Alcoholic and non-alcoholic steatohepatitis: global perspective and emerging science.

Alcohol and high-fat diet are two major risk factors responsible for metabolic diseases, which are manifested as steatohepatitis and liver cancer in the liver, and chronic pancreatitis and pancreatic adenocarcinoma (PDAC) in the pancreas. These metabolic diseases are becoming increasingly prevalent around the globe, and more importantly, their two major etiologies commonly coexist to precipitate the disease processes. To highlight the importance of these metabolic diseases, Japanese Society of Gastroenterology (JSGE) and National Institute on Alcoholism and Alcohol Abuse of National Institute

Prescription Drug and Alcohol Use Disorders: Alcohol Use Disorder.

Alcohol use is a common contributor to medical disease and a leading cause of preventable death. Because brief interventions for unhealthy alcohol use are effective, and because risk factors for unhealthy alcohol use are not always immediately identifiable, universal screening is recommended for adults. For patients who screen positive, physicians should assess further to diagnose alcohol use disorder, determine risk of withdrawal symptoms, and recommend an appropriate level of treatment. Outcomes of alcohol use disorder management are improved with evidence-based psychosocial therapy, but the

Use of alcohol biomarkers to identify alcohol misuse in organ donors.

Phosphatidylethanol is a direct alcohol biomarker for identifying alcohol misuse. It carries several advantages over other alcohol biomarkers, including a detection half-life of several weeks and little confounding by patient characteristics or organ dysfunction. The aim of this study is to derive an optimal phosphatidylethanol cut point to identify organ donors with alcohol misuse, and to assess the impact of alcohol misuse on organ allocation. Discrimination of phosphatidylethanol was evaluated using the area under the ROC curve from a mixed effects logistic regression model. Phosphatidyleth

[Fetal alcohol syndrome is not the only consequence of prenatal alcohol exposure].

Fetal alcohol syndrome is not the only consequence of prenatal alcohol exposure  The prevalence of fetal alcohol syndrome and fetal alcohol spectrum disorders in larger communities in USA is now updated to 0.4 % and 4.8 % respectively. Affected individuals bear witness to disease symptoms from many organ systems in addition to the brain and behavioural dysfunctions. In the light of modern epigenetic research, early alcohol exposure appears to play a hidden role in fetal reprogramming. The underlying mechanisms explain the »developmental origin of health and disease«, which has an impact on com

Alcohol Vapor Inhalation as a Model of Alcohol-Induced Organ Disease.

BACKGROUND: Chronic intermittent ethanol vapor (CIEV) exposure has been used extensively to produce rodent models of alcohol dependence, but unlike other models of alcohol abuse, CIEV has not been assessed as a model of end-organ damage. The purpose of this study was to characterize the effects of CIEV on peripheral organ systems affected by alcohol abuse, including the liver, lungs, and cardiovascular system. METHODS: Adult male Sprague-Dawley rats were exposed to daily CIEV for a period of 8 weeks (14HR ON/10HR OFF), producing blood alcohol levels of ~200 mg/dl. Controls were exposed to roo

Alcoholic Ketosis: Prevalence, Determinants, and Ketohepatitis in Japanese Alcoholic Men.

AIMS: Alcoholic ketosis and ketoacidosis are metabolic abnormalities often diagnosed in alcoholics in emergency departments. We attempted to identify determinants or factors associated with alcoholic ketosis. METHODS: The subjects of this cross-sectional survey were 1588 Japanese alcoholic men (≥40 years) who came to an addiction center within 14 days of their last drink. RESULTS: The results of the dipstick urinalyses revealed a prevalence of ketosis of 34.0% (±, 21.5%; +, 8.9%; and 2+/3+; 3.6%) in the alcoholics. Higher urine ketone levels were associated with higher serum total bilirubin,

Focus on: biomarkers of fetal alcohol exposure and fetal alcohol effects.

One of the ongoing challenges for the accurate diagnosis and treatment of children with fetal alcohol spectrum disorders (FASD) is the difficulty of confirming whether a mother drank during her pregnancy. Commonly used screening questionnaires often are unreliable, and current established biomarkers of alcohol consumption are not sensitive enough for use with many pregnant women. These limitations underscore the critical need to develop novel biomarkers with greater sensitivity for detecting moderate levels of drinking during pregnancy for longer periods of time after the last drinking episode

Women's alcohol consumption and risk for alcohol-exposed pregnancies in Russia.

AIMS: Alcohol-exposed pregnancies (AEP) are the direct cause of fetal alcohol spectrum disorders (FASD). This study examines drinking patterns among pregnant and non-pregnant women of childbearing age in Russia, a country with one of the highest levels of alcohol consumption in the world. DESIGN: Cross-sectional survey. SETTING: Seven public women's clinics in two locations: St Petersburg (SPB) and the Nizhny Novgorod region (NNR). PARTICIPANTS: A total of 648 pregnant and non-pregnant childbearing-age women. MEASUREMENTS: A face-to-face structured interview assessed alcohol consumption, p

Alcohol-induced Cushing syndrome. Hypercortisolism caused by alcohol abuse.

