Between 1974 and 1984, we performed 106 ciliary body excisions in a total of 90 eyes. Of the 85 eyes for which at least six months of follow-up data were available, 72 had secondary partial angle-closure glaucoma with aphakia, ten had secondary partial angle-closure glaucoma without aphakia, and three had secondary open-angle glaucoma. The clinical course and visual, refractive, and keratometric results of a consecutive series of epikeratophakia procedures carried out by the author are presented. Indications for the procedure included keratoconus and adult and paediatric aphakia. Follow up time ranged from two to fourteen months. The first five patients operated on received commercially obtained cryolathed lenticules. The final three cases received lenticules which were lathed by the author at room temperature using a recently developed technique.

Scleral fixation ofsulcus intraocular lenses is performed by most surgeons to correct aphakia in eyes with insufficient capsular support in which intraocular lenses cannot be implanted into the bag or sulcus. The implantation of foldable lenses maintains the advantages of small incision surgery, working with a closed system, less astigmatism, faster wound healing, and early visual rehabilitation. The authors describe a technique for using a single-piece hydrophilic foldable acrylic lens with no eyelets for scleral fixation.

Midtrimester amniocentesis has rarely been reported to cause ocular damage. Findings in the literature include corneal perforation, a vascularized corneal leukoma, a non pigmented epithelial cyst of the anterior chamber, eyelid coloboma, microphthalmos, retinal hole, axial myopia and retinal detachment. We describe the case of a paralimbal scleral perforation with iris prolapse, distortion of the pupil, aphakia due to resorption of the lens and a chorioretinal scar possibly caused by an amniocentesis needle.

Fifteen patients with diplopia associated with prior scleral buckling, prior penetrating keratoplasty, severe corneal scarring, monocular aphakia, long-standing strabismus, or prior vertical offsets of the horizontal recti were thought to have disruption of fusion because diplopia could not be eliminated with prisms. They were each found to have a substantial symptomatic cyclotropia unassociated with an obvious dysfunction of an oblique muscle. In 13 patients, diplopia resolved after the cyclotropia was corrected surgically.

The first 46 patients to undergo surgery for squint with adjustable sutures at Moorfields Eye Hospital, City Road, London, are presented. The indications for surgery were: dysthyroid ophthalmopathy, fourth nerve palsy, monocular aphakia with strabismus and miscellaneous conditions. The technique of surgery is briefly described. The results show a high incidence of alignment, indicating the value of the technique and this is supported by long-term follow-up data where available. Treatment failures are analysed in detail.

Scheimpflug photography gives increased depth of field, enabling a sharp optical section through the whole anterior segment of the eye to be obtained without leaving any structures out of focus. With the Topcon SL-45 the Scheimpflug optical section can be reproduced exactly. It is the method of choice for observing lens opacities. In addition, Scheimpflug photography offers an improved method of monitoring natural courses and/or the effects of treatment in, e.g., keratoconus, lenticonus, aphakia and pseudophakia.

To correct third order (primary) oblique astigmatism of thin spectacle lenses outside an approximate lens power range of -22 D to +7 D, one surface must be aspherized. Aspherizing is of particular importance in the high positive power range used for the correction of aphakia. Graphs are presented showing the required front surface asphericities and power drops required to correct oblique astigmatism in this range. The results are a guide to the required asphericities in thick lenses.

Sixty eyes of 58 patients who underwent scleral buckling procedures were prospectively evaluated with postoperative fluorescein angiograms. Forty-three percent of these eyes showed angiographic cystoid macular edema (CME). Aphakic patients showed a 64% incidence of cystect the final visual acuity of aphakic patients but significantly reduced the visual recovery in the phakic group. The incidence of CME was not related to preoperative foveal detachment, duration of aphakia, duration of detachment, surgical approach, or refraction.

Hopeless glaucoma cases fluctuate between excessive pressures and phthisis. Due to the underlying disease (neovascularization in the chamber angle, chronic inflammation, dysgenesis, aphakia) the customary surgical pressure-lowering operations often fail and medication is also useless. As a "last-ditch" measure, there remains only destruction of the ciliary body. In the Zurich eye department, noninvasive cyclophotocoagulation has been performed since 1987. The present paper reports on the first cases with long-term follow-up.

We present a new intraocular lens (IOL) fixation technique that reduces the presence of intraocular knots and eliminates the need for scleral flaps while attaining excellent central positioning of the IOL. It also provides rapid visual rehabilitation from the insertion of a foldable IOL through a small corneal incision. This single-loop suture technique is an easy, convenient, and practical procedure, with minimum complications in the treatment of aphakia and the management of cataract surgery in eyes without capsular support.

The authors describe two brothers with Marfan syndrome who developed severe complications after iris-suture fixation of an intraocular lens (IOL). A secondary iris cyst developed in one and the IOL dislocated into the vitreous in the other, requiring pars plana vitrectomy and IOL removal. Although the visual outcome was good in both cases, severe complications of iris-suture fixated IOLs must be considered when surgical correction of aphakia is attempted in children with Marfan syndrome.

Ten cases of combined strabismus surgery and intraocular lens implantation were reviewed to determine if there were increased risks and complications. A review of these cases revealed the contrary was true; i.e., there were no complications arising from the combined surgery and there were benefits in the decreased hospitalization and operative time, less mental trauma, and improved muscle surgical outcome. From these results, we conclude that combined intraocular lens and strabismus surgery is efficacious and indicated in patients with strabismus and cataracts or aphakia.
