[Emergency situations in drug addicts. Classification, diagnostic and therapeutic measures].

In drug addicts emergency situations can occur which differ only slightly in symptoms but require different therapeutic measures, depending on type of the drug and symptomatology. Diagnostic and therapeutical difficulties in emergency situations, especially the not unfrequent late complications and the considerable danger of suicidal actions require in general the immediate hospitalization. Intensive care and surveillance are necessary. The adequate therapy of ventilatory and circulatory functions is of great importance and should be started by the first consulted physician before transport to

Administration of obidoxime tablets to man. Plasma levels and side reactions.

Twenty-four male volunteers were given obidoxime tablets in quantities ranging from 1.84-3.58 g in a single dose, or 7.36 g divided into 4 equal doses. With the lowest dose, average peak plasma level of the drug was 1.9 mug/ml and after the highest single dose it was 5.6 mug/ml, both attained 1.5 h after administration. In the multiple-dosed individuals, plasma levels of the oxime increased gradually following each additional dose, reaching a peak of 3.5 mug/ml after the last dose. Thirteen individuals complained of one or more of the following side effects: pallor, nausea, pyrosis, headache,

[Fatal arsenic trichloride poisoning (author's transl)].

Report on a 24-year-old chemical engineer who committed suicide by means of arsenic trichloride. The clinical course and the autopsy findings are discussed briefly. The results of the toxicological examination leave no doubt as to an acute arsenic poisoning. In addition to this, extreme corrosion was found, especially in the region of the oesophagus. If an anamnesis on this patient had not been present one would probably have been satisfied with a diagnosis of acid poisoning and the possibility of intoxication by arsenic would not have been taken into consideration. This case indicates that es

A practical approach to managing agitated toxidromes.

Agitated patients may present with life-threatening emergency. Differential diagnoses span a wide range, conveniently remembered by the acronym VITAMINS (Vascular, Infections, Toxins/Trauma, Auto-immune/-inflammatory, Metabolic, Iatrogenic, Neoplastic, and pSychiatric aetiology). Common toxidromes include anticholinergic syndrome, serotonin toxicity, sympathomimetic toxicity, neuroleptic malignant syndrome, and withdrawal from alcohol or sedative-hypnotics. Diagnosis is primarily clinical, based on exposure history and characteristic signs. A focused physical examination is often sufficient wh

Hyperglycemia as an atypical finding of malignant hyperthermia: A case report and literature review.

Malignant hyperthermia (MH) is a rare but life-threatening disorder induced by volatile anesthetics or depolarizing muscle relaxants. Studies have reported the increased prevalence of hyperglycemia among patients susceptible to MH. Herein, we present the case of a 13-month-old toddler who had successfully undergone dextro-transposition of the great arteries and required main and bilateral pulmonary artery augmentation because of residual pulmonary atresia. The patient exhibited atypical manifestations of MH including hyperglycemia with an inadequate insulin response during the perioperative pe

Interesting Case of Celphos Poisoning with G6PD Deficiency: Is There a Correlation?

Celphos poisoning is one of the common poisonings in agricultural countries such as India, mainly in rural areas. Its clinical manifestations vary depending on the amount of poison consumed and the time since ingestion. It has a very high fatality rate. As of now, there is no specific antidote available for this poisoning. Its management includes intensive monitoring along with immediate initiation of supportive care. It is associated with multisystem involvement with multiorgan failure in severe cases, but hematological complications are not very common. Here, we present a successfully treate

Shades of Blue: A Case Series of Acquired Methemoglobinemia.

Acquired methemoglobinemia (MetHb) is a rare but potentially life-threatening condition that has varied etiology, usually toxin- or drug-induced. We had five cases of acquired methemoglobinemia during six months. Their presentation varied from an asymptomatic state to respiratory distress. The presence of cyanosis and low oxygen saturation (SpO2), despite normal partial pressure of oxygen (PaO2) and chocolate brown-colored blood, were diagnostic clues present in all cases. A high level of methemoglobinemia was detected on arterial blood gas (ABG), confirming the diagnosis. Methylene blue was u

Too much of a good thing: Bicarbonate toxicity following treatment of sodium channel blocker overdose.

Although sodium bicarbonate can be a life-saving antidote for patients with overdoses resulting in sodium channel blockade, there has been a concerning rise in cases referred to the Poisons Information Centre where inappropriately large doses of bicarbonate have been used resulting in iatrogenic harm. We present a series of three clinical cases where excessive bicarbonate was used to treat poisonings and discuss our approach to managing cardiotoxicity secondary to sodium channel blockade. Serial blood gas analysis should be performed when using bicarbonate to ensure pH targets are met and seve

Cardiogenic shock due to yew poisoning rescued by VA-ECMO: case report and literature review.

