Ankle and foot tuberculosis: a diagnostic dilemma.

AIM AND OBJECTIVE: To know the biological behavior of ankle and foot tuberculosis (AFTB) and to know the reasons for delay in diagnosis and treatment of AFTB in our population. MATERIALS AND METHODS: Patients with non-healing ulcers/sinuses/swellings in the ankle and foot region are the subjects of present study. Detailed clinical history, physical examination and relevant investigations were done in all cases. Pus/wound discharge for acid fast bacillus (AFB) study and biopsy from wound margin/sinus tract was taken in all the cases. RESULTS: During the period from July 2007-June 2012, 20 cas…

[EFFECTIVENESS OF ARTHROSCOPY FOR ANKLE IMPINGEMENT SYNDROME].

OBJECTIVE: To explore the effectiveness of arthroscopy for ankle impingement syndrome. METHODS: Between March 2009 and April 2013, 30 patients with ankle impingement syndrome were treated. Among them, there were 22 males and 8 females with an average age of 28.6 years (range, 16-55 years). Twenty-six patients had a history of obvious ankle sprains. The disease duration was 6-62 months (mean, 21.5 months). All cases had ankle pain, limitation of activity, and positive results of ankle impact test. According to Meislin scoring criteria, 5 cases were rated as good, 8 cases as medium, and 17 case…

Ultrasound findings of the painful ankle and foot.

OBJECTIVES: To document the prevalence and spectrum of musculoskeletal ultrasound (MSKUS) findings at different parts of the foot. MATERIALS AND METHODS: All MSKUS studies conducted on the foot during a 2-year period (2012-2013) at the Department of Radiology were reviewed. Demographic parameters including age, gender, and MSKUS findings were documented. RESULTS: Three hundred and sixty-four studies had been conducted in the 2-year period. Ninety-three MSKUS evaluations were done for the ankle, 30 studies for the heel, and 241 for the rest of the foot. The most common MSKUS finding at the an…

MR imaging of the paediatric foot and ankle.

Radiography is the mainstay for initial evaluation of paediatric foot and ankle pathology. MRI is the preferred exam for further characterisation of the majority of these conditions. The modality features high sensitivity and specificity for this purpose with few exceptions. Findings on MRI will often dictate patient referral and further management, and are frequently required for surgical planning. This article will provide an overview of a variety of pathologies that afflict the foot and ankle in children. These include tarsal coalition, osteochondral lesions, osteonecrosis, osteochondroses,…

Posterior ankle impingement in dancers and athletes.

The diagnosis of posterior ankle impingement requires an accurate history and specific examination. Computed tomography is a useful investigation to diagnose bony impingement, especially where plain radiography and/or magnetic resonance imaging are sometimes inconclusive. Accurate ultrasound-guided steroid/anesthetic injections are useful interventions to locate the symptomatic lesions and reduce symptoms and occasionally prove curative. If surgical debridement or excision is deemed necessary, arthroscopic surgery via a posterior approach is recommended to excise impingement lesions with a qui…

Gynaecologists and the treatment of Garibaldi's ankle wound.

The hero of Italian Unification, Guiseppe Garibaldi, sustained a bullet wound to the ankle that caused an international sensation at the time as various surgeons from Italy, France, Russia and Great Britain vied to manage the injury. In particular the question was whether the bullet was still present. Eventually Nélaton from France won the race by the use of a porcelain-tipped probe which stained with the presence of lead. The actual extraction of the bullet was with the use of an expandable tent. It seems probable that Dr. James Marion Sims, the well-known American gynaecologist, supplied ten…

Acute ankle sprain in a mouse model.

INTRODUCTION: Ankle sprains remain the most common orthopedic pathology. Conducting long-term studies in humans is difficult and costly, so the long-term consequences of an ankle sprain are not entirely known. PURPOSE: The objective of this study is to develop and test a mechanical ankle instability model in mice. METHODS: Thirty male mice (CBA/2J) were randomly placed into one of three groups: the transected calcaneal fibular ligament (CFL) group, the transected anterior talofibular ligament (ATFL)/CFL group, and a SHAM group. Three days after surgery, all of the mice were individually hous…

Arthroscopic synovectomy of the ankle in rheumatoid arthritis.

PURPOSE: To evaluate the outcome of arthroscopic synovectomy of the ankle joint in patients with early-stage rheumatoid arthritis (RA). METHODS: Between 2005 and 2009, 18 consecutive patients with RA involving the ankle underwent arthroscopic synovectomy. Pain was measured using a visual analog scale (VAS), and clinical outcome was determined by calculating the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale score with a mean follow-up of 5 years (60 months). Assessments were performed preoperatively, at 6 and 12 months postoperatively, and then yearly thereafter. Cli…

Orthopaedic problems about the ankle in hemophilia.

Hemophilia is an inherited recessive, sex-linked bleeding disorder. The lack of sufficient coagulation factor VIII produces hemophilia A, and the lack of factor IX causes hemophilia B. The prevention and treatment of the disease requires intravenous infusion of the deficient factor. Hemophilic patients present with multiarticular joint degeneration (hemophilic arthropathy), secondary to recurrent hemarthroses. With the availability of deficient factors, hemophilic patients requiring elective ankle surgery can undergo such surgery with a high expectation of success. A thorough analysis of each …

Complications associated with foot and ankle arthroscopy.

Despite a late start within the realm of arthroscopy, foot and ankle arthroscopy proves to be an important diagnostic and treatment tool for the foot and ankle specialist. As indication for arthroscopy increases, complications associated with foot and ankle arthroscopy must be revisited. We reviewed 405 foot and ankle arthroscopic procedures performed on 390 patients in 4 different facilities over a 3-year period extending from January 2005 to August 2008. Two-hundred-sixty foot and ankle arthroscopic procedures on 251 patients met our inclusion criteria. A total of 246 ankle and 14 posterior …

Ankle injuries and fractures in the obese patient.

Ankle fractures are a common orthopedic injury. Certain ankle injuries have been associated with patient demographics such as obesity and smoking. Obese patients are more prone to severe ankle injuries. Naturally, these injuries affect the lower extremity mobility significantly, which itself is a risk factor for obesity. Although obese patients have increased complications across the board, there are specific techniques that can be used to assure the best possible outcome. The perioperative, surgical, and postoperative considerations as well as the outcomes are discussed in this article.

Dance medicine of the foot and ankle: a review.

All forms of dance are highly demanding activities, with a lifetime injury incidence of up to 90%. Most dance types are stressful, particularly on the dancer's forefoot, but certainly there is no area of the foot or ankle that is exempt from potential injury. Dancers often have unusual difficulties related to the dynamic biomechanical forces required by their individual dance form. A thorough understanding of these movements guides the physician to the cause of the injury, particularly in understanding specific overuse injuries. This article discusses biomechanics of the foot and the imaging a…
