Epidural Injections for Lumbar Radiculopathy or Sciatica: A Comparative Systematic Review and Meta-Analysis of Cochrane Review.

BACKGROUND: Epidural injections are one of the commonly performed procedures in managing low back and lower extremity pain. In the past, Pinto et al and Chou et al performed systematic reviews and meta-analyses with a recent update from Oliveira et al showing lack of effectiveness of epidural steroid injections in managing lumbar radiculopathy. In contrast, multiple other systematic reviews and meta-analyses have supported the efficacy and use of epidural injections utilizing fluoroscopic guidance. STUDY DESIGN: A systematic review and meta-analysis of randomized controlled trials (RCTs) of e

Serious complications related to regional anesthesia: results of a prospective survey in France.

BACKGROUND: Serious complications related to regional anesthesia have previously been described primarily in case reports and retrospective surveys. The authors prospectively evaluated a multicenter series of regional anesthetics, using preplanned criteria to measure the incidence and characteristics of associated serious complications. METHODS: Requests were sent to 4,927 French anesthesiologists in advance of a subsequent 5-month study period. Participating anesthesiologists were asked for detailed reports of serious complications occurring during or after regional anesthetics performed by

Dexmedetomidine Increases the Latency of Thermal Antinociception in Rats.

Recent reports have stated that dexmedetomidine (DEX), an α2-adrenoreceptor agonist, enhances the local anesthetic effects of ropivacaine and prolongs its effective duration. However, little is known about the effect of a combination of DEX and lidocaine on anesthetic duration. Therefore, we investigated whether DEX can prolong the local anesthetic effect of lidocaine, using the thermal paw withdrawal test in Wistar rats in order to measure local anesthetic duration. We subcutaneously injected 50 μL of either normal saline, 2% lidocaine, a combination of 0.5 μg/kg DEX and 2% lidocaine, or a co

Lidocaine-prilocaine cream for local-anesthesia chalazion incision in children.

BACKGROUND: Chalazia are not uncommon in children. If conservative therapy for them fails, incision and drainage are necessary and usually require a general anesthetic, which increases costs and risks. PATIENTS AND METHODS: Five children (3 to 8 years old) had a eutectic mixture of lidocaine and prilocaine applied to the skin of the eyelid for 1 hour. Then local anesthetic was injected into the same site. RESULTS: The local anesthetic injection was painless, permitting successful incision and drainage in all cases. CONCLUSION: Lidocaine-prilocaine cream permits local anesthetic injection in

Anesthesia in malignant hyperthermia susceptible patients.

The intra- and postoperative course of 30 general and 3 regional anesthetics in 27 MH-carriers verified by in vitro contracture tests is reported. None of the patients received dantrolene prophylactically. Disposable tubings were used for ventilation, vaporizers and soda lime were removed. ECG, esophageal temperature, blood pressure, oxygen saturation, and end tidal pCO2 were monitored. Minor tranquilizers were offered for premedication. Fentanyl, thiopentone, nitrous oxide, non depolarizing relaxants, neuromuscular antagonists and naloxone were used. In three patients, surgery was performed d

Differential effects of phenobarbital and pentobarbital on isolated nervous tissue.

Epileptiform after discharges evoked by repetitive electrical stimulation of chronically isolated cortical slabs (cat) were shortened by low doses of phenobarbital but not affected by hypnotic doses of pentobarbital. Both pentobarbital and phenobarbital raised threshold and lowered spike amplitude in isolated sciatic nerves. The action of both drugs was increased by reducing Na in the medium and by decreasing the Ringer's pH. Similar to the action of other general anesthetics, the axonal effect of pentobarbital was enhanced by D2O replacement for H2O in the Ringer's (suggesting that tissue wat

Paresthesias in dentistry.

Alterations to normal oral sensory function can occur following restorative and surgical dental procedures. Paresthesia is defined as an abnormal sensation, such as burning, pricking, tickling, or tingling. Paresthesias are one of the more general groupings of nerve disorders known as neuropathies. This article reviews the extent of this oral complication as it relates to dental and surgical procedures, with specific emphasis on paresthesias associated with local anesthesia administration. This review establishes a working definition for paresthesia as it relates to surgical trauma and local a

Protective effects of anesthetics on the spinal cord.

Since several clinical data have suggested that the incidence of neurological deficit after aortic surgery has not changed appreciably over the last 50 years, anesthesiologists as well as vascular surgeons have attempted to resolve this clinically important issue by employing various strategies to prevent ischemic spinal cord injury. With respect to inhalational anesthetics, it is thought that isoflurane as well as sevoflurane preconditioning might provide neuroprotective effects against spinal ischemia via activation of TWIK-related K channels-1 or the potassium ATP channel. Glutamate recepto

[In vitro effects of anesthetics on hydrogen peroxide generating system of the thyroid gland].

Effects of anesthetics on thyroidal hydrogen peroxide generating system was studied using porcine thyroid follicles. This system is indispensable in thyroid hormone synthesis and seems also to play an important role in regulation of thyroid hormone release. The results indicated that H2O2 generation was controlled by Ca2+ ion mobilization and protein kinase-C linked signal transduction system. It was also demonstrated that thiopental, halothane, and enflurane had inhibitory effects on H2O2 generation system in thyroid follicle cells, and 50% inhibitory concentrations (IC50) of each anesthetic

Should lidocaine routinely be used to prevent pain on propofol injection during induction of general anaesthesia or sedation?

Anaesthetists commonly debate whether lidocaine should be added to propofol for anaesthetic induction. The benefits include pain relief and improved patient satisfaction, but disadvantages include the effect on the emulsion itself, the efficacy of propofol once it has been mixed, and the effect on growth of bacteria in the syringe.

Microdermabrasion with Calcium Hydrogen Phosphate for Enhancing Skin Permeation of Lidocaine-tetracaine Eutectic Microemulsion Gel.

BACKGROUND: Transdermal drug delivery (TDD) offers a convenient administration for treating local or systemic diseases. However, the dense "brick-mortar" structure of the stratum corneum hinders the skin permeation of most of bioactive molecules, which highly constrained the application of TDD. METHODS: This study analyzed stratum corneum disruption and enhanced permeation by gently rubbing calcium hydrogen phosphate (Chp) particles of varying sizes on skin. Enhanced anesthetic efficacy of Chp-containing microemulsion gel (MG) loaded with lidocaine-tetracaine eutectic was evaluated in guinea

Topical Anesthetics and Premature Ejaculation: A Systematic Review and Meta-Analysis.

This meta-analysis was conducted to assess the effectiveness of topical anesthetics in preventing premature ejaculation. We conducted an online database search for original studies comparing topical anesthetic agents with placebo in patients with premature ejaculation. After selecting relevant articles, we extracted data on baseline characteristics and predetermined endpoints. Intravaginal ejaculatory latency time (IELT) was the primary outcome for efficacy. Mean differences and corresponding 95% confidence intervals were used to present continuous data. A random-effects model was used to pool