BACKGROUND: Cushing's syndrome (CS), a rare syndrome caused by overexposure to glucocorticoids, is difficult to diagnose. The underlying causes of CS include pituitary and ectopic adrenocorticotropic hormone (ACTH) producing tumours and adrenal adenomas or hyperplasia. Alcoholism, however, can cause similar symptoms, giving rise to a so-called pseudo-Cushing state, which aggravates the differential diagnostic dilemmas of CS. AIM: To document any specific clinical or biochemical features of alcohol-induced CS. METHODS: A Medline computer-aided search was performed to identify studies that hav

Alcohol abstinence and orthotopic liver transplantation in alcoholic liver cirrhosis.

Patients diagnosed with acute alcoholic hepatitis (AAH) are routinely managed medically and not considered suitable for orthotopic liver transplantation (OLT). The eligibility for OLT in these patients has been questioned due to the social stigma associated with alcohol abuse, based on the fact that AAH is "self-induced" with an unacceptably high recidivism rate. Many centers in Europe and the United States require abstinence periods between 6 and 12 months before OLT listing. AAH outcomes in the literature are poor, in particular due to patient noncompliance during the immediate 3 months prec

Estimating risk of alcohol dependence using alcohol screening scores.

Brief alcohol counseling interventions can reduce alcohol consumption and related morbidity among non-dependent risky drinkers, but more intensive alcohol treatment is recommended for persons with alcohol dependence. This study evaluated whether scores on common alcohol screening tests could identify patients likely to have current alcohol dependence so that more appropriate follow-up assessment and/or intervention could be offered. This cross-sectional study used secondary data from 392 male and 927 female adult family medicine outpatients (1993-1994). Likelihood ratios were used to empirical

Alcohol and adult neurogenesis: roles in neurodegeneration and recovery in chronic alcoholism.

The concept of "structural plasticity" has emerged as a potential mechanism in neurodegenerative and psychiatric diseases such as drug abuse, depression, and dementia. Chronic alcoholism is a progressive neurodegenerative disease while the person continues to abuse alcohol, though clinical and imaging studies show that some recovery may occur with abstinence. The neural plasticity observed in chronic alcoholism coupled with conflicting reports on alcohol-induced hippocampal neuropathology make this disease ripe for reconsideration in terms of the phenomenon of adult neurogenesis. This review d

Severe alcohol-induced liver disease and the alcohol dependence syndrome.

AIMS: To examine the hypothesis that patients who present in the UK to detoxification services differ from patients who present with severe alcohol-induced liver disease (ALD) with respect to severity of dependence on alcohol and other features of their drinking history. METHODS: Levels of alcohol dependence were assessed in 34 patients with severe ALD, all of whom were cirrhotic, and 34 subjects from a residential alcohol detox centre in Southampton using the Severity of Alcohol Dependence Questionnaire (SADQ). During interview, various aspects of participants' alcohol consumption were recor

The association of alcohol-induced blackouts and grayouts to blood alcohol concentrations.

The primary aim of this study was to investigate the association between measured blood alcohol concentration (BAC) and the presence and degree of amnesia (no amnesia, grayout, or blackout) in actively drinking subjects. A secondary aim was to determine potential factors other than BAC that contribute to the alcohol-induced memory loss. An interview questionnaire was administered to subjects regarding a recent alcohol associated arrest with a documented BAC greater than 0.08 g/dL for either public intoxication, driving under the influence, or under age drinking was administered. Demographic va

Determinants of alcohol use in pregnant women at risk for alcohol consumption.

The purpose of this investigation was to identify determinants of alcohol consumption based on a number of demographic and psychosocial variables in a group of pregnant women at risk for alcohol consumption. Data were collected on a sample of 232 pregnant females who agreed to participant in a multistate alcohol prevention intervention. The variables of interest included demographic measures of race, age, education, marital status, health status, employment status and if they had been involved in physical abuse during the past year. Additionally, psychosocial variables were collected on social

Fetal alcohol syndrome: does alcohol withdrawal play a role?

Alcohol use by a pregnant woman may interfere with the development of her fetus. Newborns whose mothers are intoxicated during delivery can experience withdrawal symptoms, such as tremors and even seizures. It is likely that withdrawal also can occur during fetal development. Thus, the possibility exists that withdrawal by the pregnant woman may exacerbate alcohol's adverse effects on her fetus. One potential mechanism through which alcohol withdrawal might damage the fetus involves the receptor for the neurotransmitter glutamate (i.e., the N-methyl-D-aspartate [NMDA] receptor). This receptor

[Acute rhabdomyolytic alcoholic myopathy associated with acute alcoholic hepatitis].

Acute rhabdomyolysis (ARM) is a clinico-biological syndrome characterized by the lesion of the skeletal muscle resulting in a disorder of the permeability at the membrane of the muscle cells, with the release of intracellular substances to the plasma, mainly proteins and ions. We present the case of a chronic alcoholic patient which, after a period of several days in which he increased his consumption of alcohol, developed clinical signs of acute rhabdomyolytic alcoholic myopathy (AREM) associated to acute alcoholic hepatitis (AAH). In the review of the literature, we did not find any descript
