Suicide and Psychosis: Results From a Population-Based Cohort of Suicide Death (N = 4380).

Approximately 5% of individuals with schizophrenia die from suicide. However, suicide in psychosis is still poorly characterized, partly due to a lack of adequate population-based clinical or genetic data on suicide death. The Utah Suicide Genetics Research Study (USGRS) provides a large population-based cohort of suicide deaths with medical record and genome-wide data (N = 4380). Examination of this cohort identified medical and genetic risks associated with type of suicide death and investigated the relative contributions of psychotic and affective symptoms to method of suicide. Key differen

Preferences for different insomnia treatment options in people with schizophrenia and related psychoses: a qualitative study.

Symptoms of psychosis such as hallucinations and delusions can be intrusive and unwanted and often remain treatment-resistant. Due to recent progress in basic and clinical sciences, novel approaches such as sleep-based interventions are increasingly becoming offered to address the physical and mental health issues of people with severe mental illness. While the primary outcome is to improve sleep, studies have demonstrated that interventions that target symptoms of insomnia can also produce improvements in the severity of psychotic symptoms, quality of life, and functional outcomes. This study

[Case of graves' disease with remarkable psychiatric symptoms].

A 42-year-old woman visited our hospital with palpitations, excessive sweating, and finger tremors in March 2011. She was diagnosed with Graves' disease based on the following test results: thyroid stimulating hormone < 0.01 μU/ml, free thyroxine 6.15 ng/ml, and thyrotropin receptor antibody 7.8 U/ml. Treatment with methimazole 30 mg and propranolol 30 mg was started, and her thyroid function showed improvement. However, significant manic symptoms, irritability, hallucinations, and delusions were noted, and she was hospitalized for her own protection in May 2011. Although treatment with aripip

Advanced research on dopamine signaling to develop drugs for the treatment of mental disorders: Ser311Cys polymorphisms of the dopamine D2-receptor gene and schizophrenia.

Schizophrenia is a debilitating and complex mental disorder with a prevalence of approximately 1% worldwide. The etiology remains unclear, despite massive research efforts. Hyperactive dopaminergic signal transduction in the central nervous system is suggested to be involved in the pathophysiology of schizophrenia (the dopamine hypothesis). The dopamine D(2)-receptor (DRD2) gene is thus a promising candidate for associations with risk of schizophrenia. We investigated DRD2 and found a novel missense nucleotide change causing an amino acid substitution of serine with cysteine at codon 311 (Ser3

Identifying hypomanic features in major depressive disorder using the hypomania checklist (HCL-32).

BACKGROUND: Recent studies have challenged the traditional unipolar/bipolar divide with increasing support for a more dimensional view of affective disorders. We here examine the occurrence of hypomanic symptoms in individuals with a history of major depression selected to exclude indicators of underlying bipolarity. METHODS: The presence of hypomanic symptoms was assessed by the Hypomania Checklist (HCL-32) self-report questionnaire in a sample of almost 600 patients meeting DSM-IV criteria for Bipolar I disorder (BPI N=260) or Major Recurrent Depressive disorder (MDDR N=322). Subjects were

[Psychosis revealing a silent celiac disease in a young women with trisomy 21].

INTRODUCTION: Down's syndrome is characterized by an abnormal frequency of coeliac disease and by the frequent occurrence of neurological disorders such as Alzheimer-type dementia of early onset. However, psychosis is rare in Down's syndrome. OBSERVATION: We report the case of a 41-year-old woman, who presented with Down's syndrome. She was living with her parents and had a normal social life. She suddenly experienced some esthesic hallucinations, depression, anorexia, affective flattening and autistic behavior. Biological evaluation revealed macrocytosis, polyclonal IgA and IgG hypergammaglo

The Underlying Neurobiological Mechanisms of Psychosis: Focus on Neurotransmission Dysregulation, Neuroinflammation, Oxidative Stress, and Mitochondrial Dysfunction.

Psychosis, defined as a set of symptoms that results in a distorted sense of reality, is observed in several psychiatric disorders in addition to schizophrenia. This paper reviews the literature relevant to the underlying neurobiology of psychosis. The dopamine hypothesis has been a major influence in the study of the neurochemistry of psychosis and in development of antipsychotic drugs. However, it became clear early on that other factors must be involved in the dysfunction involved in psychosis. In the current review, it is reported how several of these factors, namely dysregulation of neuro

A plea for the diagnosis of hypochondriacal personality disorder.

Hypochondriacal personality disorder is identified as a separate condition by cluster analysis of data from a structured interview for assessing personality disorders, the Personality Assessment Schedule. Close examination of the patients identified as having hypochondriacal personality disorder suggests that they have common characteristics, including excessive preoccupation with the maintenance of health, distorted perception of minor symptoms so that they are elevated to the status of major disease, and frequent medical consultations and disciplines fringing on medicine. In a study of 1000

Vitamin B12 and folate status in acute geropsychiatric inpatients: affective and cognitive characteristics of a vitamin nondeficient population.

This chart review study examined the serum vitamin B12 and folate status of 102 geriatric patients newly admitted to a private psychiatric hospital. Only 3.7% were B12 deficient and 1.3% were folate deficient; 4% were anemic. Nevertheless, those with below-median values of both vitamins had significantly lower Mini-Mental State scores than patients higher in one or both vitamins. Patients with "organic psychosis" with a negative family history for psychiatric disorder had significantly lower B12 levels than those with a positive family history. In major depression, folate levels correlated neg

Clinical and cognitive factors affecting psychosocial functioning in remitted patients with bipolar disorder.

