Xenograft bone substitutes are commonly used to increase bone volume and height in the deficient posterior maxilla. The addition of enamel matrix derivate (Emdogain) could increase the efficiency of the bone healing process. The aim of this prospective randomized, controlled split-mouth design study was to compare the percentage of newly formed bone in sinus floor augmentation with deproteinized bovine bone mineral with or without the addition of enamel matrix derivative after 6 months of healing. Sixteen bilateral sinus floor augmentation procedures were performed. Deproteinized bovine bone m

BACKGROUND: While several studies have compared the performance of alveolar ridge preservation (ARP) using different bone substitutes in terms of histological outcomes and maintenance of ridge dimensions, there is a lack of data comparing implant outcomes in ARP-treated sites with different grafts. The aim of this retrospective cohort study was to compare the performance of dental implants placed in areas that underwent ARP with either allograft or xenograft, focusing on marginal bone loss, implant failure, and peri-implantitis.
METHODS: Patient records were examined for those who received at

An esthetic implant-supported rehabilitation continues to be a major challenge in patients with a thin periodontium. Ridge preservation and immediate implant placement are intended to preserve the hard tissue volume and prevent preimplant bone loss following tooth extraction. Since these techniques are almost always combined with bone grafting, primary wound closure is indispensable. Therefore, a technique for reliable wound closure was developed. This technique employs a combined epithelized-subepithelial connective tissue graft, leaves the mucogingival line in its place, and has the added ad

Part I of "Revisiting the Hopeless Ridge" highlighted the higher complication rates, greater resorption profile, and lower implant success rates associated with autogenous block grafts. The conclusions described in that article were based on a comprehensive literature review, rather than an individual clinician's experience in clinical practice. Additionally, the idea that such grafts were the gold standard for traditional dental implant-associated bone regeneration was challenged. This article explores the advantageous properties of new commercially available allograft bone in a variety of cl

BACKGROUND: Autogenous osseous transplants have advantages, including the maintenance of bone density and high concentrations of growth factors. Little is known about the potential long-term influence of periodontal biotype on the volume maintenance of block grafts.
METHODS: Forty patients who received autogenous block transplants prior to implant placement were included in the study. Digital photographs and periapical radiographs were taken after restoration. Clinical parameters, including bleeding on probing, mobility, suppuration, mucosal recession, and buccal tissue transparency, were rec

Based on clinical experience, the minimum required alveolar ridge width must be of 5-6 mm in patient evaluation for the placement of endosseous dental implants. In comparison with other techniques, the use of autogenous mandibular chin grafts to enlarge thin alveolar ridges seems to be very efficient with a success rate of 88.57%. Advantages are: only one surgical field; minimal resorption after grafting; good predictability; minimum postoperative complications. The surgical technique is described step by step and a longitudinal study in 35 patients shows the high predictability of the method.

The popularity of osseointegration for the replacement of anterior teeth has placed an increased demand on the preservation of normal gingival anatomy. This has led to the development of a variety of soft-tissue and bone-augmentation techniques that either prevent tissue collapse or restore previously damaged areas of the alveolus. With bone grafts, barrier membranes, and autogenous connective-tissue grafts used in combination with each other or separately, lost structures can be reconstructed into a normal configuration for optimum esthetic restorations.

Guided bone regeneration is a new technique that evolved following the guided tissue regeneration procedure for regeneration of lost periodontium. The objective of guided bone regeneration is to promote bone formation in osseous deformities either before or in conjunction with endosseous implant placement. Osseous defects consist mainly of extraction sites, dehiscences or fenestrations, and localized ridge deformities. In addition, bone defects may either provide natural spacemaking or be nonspacemaking. A plethora of publications indicate that the guided bone regeneration technique can be use

The purpose of this study is to present results obtained with a new procedure for reconstruction of the severely atrophied maxillary alveolar ridge that involves the use of intramembranous corticocancellous bone grafts obtained from the mandibular symphysis fixed to the residual bone by endosseous implants. A total of 107 implants were installed in grafted regions in 26 patients. The follow-up period ranged from 6 to 32 months, with a mean of 16 months. In partially edentulous patients the bone grafts were fixed with implants to the residual bone as 1) onlay graft to the alveolar ridge (8 impl

OBJECTIVES: This case series evaluated the clinical efficacy of the novel "lateral approach" combined with an enamel matrix derivative (EMD) and bone grafting in the regenerative surgical treatment of intrabony defects associated with an edentulous ridge.
MATERIAL AND METHODS: The innovative flap, called the "lateral approach," is explicitly designed for regeneration of unchallenged isolated intrabony defects associated with edentulous alveolar ridges. The flap is defined by a curved vertical incision on the buccal side opposite the treated defect and a sulcular incision on the buccal and def

The aim of this pilot study was to present a novel technique for the management of the Schneiderian membrane during maxillary sinus lift surgery using plasma rich in growth factors (PRGF). Eight maxillary sinuses were augmented in 8 patients. Two small perforations of the Schneiderian membrane occurred during the lifting procedure, which were solved using the PRGF clot before grafting the site with PRGF and anorganic bovine bone. With the exception of 1 patient who experienced pain following an acute sinus infection after 3 days of uneventful healing, the patients' postoperative quality of lif

Both hard and soft tissue undergo change after tooth extraction. In particular, the bone tissue surrounding teeth with fenestration or dehiscence defects undergoes dramatic change following tooth extraction, which can compromise further rehabilitation of the area. Adequate alveolar bone volume and keratinized mucosa are critical to the success of implant therapy. Therefore, the anatomic dimension of the alveolar ridge must be adequate to achieve an esthetically acceptable outcome of implant therapy. Previous studies have proposed many clinical techniques for preserving the extraction socket. T

This case study was conducted to compare the regenerative potential of bovine xenograft versus demineralized freeze-dried bone allograft in a thermoplastic biologic carrier in regards to achieving regeneration of bone width as well as potential bone height. The objective for the patient was to gain adequate bone volume for the placement of dental implants using the two different forms of graft material--one with a slow resorption time and the other in a gelatin matrix to provide dimensional stability. The results of this case report are inconclusive to demonstrate the superiority of one of the

UNLABELLED: Anterior maxillary implantation is a challenging treatment for both the surgeon and prosthodontist due to high esthetic demands in this area. However, it is the most traumatized and most exposed region to habits. Prompt and appropriate management can significantly improve prognosis of many dentoalveolar injuries, especially in young patients. Unfortunately, many traumatized teeth are overtreated or left untreated, which lead to a much more complicated treatment at the time of permanent restoration at adulthood. The facial cortical plate over the roots of the maxillary teeth is thin

Before dental implants are placed, an evaluation of a presurgical bone site with tomograms will reveal information not available from panoramic or periapical radiographs. This article illustrates the importance of making tomograms before the placement of dental implants to determine the actual height, width, inclination, and undercut of alveolar bone; the shape, cortication, and irregularities of crestal alveolar bone; the density of alveolar bone; the relative location of anatomical landmarks, such as mandibular canal, maxillary sinus, nasal fossa, incisive canal, submandibular gland fossa, e

In narrow alveolar ridges, dehiscences can occur at the time of implant placement. This report describes several cases in which buccal dehiscences at one-stage, nonsubmerged ITI implants were covered with deproteinized bovine bone mineral (Bio-Oss) and a nonresorbable e-PTFE membrane. The dehiscences ranged from 5 to 8.5 mm. The head of the implant perforated the membrane and was not covered by the mucoperiosteal flap. Membranes were removed after 12 to 20 weeks. One of the seven implants failed to osseointegrate. In four cases, no residual defect was found (100% coverage). In two cases, 63.5%