Ingestion of leaves of the European yew tree (Taxus baccata) can result in fatal cardiac arrhythmias and acute cardiogenic shock. This cardiotoxicity derives from taxine alkaloids that block cardiac voltage-gated sodium and calcium channels. Prompt initiation of venoarterial extracorporeal membrane oxygenation is essential to bridge these critically ill patients to recovery, as there is no antidote available. We here report a 39-year old patient with toxic cardiogenic shock after yew poisoning, who was successfully rescued by venoarterial extracorporeal membrane oxygenation and had a full neur

Plasmapheresis in lethal yellow phosphorus poisoning: a scope for recovery.

A 16-year-old girl presented with grade 3 hepatic encephalopathy (HE) following suicide attempt after consuming a lethal dose of yellow phosphorus containing rodenticide. Although she was a candidate for liver transplantation, it could not be done. In the absence of a specific antidote for yellow phosphorus poisoning, the patient was managed conservatively. In addition, low volume-therapeutic plasma exchange (LV-TPE) was initiated, which resulted in a dramatic improvement in HE. Although liver transplantation is the definitive treatment, this case has shown that TPE has a promising role as a '

Neuronal delivery of antibodies has therapeutic effects in animal models of botulism.

Botulism is caused by a potent neurotoxin that blocks neuromuscular transmission, resulting in death by asphyxiation. Currently, the therapeutic options are limited and there is no antidote. Here, we harness the structural and trafficking properties of an atoxic derivative of botulinum neurotoxin (BoNT) to transport a function-blocking single-domain antibody into the neuronal cytosol where it can inhibit BoNT serotype A (BoNT/A1) molecular toxicity. Post-symptomatic treatment relieved toxic signs of botulism and rescued mice, guinea pigs, and nonhuman primates after lethal BoNT/A1 challenge. T

Ponatinib and other CML Tyrosine Kinase Inhibitors in Thrombosis.

Abl1 kinase has important biological roles. The Bcr-Abl1 fusion protein creates undesired kinase activity and is pathogenic in 95% of chronic myeloid leukemia (CML) and 30% of acute lymphoblastic leukemia (ALL) patients. Targeted therapies to these diseases are tyrosine kinase inhibitors. The extent of a tyrosine kinase inhibitor's targets determines the degree of biologic effects of the agent that may influence the well-being of the patient. This fact is especially true with tyrosine kinase inhibitor effects on the cardiovascular system. Thirty-one percent of ponatinib-treated patients, the t

[Anticoagulation-direct oral anticoagulants].

Since direct oral anticoagulants (DOAC) have become available, use of anticoagulant treatment has become easier and safer-for patients suffering from thromboembolic diseases as well as for patients with atrial fibrillation: Because of constant bioavailability, fixed dose regimen treatment is possible, monitoring not necessary and severe bleeding complications-particularly intracranial hemorrhages-rare in comparison to vitamin K anticoagulants. To gain all these advantages, it is essential to give DOAC in the correct dosage. Dose reduction of single DOAC has to be considered depending on underl

[MANAGEMENT OF ANTICOAGULANT AND ANTIPLATELET THERAPY IN PATIENTS UNDERGOING PLANNED ENDOSCOPIC PROCEDURES].

Antithrombotic and antiplatelet therapy is widely used for primary and secondary prevention of venous and arterial diseases. Hemorrhage is a frequent complication in patients taking these drugs, especially during and post invasive procedures. There are several oral anticoagulants and antiplatelet drugs that differ from each other in the mechanism of action and metabolism, the need for drug monitoring, antidote, and the peri-procedural management of the drug. The risk of bleeding in gastrointestinal endoscopic procedures can be high (≥ 1.5%), depending on the procedure. Patients taking antithro

Clinical Update: The Risk of Opioid Toxicity and Naloxone Use in Operational K9s.

The increasing use of opioids (e.g., fentanyl, carfentanil) for illicit drug manufacturing poses a potential life-threatening hazard to law enforcement officers and first responders (e.g., EMS, fire and rescue) who may unknowingly come into contact with these drugs during the course of their daily activities. Similarly, Operational canines (OpK9s) of all disciplines-detection (drug, explosive, accelerant), patrol, tracking, search and rescue, and others-are at risk for accidental illicit opioid exposure. The most serious adverse effect of opioid exposure is respiratory depression leading to sl

Adolescent with prolonged toxidrome.

A 13-year-old female was presented to the emergency department following an intentional ingestion. The patient developed significant toxicity including multiple, discreet tonic-clonic seizures. Despite appropriate resuscitation and antidotal management, the patient's symptoms persisted for more than 36 hours post-ingestion. An upright abdominal radiograph was performed revealing a radiopacity suggesting a pharmacobezoar. An esophagogastroduodenoscopy was performed with successful removal of a tennis ball-sized pharmacobezoar. The patient's symptoms subsequently subsided and she recovered fully