Impaired interpersonal, social, and occupational functioning is very often observed in patients with bipolar disorder, not only at the acute stages of the illness but in remission as well. This finding raises the question of multiple factors that might affect psychosocial functioning in bipolar patients, such as residual subsyndromal symptoms and neuropsychological deficits. Social cognition impairment, especially impaired Theory of Mind (ToM), might also play an important role in bipolar patients' every-day functioning, similarly to what was found in patients with schizophrenia. The present s

[Homicide by mentally ill: clinical and criminological analysis].

The present study analysed the characteristics of homicide in internees according to the Social Defence system in Belgium. The Social Defence system was inaugurated in 1930, following the ideas of Adolphe Prins, a Belgian specialist in the criminal law. The Social Defence system concerns those offenders who are considered as mentally ill at large. The concept of mental illness encompasses the classical notion of "dementia" but also those people with mental unbalance as personality disordered offenders and mentally retarded persons. In the present study, we considered all those internees who co

["Endogeniform" affective psychoses and normal pressure hydrocephalus].

Four patients with neuroradiological evidence of normal pressure hydrocephalus and psychiatric diagnosis of affective psychoses with major depression--like characteristics are presented. Three patients underwent shunting operations following an observation period of 2 to 6 years. During the post-operative follow-up of 3 to 7 years, resp., no clinical changes were noted. In our opinion, and following critical evaluation of the available literature, major depression--like affective disorders in normal-pressure hydrocephalus are not influenced by shunting operations, unless they occur simultaneou

[Family background of patients with slowly progressive childhood schizophrenia with a syndrome of pathologic fantasizing].

The authors studied the familial background of 50 probands suffering from slowly progressive infantile schizophrenia with the morbid fantasy making syndrome. A total of 110 firstkin relatives (100 parents and 10 siblings) were examined. The incidence of manifest psychoses in parents was 16% (8 persons) in the first group of probands and 6% (3 persons) in the second group. "Subclinical forms of psychosis" and personality abnormalities of the type of sthenic schizoids were also prevalent among the relatives of the second group probands (22% and 28%, respectively). Parents denoted as schizoids wi

[Communicating hydrocephalus. Clinical aspects, diagnosis, pathogenesis].

A description of the clinical symptoms of the communicating hydrocephalus (normal-pressure hydrocephalus = NPH) is given: progressive psycho-organic alterations, ataxia, tetraspastic signs, urinary incontinence, without increased intracranial pressure. Diagnosis can be confirmed by use of pneumencephalography, isotope-cisternography and tomography of the brain by use of a water-solubile contrast fluid. Skull radiographs. EEG, testing of liquor and psychological examinations are of less diagnostic value. The illness is the result of a disturbance of liquor resorption and circulation. In this co

Patterns of Management of Patients With Dual Disorder (Psychosis) in Italy: A Survey of Psychiatrists and Other Physicians Focusing on Clinical Practice.

Patients with severe psychotic disorders such as schizophrenia, schizoaffective, and bipolar disorders frequently suffer from concomitant substance use disorders (SUDs)-Dual Disorder (DD) patients. In order to better understand current practices for management of patients with psychotic episodes and concomitant SUD in Italy, we carried out a survey of psychiatrists on current routine practice among prescribers. These aspects can help to identify at-risk patients, improve current prescribing practices, and favor early intervention. An ad hoc survey of 17 questions was administered to psychiatri

[Organic personality disorder as a form of presentation of Huntington's disease].

The psychiatric manifestations of certain neurological diseases frequently are unfamiliar to psychiatrists, which often makes it difficult to select the appropriate therapeutic approach. The clinical case presented exemplifies a case of Huntington disease (HD) which debut with insidious psychiatric symptoms. The most frequent psychiatric presentations in HD are affective and psychotic; however, our case would be better defined as organic personality disorder. This diagnosis in the context of HD is uncommon in the literature.

Birth complications and subsequent negative symptoms in schizophrenia.

A sample of 29 men with an initial psychotic episode and diagnosis of schizophrenia as young adults were followed into middle adulthood. All had prior child guidance clinic contact. The negative symptoms of affect deficit in young adulthood but not the positive symptoms of thought disorder were predicted by a history of birth complications. Subjects with birth complications who developed negative symptoms had a stable deficit over the follow-up period.

The impact of child sexual abuse on the course of bipolar disorder: a systematic review.

OBJECTIVES: The aim of this review was to elucidate the impact of child sexual abuse on all clinical phenomena that occur after the onset of bipolar disorder, including associated clinical features that are not part of the diagnostic criteria for the disorder. METHODS: Five databases were searched and supplemented with a hand search of reference lists from retrieved papers. Study quality was assessed using a validated quality assessment tool. Blind assessments of study eligibility and quality were conducted by two independent researchers to reduce bias, minimize errors, and enhance the reliab

Impact of childhood adversities on specific symptom dimensions in first-episode psychosis.

BACKGROUND: The relationship between childhood adversity (CA) and psychotic disorder is well documented. As the adequacy of the current categorical diagnosis of psychosis is being increasingly questioned, we explored independent associations between different types of CA and specific psychotic symptom dimensions in a well-characterized sample of first-episode psychosis (FEP) patients. METHOD: This study involved 236 FEP cases aged 18-65 years who presented for the first time to psychiatric services in South London, UK. Psychopathology was assessed with the Positive and Negative Syndrome Scale
