Acute Deltoid Ligament Repair in Ankle Fractures: Five-year Follow-up.

Direct repair of deep deltoid ruptures after traumatic ankle fracture is not commonly performed. Previous studies overlook the contributions of the medial deltoid to overall ankle stability and long-term patient satisfaction. Historically, deep deltoid injuries have been addressed indirectly through syndesmotic ligament repair. This technique fails to restore, however, the anatomic function of the primary medial stabilizing structure. The oversight of direct deltoid repair may be one contributing factor to the less than optimal outcomes after ankle fractures with syndesmotic injuries. This art

Trauma of the pediatric ankle and foot.

This article presents a brief overview of the injuries to the ankle and foot encountered in children and adolescents. Trauma to the ankle or foot may result from acute, chronic, or repetitive forces. The role of the different imaging modalities in the assessment of ankle and foot trauma in the growing patient is discussed. Plain radiographs remain the mainstay in the diagnosis of most acute traumas, whereas CT may be helpful to unravel the complex anatomy of certain fractures like the triplane or juvenile Tillaux fracture. In the evaluation of chronic injuries, including osteochondrosis dissec

The influence of shaft stiffness on joint kinematics and kinetics during hiking.

Hiking boots provide an interface for walking in challenging environments, typically equipped with a shaft to provide ankle joint stability in rough terrains. Currently it is unclear if the ankle joint is stabilized to an extent that protects against ankle injuries, and if so, to what degree this added ankle stability sacrifices ankle mobility and hence decreases efficient gait propulsion. The aim of the present study was to compare the effect of shaft construction and stiffness on lower extremity kinematics and kinetics during level and step-down walking to simulate hiking conditions. Thirtee…

Gait study of surgical treatment and conservative treatment of acute ankle sprain: a single-center cohort study.

OBJECTIVES: Although there are different ways, the choice of treatment for grade-I-II anterior talofibular ligament (ATFL) injuries caused by acute ankle sprain is still controversial. The primary purpose of our study was to compare the efficacies of modified Broström-Gould surgery and conservative treatment in grade-I-II ATFL injuries caused by acute ankle sprain. The secondary goal was to assess the plantar pressure characteristics of ankle sprain. METHODS: Between Sep 2022 and December 2023, 63 patients diagnosed with acute ankle sprain (ATFL grade I-II injuries) were prospectively include

Peroneal tendon injuries.

Peroneal tendon pathology is a common cause of lateral ankle pain. Because of the difficulty of distinguishing peroneal tendon injuries from lateral ligament injuries in the setting of ankle trauma, early diagnosis is often missed. Failure to recognize and adequately treat these injuries can result in significant long-term disability. Focused history and physical examination are critical, as is a thorough understanding of the local anatomy and biomechanics involved. Acute injuries may be successfully treated conservatively; however, chronic injuries have demonstrated unsatisfactory response to

Assessment and management of adult ankle fractures: understanding the evidence.

Ankle fractures are a common injury. Assessment should include looking at the mechanism of injury, comorbidities, associated injuries, soft tissue status and neurovascular status. Emergent reduction is required for clinically deformed ankles. Investigations should include plain radiographs and a computed tomography scan for more complex injuries or those with posterior malleolus involvement. An assessment of ankle stability determines treatment, taking into account comorbidities and preoperative mobility which need special consideration. Non-operative management includes splint or cast, allowi

Incidence and Characteristics of Midfoot Injuries.

BACKGROUND:: The epidemiology of midfoot injuries is poorly known. It has been estimated that the incidence of Lisfranc injuries (intra-articular injury in the tarsometatarsal joint) is 1/55 000 person-years and the incidence of Chopart injuries (intra-articular injury in the talonavicular and calcaneocuboidal joint) 4/100 000 person-years. The purpose of our study was to assess the computed tomography (CT) imaging-based incidence (per 100 000 person-years) and trauma mechanisms of midfoot injuries. METHODS:: All CT studies performed due to acute injury of the foot and ankle region between Ja

Foot and Ankle Complications after Tibial and Fibular Shaft Fractures.

The prevalence of foot and ankle injuries associated with tibial and fibular shaft fractures is estimated to be 25%. A myriad of injury mechanisms and surrounding soft tissue damage related to this type of fracture can lead to distal complications and sequelae in the foot and ankle. Early recognition of concomitant acute injuries and adequate management of delayed complications can optimize the functional outcomes and quality of life in most patients..

Malleolar fractures of the ankle. A comprehensive review.

Ankle injuries and associated malleolar fractures are common. The outcome of treatment depends on the treating physician's ability to properly identify the mechanism of injury and subsequently restore ankle function by accurate realignment of the normal ankle articulations. Particular attention must be paid to restoring the length and alignment of the fibula because it plays a major role in stabilizing the talar ankle component. The medial aspect of the ankle also is important. The vital role of the deltoid has been proved in many studies, and although controversy exists in the proper treatmen…

Diagnostic Value of Ultrasound in Ankle Sprain.

The present study aimed to evaluate the sensitivity and specificity of clinical tests and ultrasonography in detecting ankle ligament injuri … or sprained ligament. Ultrasonography is an effective complementary tool for primary evaluation of ankle injuries, which leads to early diagnosis and efficient quality of care. Clinical tests are not reliable to rule out the ankle ligaments injury and the results should be interpreted with caution.

Medial and lateral malleolar arteries in ankle arthroscopy: a cadaver study.

Neurovascular injury may occur during ankle arthroscopy. The majority of complications are neurological injuries; however, vascular injuries do exist. Neurovascular structures are especially vulnerable during portal placement and debridement of anterior structures. Routine anteromedial and anterolateral portals are generally accepted to be safe; this is different from the anterocentral portal, which is associated with a higher risk of injury. However, injuries may occur in these relatively safe portals. The purpose of this cadaver study was to examine other relatively minor neurovascular struc
