comparison to Monoclonal Antibodies (mAbs) for RSV Prevention in Infants and Children.

Respiratory Syncytial Virus (RSV) causes over 50,000 hospitalizations annually among children under five years of age, leading to long-term consequences, such as asthma. Monoclonal antibodies (mAb) have been recommended for prevention, but their limitations have prompted the search for alternative preventive measures. The recent Food and Drug Administration (FDA) approval of a maternal RSV vaccine with 80% efficacy in protecting infants up to 90 days post-birth marks a significant advancement. Our narrative review investigates the differences in RSV immunization in pregnant mothers versus infa
